Scattered-site and site-based housing for families with children who are coming directly from a shelter. Housing services include:

• Housing navigation assistance: Provides housing search assistance, contacts and recruits landlords, assists households with completing applications

• Rent payment and move-in assistance: Provides financial assistance such as rent subsidy, security deposit, or application fees to move into permanent housing

• Case management and services: Provides services and supports, including services such as assessing barriers to housing stability, making referrals and connections to community resources, partnering for housing stability

Pharmacy services

Helps individuals to gain skills for the workforce. This is achieved by either volunteering first with the agency to possibly obtain employment with Mad Dads or attending one of the classes offered. Classes teach financial literacy, interview skills, job search strategies, job training/skills development, job placement, and personal development.

• Access to books, audiobooks, large print materials, magazines, musical CD's, and local newspapers

School-based advocacy for students experiencing family or teen dating violence, including:

• Training, information, and legal advocacy to parents of children with disabilities to help them secure the education and services guaranteed to their children by law

• Legal representation (only takes a few cases each year) to adults and children with disabilities, in both individual and class action cases involving their rights to employment, education, transportation, housing, and access to public accommodations

• Training and speakers for the disability community, public entities, and private businesses on the provisions of the ADA and its relationship to other state and federal laws

Minnesota's Medicaid program for people with low income. Most people who have MA receive health care through health plans. Members who do not receive health care through a health plan receive care on a fee-for-service basis, with providers billing the state directly for services they provide. MA pays for a variety of services like doctor visits, prescriptions, and hospital stays. Some services and prescriptions may require prior approval. MA pays for medical bills going back three months from the month the application is received. NOTE: People with disabilities who work, even if they're over the income or asset limits of MA, may still qualify for a program called Medical Assistance for Employed Persons with Disabilities - MA-EPD.

Promotes community wellness through education, prevention, and health promotion activities, including:

• Blood pressure screening and other preventive health checks

• Chronic disease prevention and diabetes education programs

• Educational presentations on HIV/AIDS, sexually transmitted infections, and related public health topics

• Other services are provided once patients are admitted such as individual therapy, participation in Alcoholics Anonymous (AA)/Narcotics Anonymous (NA) groups, and peer support groups

Problems with stress, anxiety, depression, anger, social fears, and family conflict are explored in individual, marital, and family therapy sessions. Psychological testing is available to assess personality traits and dynamics, memory and attention processes, and general intellectual functioning.

Provides live interview coaching, online resume, and cover letter assistance. Also offers practice tests, an online writing lab, and skill-building tools in math, reading, and writing.

Workforce preparation, job placement, and job retention services. Works hands-on with clients to assess skills, prepare resumes, guides job-seekers through application forms, and teaches interview techniques. Customized training for job skills.

• Provides weatherization improvements to reduce eligible households' energy costs and improve the comfort level in homes.

• An energy auditor will evaluate and determine what improvements will be completed by a licensed private contractor for eligible homeowners and renters experiencing low income.

• Weatherization measures may include insulation and infiltration work, furnace maintenance, venting of water heaters, heating and exhaust systems, and installation of smoke detectors and carbon monoxide detectors when needed. This work is done as a one-time improvement to the dwelling.

Helps low-income homeowners stay safely in their homes by completing more substantial repairs than basic maintenance, including:

• Accessibility modifications (like ramps or grab bars)

• Flooring and structural repairs

• Plumbing or electrical issues

• Roof repair or replacement

Cash-grant program for adults with low incomes experiencing a household emergency. The assistance received must resolve the crisis. Adults must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. Provides short-term grant assistance for persons in a financial crisis including nonrecurring shelter and utility needs not exceeding 30 days. Payments of emergency funding are available for:

• Housing: Payments for mortgage/contract for deed foreclosures, past due rent (no more than 2 months) and damage deposits (equal to 1 month's rent)

• Utilities: Payments for municipal water and sewer services, electric, gas, fuel oil and wood. Deposits are limited to an amount equal to an average month's bill. Does not include late fees, court fees or repairs.

24 hour Assisted Living Facility

• 24 hour care provided by HHA, CNA's etc. - LPN & RN on call 24 hour a day - Meals, snacks, laundry, medication delivery, and personal care provided for elderly or disable adults.

Multi-specialty treatment through group, individual, and family therapy sessions. Treatment options focus on thorough assessment and symptom stabilization while working on healthy living skills. This program is a level of care for students who are struggling with symptoms of mental illness that interfere with their ability to function in their daily tasks for childhood or adolescence tasks. Individuals in the program attend weekdays for an average treatment of 16 - 18 days. Treatment may include: psychiatric medication management, family psychoeducation, group and individual therapy, art therapy, recreational therapy.

Provides for the final disposition of indigent individuals. Based on the deceased and next of kin preferences, options for the disposition are direct cremation or immediate burial.

• Direct Cremation: Maximum $21588. Includes local removal, care of remains, transfer of deceased to crematory, basic services of staff, authorization, crematory charge and a non-decorative remains container. May include a short time of remembrance/memorial.

• Immediate Burial. Maximum of $2,900. Includes local removal, care of remains, vault/grave box at invoice, transfer of deceased to funeral home, basic services of staff, authorization, basic casket and transfer of deceased to local area cemetery.

Toy lending library that provides up to five toys to members. Membership is yearly and based on a sliding scale fee.
