Active Seniors: Provides adults 55+ with a variety of diverse activities such as exercise, wellness, and social/recreation designed to support independence Care Consultation: Keystone Care Consultants help seniors navigate their health, economic, and social needs. Care managers meet with individuals in their homes or at one of the community centers to conduct an assessment, gather input from caregivers, and create goals. Once the assessment is complete, services may include: setting up rides to a social program, researching housing options, making a referral to an agency that helps with grocery shopping and/or cleaning, providing caregiver support, and accompanying seniors to doctor visits. Grocery Delivery: Provides free grocery delivery for low-income seniors and adults with disabilities. Those who qualify can receive monthly deliveries. Delivery can include meat, dairy, fresh produce, and shelf-stable items. In each delivery, individuals will receive an average of 25 pounds of food per person in the household. Accommodations are based on customer choice models such as vegetarian, halal, etc. depending on what is available. Deliveries will be dropped off between 12:15 pm and 1:30 pm Tuesday - Thursday by a volunteer. In-Home Help: Social workers coordinate with Recover Health to provide seniors with in-home nursing care, in-home therapies, and home health aides to assist with daily care. ?Tax Preparation Assistance: Provides tax services in coordination with AARP to seniors. Provides resources for seniors living alone as well as caregivers needing support. Services are designed to help maintain independence as long as feasible. Home-based services provided according to availability of volunteers and staff: - Assisted Transportation: Short distance volunteer drivers provide help to access medical appointments, groceries, pharmacy, etc. Limitations and restrictions apply. - Companionship: Home visits provided by volunteers to provide non-medical support to encourage socialization - Homemaking: Indoor light housekeeping services include mild laundry services, floor care, kitchen/bathroom cleaning and other indoor help. Light "fix it" services (i.e. changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver and to learn from each other - In-Home Respite: Home visits by volunteers who visit with care receiver to temporarily relieve the caregiver. (Services are not home health aide, medical or PCA). - Social Respite: Memory Cafe meets once a month providing conversation, tactile arts/crafts projects, exercise, music and games to temporarily relieve the caregiver while providing socialization for care receivers

Provides ride services with volunteer drivers who use their own personal vehicles. Volunteer drivers provide rides to clients' destinations and pickups. - Individuals on Medical Assistance, Alternative Care (AC), Elderly Waiver (EW), Veterans Administration, or other third party contractors must meet state and local eligibility guidelines and rides must be for medical reasons only - Out of county rides are limited to within approximately 100 miles of Albert Lea, which include Rochester, Twin Cities, Mankato, Owatonna, and Austin - Rides are for any purpose within the city of Albert Lea to seniors living in rural Freeborn County. Rides can be for medical appointments, business, beauty appointments, visiting relatives and friends, coffee, grocery shopping, volunteer sites, senior dining sites, and personal appointments. - Rides outside of Freeborn County must be for medical purposes only - Rides within the City of Albert Lea must be booked two days in advance. Out-of-County rides must be booked by 12 noon two days in advance. NOTE: Rides will not be provided when schools are closed due to adverse weather-related conditions

Provides volunteer driver for eligible persons. - Each eligible client (on medical assistance) has a file which says what kind of transportation the person may receive and how often it may be used (for shopping, medical appointments, etc.) - All arrangements must be made at least 24 hours in advance during office hours, Monday through Friday. - Rides for the weekend needs are available with appropriate notice. - Clients are sent on the bus unless doesn't fit needs.
Wellspring can arrange transportation for both in-town and out-of-town medical and nonmedical appointments. Can make arrangements for this during our normal office hours of 8 am to 12 noon Monday - Thursday.
Transportation for medical appointments and other essential needs such as groceries, medications, etc.: - Volunteers use their own vehicle - Unable to transport individuals with wheelchairs or scooters - Able to assist those with a cane or walker provided that the individual can get into the vehicle. Drivers cannot lift riders. - Door-to-door or door through door depending on the individual's needs - Only available to those living in the Stillwater Area School District #834 Services are part of Community Thread's THRIVE program.
- Bus and vans are accessible, but volunteer drivers' vehicles are not - Daily city service is offered 7 am to 5 pm Monday through Friday, in the following cities; Appleton, Benson, Canby, Dawson, Madison, Montevideo, and Ortonville - Service is available for travel throughout the region and to destinations outside the region: Twin Cities, Marshall, Willmar, St. Cloud; areas throughout the state with volunteer drivers and some locations to North / South Dakota (call to ask about destinations outside the region). - School Year Rides and Summer Recreation Rides are available to students attending schools and recreation programs in the following cities: Appleton, Benson, Canby, Dawson, Granite Falls, Madison, and Montevideo
