Chemical Dependency Services - Range Mental Health Center

Provides a multi-disciplinary approach toward the treatment of chemically-dependent individuals, their families, and their significant others. Services are available on both a residential or outpatient basis; Outpatient programs are available during the day or in the evening. Each client has an individual treatment plan and aftercare recommendations directed toward their specific needs and circumstances. Staff is also available for assessments, interventions, and educational presentations.

Assessment Unit - Leech Lake Band of Ojibwe

- Provides Rule 25 Chemical Health Assessments for Native American people living within the boundaries of Leech Lake Reservation. - Provides assessments for ages 11 through adult. - Referrals are received through self-referrals; courts; probation; Child Welfare; and sometimes through detox centers, hospitals and schools. - Works with approximately 45-50 providers statewide, as well as referrals to the various Leech Lake CD programs. - Clients are referred to Inpatient Treatment, Outpatient Treatment, Aftercare and Relapse Prevention programs, and Halfway Houses. - Collaborates with treatment centers, social workers, probation officers, hospitals and schools to make the best possible plans for clients to obtain and maintain sobriety. - Priority assessments for pregnant women and adolescents. - Civil commitments for clients who are very ill due to chemical/alcohol use. - Data privacy and confidentiality are strictly adhered to according to policy.

Adolescent Intervention - Northland Recovery Center - Grand Rapids

Community Wellness & Recovery Navigator Program provides resources, non-clinical assistance, advocacy, and recovery support to individuals who struggle or are in recovery from a substance use disorder.

Road to Success - PACT for Families Collaborative - Willmar

Supports positive youth development for ethnic cultural minority youth who are suspected of using alcohol or other drugs, use alcohol or other drugs, or who have family members and/or friends who are using. The program uses a pro-social approach which engages youth within their communities, schools, peer groups, and families. The program provides one-on-one support to increase knowledge, develop life skills, and overcome hurdles by fostering positive relationships.

First Steps to Healthy Babies - First Steps to Healthy Babies

First Steps to Healthy Babies is a support program that uses case management to help pregnant women struggling with substance abuse. There are case managers at Sanford Bemidji and Beltrami County. To refer a pregnant woman who lives in Beltrami County, please call Beltrami County Children's Services. To refer any pregnant woman who will receive care at Sanford Bemidji please call 218-333-2126.

Restorative Justice Circle Program - Southwest Health and Human Services

Circle brings people together and provides a process for organizing effective group communication, relationship building, decision-making, and conflict resolution. Restorative practices help create a community that honors the importance of relationships amongst all members in the community and repair relationships when harm has been caused. - Circle of Hope Circle of support for those working towards finding and maintaining ongoing recovery from substance abuse. - Family & Community Circle Creates support networks for youth transitioning home from placement and works with youth and families with a variety of familial matters that may be leading to placement or family conflict. - Juvenile Circle Sentencing An alternative response to a crime that promotes healing and safety for everyone. Included in the Circle process are those harmed by crime/negative behavior, those who commit crime, and the community. - Other Circles As needed (i.e. schools, parenting, foster parent, etc.)

The Kathleen Blatz Family Recovery Program - Ross Resources, LTD

The Kathleen Blatz Family Recovery Program offers support and an alternative to families involved in abuse, neglect and dependency cases where identified parental substance addiction is the primary factor. The Recovery Specialists offers support while monitoring clients on their pathway to recovery. Recovery Specialists serve as a liaison between the client, child protection case manager, treatment case manager and probation officer if appropriate. There is a unique blend of mentoring and monitoring

Addiction Treatment for Adolescents, Young Adults and Their Families - Hazelden Betty Ford Foundation

- Assessments: The alcohol and drug assessment process provides a diagnosis along with recommendations for follow-up care. Screenings are done via phone. - Residential: Programs include integrated mental health services for co-occurring mental health issues. Each patient's length of stay is individualized and based on the client's needs. Males and females are provided care in separate gender-specific units. Special programming for those with opioid addiction. - Outpatient: Both Day Treatment and Intensive Outpatient Programming is available. Treatment is abstinence based and uses the evidence-based Twelve Step approach. Patients receive alcohol and drug assessments that are used to develop their individualized treatment plan. For those that require more structure, an on-site sober living option is available. - Mental Health: An on-site mental health clinic provides ongoing mental health support to individuals and families with their residential care or outpatient programming - Family Services: Available to families whose loved one is in residential treatment. Siblings are also encouraged to attend. In addition, there are support groups, parent coaching, recovery apps and mental health groups for parents to help them heal along with their loved one. - Recovery Management: Addiction recovery requires daily commitment to sober living. Has post-treatment resources and services to help individuals grow their recovery and maintain their freedom from drug or alcohol addiction. - Teen Intervene: Designed specifically for youth who are experiencing mild to moderate problems associated with alcohol or other drug use, the Teen Intervene Program provides education, support, and guidance for teens and their parents. Through this 3-session, brief intervention model of care, youth work one-to-one with a counselor in identifying and changing their choices and behaviors.
