Provides services to adults and children who have developmental disabilities or related conditions, including persons with challenging behaviors, those who are mentally fragile, and those who are challenged by communication deficits and physical limitations. Included in this service: In-Home Support Home and community-based services for persons living in the family home. Programs are designed for individuals who may need skills enhancement for socialization, management of finances, housekeeping, and other activities of daily living. Also provide services to children, assist families with support, advocacy, transition, and maintenance of skills. Respite Care Services provide short-term relief from the demands which around-the-clock care places upon families who have disabled and/or chronically ill family members at home. Services are provided in the client's home or in a licensed foster home. Trained staff assume the care of these individuals for a matter of hours or days to allow the families time for rest and recreation. Semi-Independent Living Services Designed for persons living in their own home or apartment who require minimal supervision. Individual programs are tailored to provide assistance & training in paying bills, building socialization skills, accessing the community in which he or she lives. Supported Living Servicesclass="pasteContent_RTE">Provides residential services in homes for 1-4 persons. Programs provide up to 24-hour supervision. With increased structure and support, individuals are taught skills that will increase their use of community resources and enable them to live in a less restrictive environment.

Supported living settings are homes for up to four people with physical, medical, mental health and behavioral support needs. They combine a positive, nurturing environment with personalized supports and 24-hour supervision. Through customized care plans, individuals receiving support work toward achieving their goals, such as living independently in an apartment or cooking healthy meals. Services promote development in the areas of social skills, recreation, meal preparation, budgeting, self-care, and educational or vocational interests. Individuals form relationships with skilled and caring direct care professionals and nursing staff who ensure their everyday needs, as well as overall medical requirements, are being met.

Offers individualized supports for people with disabilities and their families, both in and outside the home. Services are person-centered and designed to empower individuals while providing meaningful support to families. Services include: - Consulting Services: Designated Coordinator and Designated Manager consulting for family foster care and small providers, ensuring compliance with MN Statute 245D. ?- In-Home Support Services (IHS): 1:1 skill-building, teaching, and training in the home, including community outings and activities. - Respite Care: Temporary in-home or out-of-home relief for families and caregivers, ranging from a few hours to extended stays. - Skill Development Activities: Support in areas such as cooking, fitness, health, hygiene, medication management, money handling, relationship building, safety, and social participation.

Mosher Veterans Home is a room and board residential facility, mainly for veterans, or county residents. The home provides living for individuals with private and double rooms available. Amenities include: 24-hour on-site staff, personal care services, laundry and housecleaning, medication management by an RN, dining room serving three meals a day, planned social and recreational activities, and on-site church services offered on a regular basis. Veteran residents have access to day treatment programs including incentive therapy work activities on a regular schedule at the Tomah VA Center with bus transportation provided. Non-veteran residents have access to work activities at ABC (Ability Building Center) in Caledonia with transportation provided.

- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more.
