Children and Family Programs - Horizon Public Health
Partners with families to promote healthy lifestyles and nurturing environments. Child and Family Health Services support families with the following services:
- Breastfeeding classes and resources: breastfeeding support services are available in home, at the Horizon Public Health Office Alexandria main site, and via telephone.
- Child and Teen Checkups (C&TC): complete head-to-toe health and development checkups for persons under 21 years old.
- Child passenger safety: specialists can answer questions regarding child safety seats and check your car seats.
- Family Home Visiting: registered nurses provide a variety of home visiting programs including prenatal, postpartum and ongoing parenting support services.

21st Century Community Learning Centers: In partnership with the Minnesota Department of Education, HAP works with Community School of Excellence (CSE) and Prairie Seeds Academy (PSA) to after school academic and enrichment activities that include: the arts, music, crafts, leadership and personal develeopment, STEM classes, career exploration, and additional services.
Ascend: Works with victims and at-risk youth to address the severe harm caused by their explotation and move them towards recovery and independence
Hmoob Toj Sjab: A Montesorri school in which children learn all the foundation necessities for survival in their enviroment. Children learn all the basic building blocks that make up the world within and around them. They build awareness in identifying colors, shapes, textures, and dimensions; accomplish simple chores; develop manners such as grace and courtesy; learn to care for themselves and their enviroment; and build reading, writing, and math skills.
In-Home Parenting: In-home parenting services and support to families referred by Ramsey County.
Learn and Live: Health Education Programs: Education and outreach to the Hmong Community for breast and cervical cancer awareness and screening.
Pathways to Eliminate Addiction: Adolescent and Adult Chemical Health Support program for Hmong individuals. The goal of the program is to reduce drug and alcohol use in the Hmong community by working with individuals to achieve or sustain abstinence, improve health, wellness and a quality of life.
Project Grow: After-school program for ages 14 - 21, training students in exploring leadership, agriculture, college and career, and conservation.
Saint Paul Promise Neighborhood Hmong Navigator: A HAP family navigator located at Jackson Elementary works at the school's Family Center to help connect low income families with minor children to community resources.
Transcend: Crises intervention, social supports, safety planning, and legal advocacy for Hmong and Karen women who are experiencing domestic violence.
YouthLEAD: Career education and paid internships for SE Asians ages 16 - 24. Must reside in Ramsey County.

- Home visits are offered to pregnant women, infants, and children to provide ongoing support.
Maternal Child Health Services
- All Becker County families with a newborn are contacted by a Community Health Nurse. A home visit will be offered to provide new parents with information on feeding choices, newborn cares and additional support.
Parental, Childbirth, and Newborn Education
- Educational support is offered to all residents of Becker County. Material presented is tailored to meet the specific needs including labor and delivery, breastfeeding, bottle feeding and newborn/postpartum care.
Breastfeeding Resources
- Becker County WIC office sponsors a Breastfeeding Support Group that meets monthly. Online Breastfeeding Support Group.
Car Seat Program
- Medica and Blue Plus have a car seat program for children up to 8 years of age. Education and installation of the car seat are offered to any resident of Becker County.
Child and Teen Checkups (C&TC)
- Child and Teen Checkups are offered to any Becker County resident under the age of 21. Checkups include: hearing and vision screening, developmental screening, physical exam, lead screening, immunizations and referrals to other health care providers as necessary.

- Infant, child and adult immunizations can be done at Becker County Community Health by appointment.
Follow Along Program
- Consists of a series of developmental questionnaires the parent completes and returns to Becker County Community Health. The questionnaire is scored and a letter is sent to the parent with the results. This program is free to all children in Becker County under the age of 36 months.
