Family Services - Trillium Services

Offers individualized supports for people with disabilities and their families, both in and outside the home. Services are person-centered and designed to empower individuals while providing meaningful support to families. Services include: - Consulting Services: Designated Coordinator and Designated Manager consulting for family foster care and small providers, ensuring compliance with MN Statute 245D. ?- In-Home Support Services (IHS): 1:1 skill-building, teaching, and training in the home, including community outings and activities. - Respite Care: Temporary in-home or out-of-home relief for families and caregivers, ranging from a few hours to extended stays. - Skill Development Activities: Support in areas such as cooking, fitness, health, hygiene, medication management, money handling, relationship building, safety, and social participation.

Behavioral Health Care Program - River City Therapy Center

Provides therapy and counseling services for individuals age birth - 21 and their families and caregivers that have disorders that affect social, learning, and behavior including Applied Behavior Analysis (ABA) therapy for autism spectrum disorder and related disabilities. Offers tailored services to each individual and can include expressive and receptive communication skill building, social behavior skills, activities of daily living, educational skills, and vocational skills.

Reach - Children's Mental Health Services

- Reach is a day treatment program provided to children who have serious emotional and/or behavioral problems creating problems for them in multiple settings - During the school year, day treatment is done at school in resource areas - In the summer, the program is in Grand Rapids for eight weeks, three days a week, and children are transported here by NKG Bus Transportation. You can call 218-245-2786 or 218-327-4886 to set up transportation - Inquiries need to be processed through Itasca County Human Services - Questions are referred through intake/screening at Itasca County Health and Human Services 218-327-2941
