Supported Apartments - Mary T. Inc.

Supported Apartments located in Anoka County which are ideal for individuals who need on-site supervision, providing support with activities of daily living and life skills coaching. Mary T. Inc. staff function out of a hub apartment which also serves as a community room. Staff are on-site throughout the day and are available at all times. Services are tailored to meet the individuals' needs and may include independent living skills, medication and budget management, recreation, transportation, community meals, and assistive technology.

Includes: Breastfeeding Information and Support: - Public Health Nurses, WIC Lactation Consultants and Peer Breastfeeding Counselors are available to answer questions or concerns regarding breastfeeding. WIC participants should contact (952)563-8993; Non-WIC participants should contact (952)563-8900; or email Help Me Grow South Hennepin: - Interagency review team - Early intervention for children ages birth to 5 - Testing available for developmental delays in speech/language, motor, cognitive or social/behavior problems Follow Along: - Child development and tracking program for children up to age 3 Health Screenings: - Blood pressure screenings, 12:30 pm - 3:30 pm Monday - Friday at the Public Health Center; also offered at Creekside Community Center, Edina Senior Center and Richfield Community Center - Hearing screenings offered at Creekside Community Center (952)563-4944 and Edina Senior Center (952)833-9570; call for specific dates and times - Tuberculin Skin Test (TB testing) offered at the Public Health Center, 11:30 am - 3:30 pm Monday, Tuesday, Wednesday, and Friday. Cost is $35. Immunizations and Flu Shots: Offers immunizations to individuals who are uninsured or underinsured through the MN Vaccines for Children (MNVFC) and the Uninsured and Underinsured Adult Vaccines (UUAV) programs. Immunization clinics are offered the first and third Tuesday of the month, 3 pm - 5:30 pm. No appointment necessary. A $20 donation requested per vaccination, however, no one will be turned away. The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for: - Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA) - Home and Community-Based Services Waivers based on age: A. Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW) B. Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC) - Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management) Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

- Provides assistive technology to assist with safety, communication, and automation. Assists with installation, programming, and training the user and caregiver on how to use the assistive technology. - Automation: Technology to automate can help convert traditional items to accessible items including things like voice-controlled appliances, motion sensors to open or close doors, etc. - Communication: Ipads and smart speakers can be utilized to help with eye tracking, screen reading software, and text-to-speech communication. - Safety: Helps design components or full systems to provide a safe environment for each individual's needs. Examples include window alarms, GPS tracking watches, personal emergency devices, etc.
