Home and Community Care: Provides and coordinates many services through a continuum of care for Veterans in their homes and community settings, including Home-Based Primary Care, Medical Foster Home Program, and providing programs and services that are delivered by community partners.

Blind/Vision Rehabilitation Services: Provides a continuum of care for severely visually impaired, TBI, low vision, and blinded Veterans and service members.
