Basic and In Home Support Services - Affinity Caregiver

Provides basic and in home support services for older adults and people with disabilities in their homes or in a community setting to ensure the health and safety of the person and maintain their independence within the community. Basic Support Services - Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision, or physical assistance to remain in their own homes. - 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency. - Companion services: Non-medical care, supervision and socialization to a person age 18 or older. - Night supervision: Provides overnight assistance and monitoring by an awake staff in the person's home. - Respite care services: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care. Intensive Support Services - Positive support services: Services to increase positive behavior and decrease or eliminate severe, challenging behavior. - Specialist services: Services designed to promote staff and caregiver competency to meet a person's needs in eligible areas. - Independent living skills training: Services that develop, maintain and improve the community-living skills of a person. - Semi-independent living skills: Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community. - Residential-based habilitation: Including in-home family support and supported living services for adults in their own home: Supported living services provided in a licensed site, including a foster care home, a community residential setting, or a supervised living facility.

Autism Camps - Hormel Historic Home

Offers a variety of day and weekly camps for children with Autism Spectrum Disorder: - Circle of Friends Day Camp offers 5 days of educational programming for children with Autism Spectrum Disorder (ASD) entering 1st through 5th grades. - All Access Community Explorations Camp (AACE) is a 10 session educational camp that focuses on helping participants learn new skills and use them in new situations. This camp is to address the needs of young adults with Autism Spectrum Disorder (ASD) entering 6th through 12th grades. - Camp Just for Me is a five session educational and fun experience for children on the Autism Spectrum who require more individualized attention. The camp serves children and young adults ages 7 - 17 who benefit from 1 on 1 staffing. - Little Learners Camp is three partial days of social and educational programming for children with Autism Spectrum Disorder (ASD) 3 years of age through entering Kindergarten.

Emphasizes skills necessary for independent living with the greatest emphasis on vocational training and job skills. The majority of clients are involved in supported community employment. Job placements include: - Janitorial work - Automobile detailing - Nursing home assistant - Laundry work - Lawn care - Dish washing - Recycling services - Retail crafts - Food service industry The DAC operates the following: - Recycling center - Ink pen and pin back button assembly station - Retail craft industry - Non-vocational programs - Daily hot lunches, meals are prepared according to individual client dietary needs. Program serves meals for clients with chewing problems, choking issues, diabetes, food allergies, and cholesterol concerns. - Medical monitoring and administration services in tandem with health care providers and client residential facilities - Transportation to the Regional Fitness Center for work creating and meeting fitness goals, most clients qualify for free memberships to the center Training in independent living skills and speech therapy are provided when necessary The DAC also provides job training and employment in various sub-contracted jobs

Children's Services - Solutions Behavioral Healthcare Professionals - Fergus Falls

Behavioral Intervention: Combines daily living skills, interpersonal skills, and behavioral management skills into an individualized plan to enable the child to function. Assertive Community Treatment (ACT): ACT is an evidence-based treatment for adults who have a severe and persistent mental health illness. Based on severity of symptoms and impairments, clients work with the team on a long term basis. Adult Rehabilitation Mental Health Services (ARMHS): Adult mental health services that enable individuals to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills. Behavior Health Home: Provides individuals with their care needs. The team work closely with the individual, their supporters and providers to ensure comprehensive quality of care. Community Support Program (CSP): The Community Support Program is a coordinated care and treatment program which provides a wide range of individualized treatment, rehabilitation and support services to persons with serious and persistent mental illness. The purpose of the community support program is to assist individuals to live as successfully and independently as possible in their own communities by supporting independent living and coping skills.
