- This is a Medicaid program for beneficiaries who need help in paying for Medicare services. The beneficiary must have Medicare Part A and limited income and resources. For those who qualify, the Medicaid program pays Medicare Part B premiums. Eligibility begins the month of application. Also, if eligible, retroactive coverage for up to three months prior to the month of application - Note: There can be a lag time before SSA is able to stop taking the premiums out of the recipient's benefit check but the recipient will be reimbursed by SSA for all months eligible for SLMB and/or QMB. The reverse is true when these programs terminate: recipients should be told to put aside premium costs from first month of termination on - Can apply online at www.mnbenefits.mn.gov

A voluntary managed care program for people with disabilities ages 18 - 64 who have Medical Assistance. The program is available through different health plans depending on the applicant's county of residence. Disability HUB MN will work with callers to determine the best option. Benefits of the program include: - A designated person at the health plan who helps individuals receive the care and support they need. Often referred to as a care coordinator or care navigator. - A 24-hour nurse phone line to answer health-related questions - Tips for navigating healthcare needs - The option to join a health plan advisory committee to provide insight to health plans regarding health care needs for people living with disabilities
