Targeted Case Management: Provides person-centered case management to individuals who are diagnosed with Serious and Persistent Mental Illness who are residents of Ramsey County. Case Managers help link individuals to services, which include mental health, chemical health, medical, social, vocational, legal, educational, and recreational services. Community Care Services Team (CCST): Specializes in helping individuals with mental health and substance use disorders returning to, and remaining in, community settings after extended or frequent stays in inpatient hospitals or other similar settings. Coordinates with the treatment team to provide services for individuals, supporting the development, writing, implementation, evaluation, and revision of overall treatment goals and plans in collaboration with the individual served. CCST coordinates services with other county or community agencies including but not limited to local community hospitals, mental health case management, treatment providers, and primary care.

Assertive Community Treatment (ACT): ACT is an evidence-based treatment for adults who have a severe and persistent mental health illness. Based on severity of symptoms and impairments, clients work with the team on a long term basis. Adult Rehabilitation Mental Health Services (ARMHS): Adult mental health services that enable individuals to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills. Behavior Health Home: Provides individuals with their care needs. The team work closely with the individual, their supporters and providers to ensure comprehensive quality of care. Community Support Program (CSP): The Community Support Program is a coordinated care and treatment program which provides a wide range of individualized treatment, rehabilitation and support services to persons with serious and persistent mental illness. The purpose of the community support program is to assist individuals to live as successfully and independently as possible in their own communities by supporting independent living and coping skills. Targeted Case Management: Helps adults with serious and persistent mental illness (SPMI) gain access to needed mental, medical, social, educational, vocational and other necessary services as it relates to the person's overall health needs.

Provides mental health services to children to support and improve children's social skills, emotional development, and well-being. Common children's mental health issues involve: - Engaging in risky or self-injurious behaviors - Feeling sad or worthless - Inability to cope with significant losses, traumas, major life events, and changes - Inability to control anger or aggression - Inability to manage worries, troubling thoughts, or feelings - Other difficulties related to a child's mood, behavior, thoughts, or relationships - Problems getting along with peers or adults Services include: - Assessments for mental health - Case management and ongoing support - Day treatment - Individual and family therapy - In-home counseling and skills training

Provides services to children with severe emotional disturbance. Services include: - Children's Mental Health Targeted Case Management (CM-TCM): Assist children and families in obtaining needed services, improve functioning in education, interpersonal relationships, family relationships, family functioning, and enhance daily living skills - Children's Therapeutic Support Services (CTSS): Provides in-home, school-based, or community setting for counseling services. The program provides a safe, convenient, and nurturing environment for families and children to prevent out-of-home placement.

Children's mental health residential treatment is a 24-hour program under the clinical supervision of a mental health professional, in a community setting, other than an acute care hospital or regional treatment center inpatient unit. This benefit is available for all eligible children placed in residential treatment by counties for whom the service is medically necessary.

Rehabilitative mental health services that enable the recipient to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills, when these abilities are impaired by the symptoms of mental illness. ARMHS are also appropriate when provided to enable a recipient to retain stability and functioning, if the recipient would be at risk of significant functional decompensation or more restrictive service settings without these services. - Adult rehabilitative mental health services instruct, assist, and support the recipient in areas such as: interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment-related skills, and transition to community living services. Basic living and social skills is a separately billable service as defined below. - These services may be provided to a recipient on a one-to-one basis in the recipient's home or another community setting or in groups.

Behavioral Intervention: Combines daily living skills, interpersonal skills and behavioral management skills into an individualized plan to enable the child to function. Helps the child learn and apply appropriate skills in various living settings, for the child to remain living at home. Case Management: Helps locate necessary services, make referrals as needed, and monitor the delivery of the child's mental health services. May also assist the family with support services, such as respite care. Community-Based Services: Rehabilitative services for children and their families to address mental health and behavioral needs. Provides therapeutic support services are provided in the child's home, school, daycare, foster home, relative's home or other indicated community setting. Crisis Intervention: 24-hour on-call services are available upon arrangement for crisis management. Expertise in working with co-occurring concerns, trauma, SPMI, mood disorders, and complex barriers such as homelessness and unemployment. Target Mental Health Case Management (TCM) and Intensive Case Management (ICM): Offers long term (voluntary and court ordered) case management programs provided to people with severe and persistent mental illness (SPMI).
