Targeted Case Management: Provides person-centered case management to individuals who are diagnosed with Serious and Persistent Mental Illness who are residents of Ramsey County. Case Managers help link individuals to services, which include mental health, chemical health, medical, social, vocational, legal, educational, and recreational services.
Community Care Services Team (CCST): Specializes in helping individuals with mental health and substance use disorders returning to, and remaining in, community settings after extended or frequent stays in inpatient hospitals or other similar settings. Coordinates with the treatment team to provide services for individuals, supporting the development, writing, implementation, evaluation, and revision of overall treatment goals and plans in collaboration with the individual served. CCST coordinates services with other county or community agencies including but not limited to local community hospitals, mental health case management, treatment providers, and primary care.

Targeted Case Management:
Helps adults with serious and persistent mental illness (SPMI) gain access to needed mental, medical, social, educational, vocational and other necessary services as it relates to the person's overall health needs.
Assertive Community Treatment (ACT):
ACT is an evidence-based treatment for adults who have a severe and persistent mental health illness. Based on severity of symptoms and impairments, clients work with the team on a long term basis.
Adult Rehabilitation Mental Health Services (ARMHS):
Adult mental health services that enable individuals to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills.
