Services available may include the following: - Active Cognitive Support: Caregivers provide in-person or remote check-ins to provide support, guidance and problem-solve concerns related to daily living - Activities of Daily Living: Caregivers assist with dressing, grooming, bathing, and transferring (walking) - Adaptive Support: Encourage self-sufficiency and reduce the need for assistance. ICLS caregivers work with the client to develop and practice individualized strategies that promote independence, such as using calendars, making lists, and teaching how to use assistive technology. - Community Living Engagement: Helps clients access resources, services, and activities outside the home. These services help the individual develop and maintain a support system in the community. - Household Management Assistance: Caregivers assist with meal planning and preparation, light housekeeping, laundry and budgeting/money management - Health, Safety, and Wellness: Caregivers will identify any changes in the client's health, including monitoring any existing health conditions, with referrals to case managers as needed

Light "fix it" services (i.e. changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver and to learn from each other - In-Home Respite: Home visits by volunteers who visit with care receiver to temporarily relieve the caregiver. (Services are not home health aide, medical or PCA). - Social Respite: Memory Cafe meets once a month providing conversation, tactile arts/crafts projects, exercise, music and games to temporarily relieve the caregiver while providing socialization for care receivers

Companion/Night Supervision Services: Services that help an individual work toward a therapeutic or community integration goal. Night supervision is an overnight assistance and monitoring provided in the person's own home, which includes assistance with Activities of Daily Living (ADL's), reinforcing independent living skills, etc. Homemaking/Home Management Services: Provide assistance to individuals who are unable to perform day-to-day household duties and have no one available to assist them. Services include light housekeeping, laundry, limited personal care, grocery shopping, meal preparation, etc. Individual Community Living Support (ICLS): Provides assistance and support to clients who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own home. Services include assistance with active cognitive support, Activities of Daily Living (ADLs), household management, health, safety, and wellness, and community engagement. Personal Care Assistant (PCA): Performs care services to clients unable to live independently in the community without assistance. Services include assistance with Activities of Daily Living (ADLs), grooming/personal hygiene, light cleaning, cooking, etc. They also engage clients in activities such as reading, talking, community support, etc. Personal Support Services/Individualized Home Supports Without Training: Provided in the individual's home to achieve their full potential, to increase independence, and to meet community inclusion goals that are important and based on their need. Established to assist with Activities of Daily Living (ADLs), participation in community activities, etc. Respite Care: Short-term care services provided due to the absence or need for relief of the family member(s) or primary caregiver normally providing care. Respite can be provided in-home or out-of-home.

Home Health Aides assist with bathing and personal care, meal preparation, and assistance with hair and oral care. Physical therapists, occupational therapists, and speech therapists provide services also. - Immunizations: available for children and young adults, birth through age 18. Hepatitis B and Tdap for a fee for adults. Flu shots available from October through December. - Mantoux testing (tuberculosis) - Loan closet - Maternal and child health (car seats available for low-income families) - Prenatal care and education including safety and proper use of car seats - School health nurses consult with teachers on topics such as AIDS, teen pregnancy prevention, self-breast exams, and nutrition. Preschool screenings as well as immunization, vision, and hearing clinics.
