- The Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program, know in Minnesota as the Child and Teen Checkups (C&TC) Program, is a benefit for Medical Assistance and Minnesota Care. C&TC provides comprehensive health care for children and teens, newborn through the age of 20, who are enrolled in Medical Assistance (MA) or MinnesotaCare - The goal of the C&TC Program is to improve the health of eligible children and teens and reduce the negative impact of health problems. Medical care costs can be reduced by providing preventive medical care and early diagnosis and treatment of conditions that threaten children's health - Public Health provides outreach through letters and phone calls to new and current participants in the C&TC program

Includes: Breastfeeding Information and Support: - Public Health Nurses, WIC Lactation Consultants and Peer Breastfeeding Counselors are available to answer questions or concerns regarding breastfeeding. WIC participants should contact (952)563-8993; Non-WIC participants should contact (952)563-8900; or email Help Me Grow South Hennepin: - Interagency review team - Early intervention for children ages birth to 5 - Testing available for developmental delays in speech/language, motor, cognitive or social/behavior problems Follow Along: - Child development and tracking program for children up to age 3 Health Screenings: - Blood pressure screenings, 12:30 pm - 3:30 pm Monday - Friday at the Public Health Center; also offered at Creekside Community Center, Edina Senior Center and Richfield Community Center - Hearing screenings offered at Creekside Community Center (952)563-4944 and Edina Senior Center (952)833-9570; call for specific dates and times - Tuberculin Skin Test (TB testing) offered at the Public Health Center, 11:30 am - 3:30 pm Monday, Tuesday, Wednesday, and Friday. Cost is $35. Immunizations and Flu Shots: Offers immunizations to individuals who are uninsured or underinsured through the MN Vaccines for Children (MNVFC) and the Uninsured and Underinsured Adult Vaccines (UUAV) programs. Immunization clinics are offered the first and third Tuesday of the month, 3 pm - 5:30 pm. No appointment necessary. A $20 donation requested per vaccination, however, no one will be turned away. Public Health Nurse services: - Home visits for prenatal, postpartum and parenting - Services for adolescents, pregnant teens, and teen parents; education, nursing services, and health counseling provided in the home and at community sites - Education, counseling, and support for pregnant mothers, new parents, and families - Resource for health care questions, food concerns, child growth, development, and safe living conditions WIC Supplemental Nutrition Program: - For infants and children to age 5 and pregnant, postpartum, and breastfeeding women - Nutrition education/food voucher program Senior Health: - Home-based senior services; public health nurses provide home visits at no cost - Assistance finding resources - Community-based senior services; Talk With the Nurse provides group health topics, blood pressure check, and one-to-one help for health questions - Home safety assessments

Partners with families to promote healthy lifestyles and nurturing environments. Child and Family Health Services support families with the following services: - Breastfeeding classes and resources: breastfeeding support services are available in home, at the Horizon Public Health Office Alexandria main site, and via telephone. - Child and Teen Checkups (C&TC): complete head-to-toe health and development checkups for persons under 21 years old. - Child passenger safety: specialists can answer questions regarding child safety seats and check your car seats. - Family Home Visiting: registered nurses provide a variety of home visiting programs including prenatal, postpartum and ongoing parenting support services. - Follow Along Program: for parents of children ages birth - 5 at no cost to learn if your child is talking, moving, playing and developing on track for his/her age.
