The Scattered-Site Housing Program works directly with Coordinated Entry to provide permanent supportive housing within Ramsey County to women with a history of mental illness and/or chemical dependency, assisting with their goal towards recovery. Housing Navigators work to identify and remove barriers to housing, and work with landlords in the community to secure housing of the participant's choice. Upon moving in, wrap-around support services are provided by Case Managers and Peer Support Specialists to provide tenancy sustaining services as well as work on person-centered, long-term goals towards stability, self-acceptance, and empowerment.

• Community Supports: Connects people with community supports, such as home health care, meal preparation/delivery, transportation, and hiring someone to help at home or with yard work

• Health Insurance Counseling: Helps people with questions about Medicare and other health care questions

• Housing Resources: Helps people understand their housing options, such as assisted living requirements, comparing housing services and costs, and more

• Preadmission Screening (PAS): Manages preadmission screening (PAS) for anyone planning to be admitted into a Minnesota nursing home

• Prescription Drug Assistance: Helps those who qualify find ways to reduce prescription drug costs, including finding manufacturers' patient assistance programs and completing applications

Youth Transition Services:

• Assist youth ages 14 - 22 years who are transitioning from high school into the next phase of their life with learning the skills necessary and acquiring the resources needed to be successful

Coordination of behavioral resources. Goal is to reduce the need for hospitalizations and allow people to successfully remain in the community. Services:

• Preventative behavioral interventions, assessments, and crisis support planning

• Technical assistance and training to families, day programs, schools, residential programs, and care providers serving individuals with intellectual disabilities and/or related conditions as well as individuals receiving services though a CADI waiver

• Emergency telephone consultation

• Coordination of transitional housing within the Metro Crisis Coordination Program (MCCP) crisis provider system

• Support to reduce behavioral crisis that adversely affect the residential, work, or educational placements of people with intellectual disabilities and/or related conditions

• 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services.

• Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution.

• Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy.

• Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community.

• Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals.

• Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people.

• Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently

• Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Also, supports student youth as they search for and find competitive employment and helps them explore post-secondary education.
