Assertive Community Treatment (ACT):ACT is an evidence-based treatment for adults who have a severe and persistent mental health illness. Based on the severity of symptoms and impairments, clients work with the team on a long-term basis.Adult Rehabilitation Mental Health Services (ARMHS):Adult mental health services that enable individuals to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills.Behavior Health Home:Provides individuals with their care needs. The team works closely with the individual, their supporters, and providers to ensure comprehensive quality of care.Community Support Program (CSP):The Community Support Program is a coordinated care and treatment program that provides a wide range of individualized treatment, rehabilitation, and support services to persons with serious and persistent mental illness. The purpose of the community support program is to assist individuals to live as successfully and independently as possible in their own communities by supporting independent living and coping skills.Targeted Case Management:Helps adults with serious and persistent mental illness (SPMI) gain access to needed mental, medical, social, educational, vocational, and other necessary services as it relates to the person's overall health needs.

Cash-grant program for adults with low incomes experiencing a household emergency. Adults can receive assistance once every 18 months. The assistance received must resolve the crisis. Adults must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help.Short-term grant assistance for persons in a financial crisis including utility shut-off (electric, heating, gas and water service payment assistance), eviction and foreclosure.Applicants must show how they have spent their income from the last 60 days to qualify.

Case management services include:- 24 hours/7 days a week emergency availability- Advocacy and education- Assessments and consultations- Assisting with health care advanced directives- Coordinating and attending appointments- Crisis intervention- Hospital visits and discharge planning- Identifying and coordinating resources and services- Providing case management reporting- Relocation assistance

Nurses can provide medical advice on what potential options are available such as virtual care, primary care, urgent care options, or seek emergency care if needed at an Essentia Health location.

Service provides work orientation, work activity training, community orientation, and focus on personal habits as related to work and other aspects of daily living. Employment opportunities for individuals with challenges, due to mental disabilities or related diagnosis.

Patients who need to speak with a financial counselor, need cost of care estimates, or determine if they may qualify for financial assistance to pay for procedures, may reach out about options. Financial counselors can help individuals understand patients' responsibilities and may offer no interest payment plans.

Program offering therapy that examines behavior in terms of motivation at the unconscious level. Therapy focuses on the mental and emotional processes that are experienced and developed in early childhood, and their effects on the individual's past and current behavior and mental states.Individual, family, group, and couples therapy available.

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers.Works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child. Services may include:- Activities and classes for parents and grandparents to strengthen parenting skills, activities, and make friends- Book and toy lending library- Dad and child programs- Early childhood screening for children ages 3 - 5- Home visits- Information on community resources and online parent video topics- Parent-child activities- Parent discussion groups- Play and learn activities for children- Programs for non-English language learners- School readiness program for children age 3 1/2 - 4- Single parent program- Special events for the entire family- Workshops on specific topics

Community-based treatment program designed to reduce symptoms of depression and improve quality of life. The program consists of 6-8 in-home counseling sessions that focus on the following goals:- Becoming socially and physically active - Scheduling enjoyable activities - Solving problems

Offers employment services at no cost, including job and career training, as well as employment and placement opportunities for youth. These programs are for youth who have nowhere else to turn for help to find employment, complete their education, and who need support to move forward.Services include paid internships, tuition to train for a new career, help finishing school or getting a GED, help with job hunting, and transportation assistance (such as bus fare cards).Participants in Avivo's youth and young adult employment programs can also access short-term credentialed training options for higher-wage jobs, such as:- Certified Nursing Assistance (CNA)- Forklift Operator, including Forklift Operators Certification- Culinary Arts and ServSafe certifications- Enrollment opportunities in credentialed training at Avivo Institute of Career and Technical Education

Provides intensive, community-based mental health support for children and youth experiencing significant emotional or behavioral challenges. Offers individualized therapy, skill-building, crisis intervention, and family/foster support to improve daily functioning, stabilize symptoms, and reduce the risk of out-of-home placement. Services are delivered in the home, foster placement, or community, with a focus on collaboration, flexibility, and empowering youth to thrive in their environment.

Hired supports individuals, families and youth through comprehensive employment services to learn new skills and achieve their academic and career goals. Programs are available throughout the Twin Cities, but not all programs are offered at every site.- Adult Employment/WIOA Program: The Workforce Innovation and Opportunity Act (WIOA) serves individuals who are looking to improve their skills and advance their careers- Career Training Program: Provides counseling, training, workshops, and job search assistance. Participants build skills and gain professional qualifications to find employment that is a right fit for them- Family Stability/MFIP - Minnesota Family Investment Program: Assistance for parents who receive MFIP to help them build family-sustaining careers- Youth Services: Provides coaching, support, and training to help youth and young adults complete a diploma or GED, explore careers, and gain employment in a field they are interested

Telephone Recovery Support (TRS):A peer-to-peer support program for individuals seeking to initiate and/or maintain recovery from drug/alcohol addiction. TRS volunteers reach out over the phone and check-in with individuals; and, if needed, connects them to resources needed to achieve and maintain recovery. This support is limited to the telephone.Recovery Coaching: Includes all Recovery Meetings and site-based Recovery Coaching. Recovery Meetings offer an open format where people can connect with others who are initiating and maintaining recovery. Groups range from the population served to the languages offered. Site-based Recovery Coaching provides opportunities to connect with a Recovery Coach in a one-to-one setting and connects individuals to resources to strengthen their recovery and remove barriers.

We have the experience to make the right recommendations for your specific needs. We have certified ATP's(Assistive Technology Professional) to fit patients to custom wheelchairs and durable medical equipment. Our certified ATP's will evaluate the individual's physical, environmental and social situations to help determine the appropriate equipment for comfort, support and to help maintain independence. A donation is tax deductible and the equipment donated will help another person living with ALS.
