Medicare Savings Programs have four sub-programs: Qualified Disabled and Working Individuals (QDWI): Helps pay for Part A premiums only. Qualifying Individual (QI): Helps pay for Part B premiums only. Specified Low-Income Medicare Beneficiary (SLMB): Helps pay for Part B premiums only. Qualified Medicare Beneficiary (QMB): Helps pay for Part A and B premiums as well as deductibles, coinsurance, and copayments. Monthly income and countable resource limits apply. Countable resources include any checking or savings funds, stocks, and bonds. A home, one car, a burial plot, $1500 in burial expenses (if the amount is set aside), furniture, and other household items are not applied toward the resource limit. Participants pay for Medicare services that aren't covered, such as routine physicals and prescription drugs. Generally under these programs, premiums are put back into the monthly Social Security check, increasing participants' monthly income.

Provides law enforcement and community services.

Provides temporary, family-style housing to men experiencing homelessness. Offers free meals, showers, and laundry access.

Cash-grant program for adults with low incomes experiencing a household emergency. Adults can receive assistance once every 12 months. The assistance received must resolve the crisis. Adults must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. ?Emergency General Assistance provides short-term, one-time assistance for persons in a financial crisis because of fire, flood, storm, illness, accident, theft, utility shut-off, eviction, foreclosure, moving expense, necessary home repairs, or need for furniture or appliance replacement or repair.

Provides establishment and enforcement of child support orders. Services include:

• Collecting current and past-due support payments

• Establishing parentage

• Establishing and enforcing child support orders for basic, medical, or child care support

• Locating non-custodial parents

• Reviews and modifies court orders for support

Prepares new Americans to work in the hospitality industry as hotel housekeepers. 6-week class runs throughout the year and focuses on industry-specific English language training, skill training and soft-skill development. After training, the IIMN employment specialists provide employment assistance, placement and ongoing employment support. Graduates from the Hotel Housekeeping Program, Cashiering Skills, Professional Leadership Training, Dietary Aide Training work throughout hotels and in other cleaning-based positions in the Twin Cities. The Institute also offers advanced training for housekeepers wanting to move up in hospitality.

Provides handmade blankets to seriously ill and traumatized children:

High School Teen Center

Provides supplemental food according to family size. Clients may use the food shelf up to two times per month. Donations can be dropped off at the east door at any time.

Bi-CAP is one of six partners (five Community Action Agencies, and Evergreen Youth and Family Services) implementing the DHS Service Grant. Service dollars are available to households who are experiencing long-term homelessness my MN State definition. To qualify, participants must have experienced one year of consecutive homelessness or have experienced four episodes of homelessness in three years. This program is available in Cass and Beltrami Counties and is funded by the State Department of Human Services (DHS).

A vocational rehabilitation program that offers community-based services, including case coordination; training in basic living skills, social skills, leisure skills, mental health maintenance, and work adjustment; and vocational training in fields such as janitorial work, construction, mail handling, and more. The program includes on-the-job training where trainees work about three hours per day, preparing for ongoing participation in the Jobs Program. The training duration ranges from three to nine months. Services:

• Day Camp Ages 4 - 14: Provides fun and challenging activities in both small and large group settings. Campers can check out camp and still come home each night. Eight day camp locations around the metro area: Lakeville, Lino Lakes, Long Lake, Loretto, Monticello, Mound, Rochester, and Rosemount, Minnesota and Hudson, Wisconsin

• Overnight Ages 7-17: Campers get a well-rounded experience with activities like horseback riding, swimming, ropes, kayaking, and camp fires. 3 - 26 day and single gender sessions are available. Four overnight camp locations in Eveleth and Loretto, MN and Amery and Hudson, WI

• Teen Wilderness Ages 11-18: Campers start in base camp then head out on canoeing, backpacking and rock climbing trips. Sessions range from 5 days to 1 month or more. Two locations Grand Marais and Ely, Minnesota.

• Family Camp All Ages: Families get away and come together through opportunities for growth, spiritual development and environmental awareness. YMCA Camp du Nord is located on the edge of the Boundary Waters Canoe Area Wilderness and Camp Northern Lights on the edge of the Superior National Forest.

• Outdoor learning programs that can be tailored to group size, program lengths, and curriculum. Also provides learning opportunities delivered through special events.

Financial assistance and/or scholarships available

• Youth activities such as gymnastics, soccer, skiing, outdoor skating, baseball, batting cage, computer skills, Tae Kwon Do, study skills instruction, and first aid

• Swimming lessons, water aerobics, Aquacise, Swimnastics, lifeguard instruction, swim club, lab pool, and open swimming

• Safety classes for motorcycles and mopeds

• Defensive driving for seniors

• Various craft and hobby instruction

• Health and fitness activities such as dance classes, yoga for adults, Tae Kwon Do for adults, health education classes, and beginners golf

Assistance paying child care costs for children up to age 12 and for children with special needs up to age 14. Child care costs may be paid for families while parents work, look for work, or attend school. Family size, family income, and participation in authorized activities are considerations in determining eligibility for and amount of assistance, along with available funding.

A private, non-profit organization serving Clearwater County. (Requests from outside of the county will not necessarily be denied) The purpose of Salvation Army is to provide moral and spiritual support, material goods, food, shelter, and transportation for those unable to provide for themselves. Every situation is reviewed individually Conditions of service:

• Will maintain anonymity and confidentiality

• Will require name, address and Social Security number in order to provide ANY services.

The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for:

• Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA)

• Home and Community-Based Services Waivers based on age:

A. Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW) B. Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC)

• Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management)

Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.
