Waivered Services: - Available to persons with a variety of disabilities - Waivered services are flexible and can be provided in a variety of settings - CADI (Community Alternatives for Persons with Disabilities) Waiver is available to adults and children with disabilities who need a nursing facility level of care: - TBIW (Traumatic Brain Injury Waiver) is available to adults and children who have had severe brain injuries from an accident, or due to stroke, heart attack or disease - MR/RC (Mental Retardation and Related Conditions) Waiver provides supports to adults and children with developmental disabilities - EW (Elderly Waiver) provides supports to seniors at home or in adult foster care who would otherwise have to live in a nursing home - AC (Alternative Care) Waiver provides supports to seniors at home or in adult foster care who would otherwise have to live in a nursing home, who because of their assets don't qualify for traditional Medical Assistance programs. Waivers pay for services such as personal care assistants, independent living skills counseling, homemaking, respite care, in-home family support, and other services Personal Care Attendants: - Provide assistance with daily living activities such as hygiene, grooming, dressing, taking medications, simple housekeeping, and other tasks - PCAs also provide behavioral monitoring and supervision to help clients with behavior problems - PCAs can help clients with FAS to live in a variety of settings, including foster care, their own homes and apartments, and family homes Other services: - In-home family supports and respite RSI provides support to caregivers and families of people with disabilities - Respite services gives caregivers and families a break for a few hours to a few days at a time. Respite can be provided in or outside of the family home - In-Home family supports services helps adults and children with daily life management skills, such as self-care, socialization, sensory/motor development, behavior management, community living and healthy recreation. - Respite and in-home family supports are waivered services programs - Adults and children who do not qualify for the MR/RC waiver may be eligible to receive similar kinds of help through the CADI or PCA programs

- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Six long-term residences in Saint Paul's Crocus Hill neighborhood, which serve as transitional, self-governing sober living facilities for men and women. Housed separately, men and women work closely with sponsors to create a safe and supportive environment while establishing a solid foundation for recovery. Each resident commits to living at the residence for a minimum of six months.

Children's Therapeutic Services and Supports (CTSS) aim to help children: - Restore emotional, social, and behavioral skills that have been impacted by mental health disorders - Gain coping skills and self-management tools to manage symptoms and related stress - Develop abilities and skills that are typical for youth of the same age without mental health symptoms Specific services provided to a youth and their family are based on: - Family needs - Family culture - Availability - Model or type of services offered in the area CTSS services can be basic community-based therapy services, such as in a home or school. They can also be more structured and focused intensive site-based services, such as day treatment. For families with complex needs and multiple providers, services must be coordinated with other providers or agencies working together in order to provide comprehensive continuity of care. CTSS services may include: - Therapy: Individual, family, and/or group psychotherapy - Skills training: Individual, family, and group to learn social, coping, communication, and/or daily life skills - Crisis plan development: For the child and family - Children's day treatment: Using trauma-focused CBT and DBT-inspired programming - Behavior skills in-home program: One hour session to help children learn specific skills to reduce symptoms of their mental health diagnosis CTSS provides counseling for children and youth for 2 - 5 hours per week at the clinic, home, school, or community setting focusing on: - Depression - Substance abuse - Sexual issues - Trauma - Anxiety - Family/relationship issues - Adjusting to transitions - PTSD Services may be a combination of skills training, psychotherapy, and crisis assistance. If appropriate, children and teens may be referred to participate in psychotherapy groups as a part of treatment. Groups are comprehensive, trauma-informed, and Dialectical Behavior Therapy (DBT)-inspired for the treatment of depression, anxiety, trauma, and other mental health disorders.
