Provides general information and licensing for persons interested in becoming foster care providers. Training and support for foster care providers on emergency, short term and long term placement.

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers.ECFE works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child.Services may include:- Book lending library- Dad and child programs- Early childhood screening for children ages 3 - 5- Information on community resources- Outdoor classroom- Parent-child activities- Parent discussion groups- Play and learn activities for children- Programs for non-English language learners- School readiness program for children age 3 1/2 - 4- Special events for the entire family- Workshops on specific topicsNOTE: Call for current class offerings and schedule of events.

Partners with eight school districts and other community organizations in the northern metro area to offer adult basic education. Services provided:- Assistance in preparing for the GED examination is available at Anoka Technical College, Blaine Learning Center, Elk River ABE, and Saint Francis ABE- Brush-up instruction in reading, writing, and math is available at all sites except Anoka-Ramsey Community College, Centennial Adult Continuing Education, and Brooklyn Center ABE- Credit Recovery Diploma available at Blaine Learning Lab and Elk River ABE- English as a Second Language class for foreign-born adults is available at all sites except Anoka-Ramsey Community College and Anoka Technical College- GED Testing Centers are located at the Anoka Technical College and the Blaine Learning Lab site- Minnesota State Diploma available at the Blaine Learning Lab and Elk River Community Education Center sites

Provides a community center offer services such as:- Counseling and support for new immigrants- Employment referral and assistance- English as a Second Language and citizenship classes- Forms and applications assistance for Social Security, Supplemental Security Income (SSI), Medicare, Medical Assistance, MinnesotaCare, public housing, childcare assistance, WIC, or Head Start programs- Health counseling and education- Home care services contracted with Medica offering homemaking and PCA services to older adults and individuals living with a disability- Parenting skills counseling- Senior program offering socialization and activities as well as seminars on health, immigration, and social services every other month- Translation services- Transportation assistance for health appointments, benefits appointments, and school- Youth programs such as a summer youth program helping children who are academically at risk, lack English skills, and have minimal education improve their academics and learning

Crisis response offered through a partnership between Zumbro Valley Health Center, South Central Human Relations Center, and Hiawatha Valley Mental Health Center.Counselors help individuals reach a place of emotional and physical safety, and then direct the them to community resources that offer longer-term support. They also provide over the phone assessments, safety plans and support. Counselors connect individuals to local teams able to meet the individual where they are and provide additional support.Mobile teams complete confidential comprehensive crisis assessments. A crisis assessment is an immediate, face-to-face evaluation by 2 crisis-trained mental health practitioners consulting with a mental health professional.The assessment helps do the following:- Assess current risks and life events - Assess level of functioning - Assess mental health symptoms - Assess the protective factors that the individual has available in the crisis - Assess mental health symptoms - Provide a support network - Take into account cultural considerationsProvides immediate intervention to aid the individual in relieving current symptoms of the mental health crisis. Teams may provide the following services as deemed therapeutically appropriate:- Coordination with other local providers - Development of coping skills - Education - Means removal - Referrals to crisis residential services if available - Referrals to emergency departments - Safety planning - Situational deescalation

Cash-grant program for families with low incomes experiencing a household emergency. Families can receive assistance once every 12 months. The assistance received must resolve the crisis. Families must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help.?Short-term, one-time grant assistance for persons in a financial crisis because of fire, flood, storm, illness, accident, theft, utility shut-off, eviction, foreclosure, moving expense, necessary home repairs, or need for furniture or appliance replacement or repair.

Offers a variety of community health services, including:- Community health promotion- Health education programs- School health programs including screening, education, and consultation to schools through contractual arrangements- Foot care clinics- Statewide Health Improvement Program (SHIP) interventions to prevent obesity by implementing policy, systems, and environmental changes to increase physical activity and intake of healthy food choices- Programs to decrease the use of tobacco and other drugs in youth and decrease the use of tobacco in all populations

Provides investigation and intervention services on behalf of vulnerable adults who are unable to act on their own behalf, manage their own affairs, or who are in immediate danger of abuse. Types of abuse may be physical, emotional, sexual abuse, unsafe or hazardous living conditions, exploitation, neglect, or abandonment. Services include:- Taking reports and investigating abuse and/or maltreatment- Removal of the individual to safer surroundings- Services necessary to remove the conditions which have created a threat to lifeNOTE: Complaints, questions, or concerns involving incidents in facilities licensed by the Health Department, such as hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living and home health agencies can be referred to the Minnesota Board on Aging Ombudsman for Long-Term Care.

Long term and short term skilled nursing home with services including: - 24/7 nursing assistants, registered nurse, and personal care - Daily activities and therapeutic recreation- Fully furnished private or semi private rooms and housekeeping- Hospice - On site physician visits- Rehabilitation therapies- Spiritual care- Three meals daily

Provides state-funded food assistance to legal noncitizens ages 50 or older who do not qualify for the Supplemental Nutrition Assistance Program (SNAP, formerly called Food Stamps or Food Support) because of their citizenship status.Monthly benefits are loaded on an Electronic Benefit Transfer (EBT) card, which looks and works like a debit card. The card can be used to buy groceries at authorized stores, farmers markets, and online retailers. ????For Organizations:Art Project Grants: Provides area organizations in the SMAC region with up to $7,000 to stimulate and encourage the creation, performance, and appreciation of the arts.Equipment/Facilities Improvement Grants:Provides up to $10,000 in matching funds to arts organizations for the purchase of equipment items and facilities improvements that will strengthen their capacity to serve the arts needs of the region.Public Arts Grants: Provides organizations and city, county, or tribal governments with up to $20,000 for large-scale physical public art projects that will leave an arts legacy in southwest MN. The project should result in sustainable arts access in the community, with a continuing impact over several years. 20% cash match required.Quick Support for Organizations Grants: Provides organizations with up to $2,500 for activities such as planning for a Public Art project, strategic planning or board training, costs to apply for 501(c)(3) status, small equipment purchases, or small arts projects.For Individual Artists:Art Study Opportunity for Youth Grants: Up to $500 to aid in the development of young artists in grades 5 - 12 through an enrichment camp or specialty study.
