- Group homes - In-home support services for individuals who live in a home of their own, but still need some assistance. Individuals receive 2 - 6 hours of assistance each week, as determined by the Olmsted County Community Services Department. Some skills include: Budgeting, cleaning, cooking, managing medication, using public transportation and other skills are provided as needed. - Semi-Independent Living Skills Program (SILS) for individuals who live in their own home, or their family's home. Individuals receive 5 - 10 hours of assistance each week, as determined by the Olmsted County Community Services Department. Some skills include: Budgeting, cleaning, cooking, managing medication, using public transportation and other skills are provided as needed.

Basic Living and Social Skills (such as household management, budgeting and shopping, relapse prevention, crisis assistance, medication monitoring, healthy lifestyles, etc.) designed to restore a client's skills necessary for managing their illness allowing individuals to live independently in the community. Community Intervention assists in facilitating changes which will allow an individual to function more independently. Community Intervention may involved an agency, employer, and the client's family. Available in person or by telephone. Medication Education: educates clients about mental illness and symptoms, the role of the medications and the their possible side effects. The services are specifically intended to enable a recipient to develop, retain, and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living. Community based ARMHS services are tailored to the client in regard to the hours and days that services are delivered.

Rehabilitative mental health services that enable the recipient to develop and enhance psychiatric stability and social competencies, when these abilities are impaired by the symptoms of mental illness. ARMHS are also appropriate when provided to enable a recipient to retain stability and functioning, if the recipient would be at risk of significant functional decompensation or more restrictive service settings without these services. Adult rehabilitative mental health services instruct, assist, and support the recipient in areas such as: - Interpersonal communications skills - Community resource utilization and integration skills - Crisis assistance - Relapse prevention skills - Health care directives - Budgeting and shopping skills - Healthy lifestyle skills and practices - Cooking and nutrition skills - Transportation skills - Medication education and monitoring - Mental illness symptom management skills - Household management skills - Employment-social skills - Transition to community living services Basic living and social skills is a separately billable service. These services may be provided to a recipient on a one-to-one basis in the recipient's home or another community setting or in groups.

Program serves meals for clients with chewing problems, choking issues, diabetes, food allergies, and cholesterol concerns. - Medical monitoring and administration services in tandem with health care providers and client residential facilities - Transportation to the Regional Fitness Center for work creating and meeting fitness goals, most clients qualify for free memberships to the center Training in independent living skills and speech therapy are provided when necessary The DAC also provides job training and employment in various sub-contracted jobs

Independent living in apartments for 16 - 21 year olds - Housing and independent living support services for homeless youth, ages 16 - 21. Please note that 16- and 17-year-olds need parent or guardian permission. We can help you negotiate with your parent or guardian if we think independent housing is the best option for you. - Housing and independent living support services for teen parents, ages 16 - 21, who are homeless or living in inadequate housing. We can help you find an apartment, complete the application, pay security deposit, and help with monthly rent. - Case management services to help you with education, employment, parenting, child care, and more. We can also help you with obtaining medical insurance and accessing medical, dental, or mental health services. - All youth participating in transitional housing have access to: Individual, and/or family counseling appropriate to the individual needs of single teens and teen parents. - Skill building workshops for single teens and teen parents to increase self-sufficiency and independence.

A safe place where counselors, therapists, and doctors create a therapeutic living environment to help students who reside in this setting gain the skills they need to succeed in their home, community, and school. Residential care and treatment for boys and girls ages 10-17 in a non-secure setting. There are 20 beds in the Cottage. Youth are referred to the Cottage by County Social Workers, County Probation Officers, Tribal Social Workers, Parents, self-referrals, or the courts. On-site educational programming is provided by Independent School District 318. Each student will earn credit for school attendance during placement at the North Homes Cottage. The following areas are addressed in each resident's plan: - Social/Interpersonal Skills - Emotional Control - Vocational Skills - Independent Living Skills - DBT and skills Students in the Residential Program participate in various program activities and receive services including, but not limited to: - Daily therapeutic/psycho-educational group discussions covering such topics as communication skills, emotions, anger management, gender issues, CD information, stress management, depression and conflict resolution - Individual counseling - Psychiatric evaluation and monitoring - Structured daily recreational/leisure activities - Structured daily schedule addressing daily living skill development - Cultural education, activities and advocacy - Community professional mental health services - On site school programming - Coordination with community resources and agencies - On site health screening and care - Spirituality services Treatment will have individual goals for everyone to complete.
