- Case management and referrals for programs such as Meals on Wheels, social services - Door-to-door transportation mostly for health related appointments or for hair salons/barbershop appointments. Clients must be able to transfer into the car on their own. Escort services may also be available, but these requests must be made far in advance. - Homemaking, yard work and home maintenance services - Nursing care - Personal care - Senior exercise classes are offered daily; transportation available. - Social outings - Speakers on senior health topics - Support for caregivers of seniors who live in the service area, and for caregivers who live in the service area even if the cared-for senior lives elsewhere, including out of state.

Caregiver Support Group (in-person and virtual options available): Meet with a trained facilitator to discuss common challenges they are facing, successes they are having, encouragement they have to offer, needs that they are seeking resources for, and many other things. This is a safe space for caregivers to meet, so they can improve self-care as well as care of their care receiver. Chronic Conditions Support Group: An opportunity to meet with other people living with a Chronic Condition or Disability to exchange helpful tips, give and receive support, and learn about new resources. Each session offers the opportunity to learn about managing common conditions in later life, as taught by a gerontologist, while also providing valuable social and emotional support from others. Conditions may include but are not limited to: - Arthritis - Depression - Diabetes - Heart disease - High blood pressure - Parkinson's, etc. When Caregiving Ends Support Group: A counselor and Elder Network volunteer uses the book, "After Caregiving Ends, A guide to Beginning Again," to facilitate discussion and help in navigating life after caregiving journey ends. Memory Café in Olmsted County: Memory Café is an opportunity for those with dementia and their caregiver to meet in our community for casual conversation and interaction with other people struggling with memory problems. Provides relaxing activities, crafts, drinks, and snacks. Memory Café in Wabasha County: Memory Café is an opportunity for those with dementia and their caregiver to meet in our community for casual conversation and interaction with other people struggling with memory problems. Provides relaxing activities, crafts, drinks, and snacks.

Services are provided by staff and trained volunteers who are residents of the community. Sessions are scheduled at a time and location convenient to parties including night and weekend hours. Dispute resolution services are available for a wide range of disputes including but not limited to: - Family conflicts - Elder care issues - Juvenile restorative justice diversion programming - Neighbor conflicts - Landlord/tenant disputes, pre-file eviction - Unmarried couple separation - Post divorce and parenting time - Roommate disagreements - Employer/employee - Business/consumer - Conflict between students in school - Conflict within neighborhood/community groups, boards of directors, businesses, condo-townhouse associations and co-ops Note: Agency is a member organization of Community Mediation of Minnesota which is a statewide single entry point for conflict resolution services. Winona Senior Advocacy is a government program providing assistance to older adults. Advocates can provide assistance for older adults to help enable them to live independently in the community. Advocates will do home visits to assess needs. Services include information and referrals for housing, employment, Medicare and supplemental insurances, other government benefits, and transportation services. In addition, advocates can provide assistance with paperwork (i.e. medical claims, benefit applications, patient assistance programs, etc.) and with money management. All services are free and confidential.

Care consultation provides adults age 60 or older, their caregivers, and families with knowledge, skills, tools, and resources to help older adults lives as independently as possible. Consultants work over the phone or in the home to identify services to meet needs. There are no income guidelines, and services can be provided for as long as needed, with no minimum or maximum visits required. Care Consultants can: - Advocate on your behalf to ensure you receive the services you need - Connect clients to services that increase health and independence - Create customized action plans - Educate regarding chronic disease management and prevention - Provide family mediation - Fill out legal documents such as health care directives - Identify community resources - Identify risks in the home that compromise health and safety - Help with managing difficult family decisions - Help with planning for the future - Help with problem solving
