Special Needs BasicCare - SNBC - Minnesota Department of Human Services
A voluntary managed care program for people with disabilities ages 18 - 64 who have Medical Assistance. The program is available through different health plans depending on the applicant's county of residence. Disability HUB MN will work with callers to determine the best option.
Benefits of the program include:
- A designated person at the health plan who helps individuals receive the care and support they need. Often referred to as a care coordinator or care navigator.
