An integrated one-stop location connecting adults to critical services to improve their health, income, housing stability, and overall well-being. Services offered:

• Access to Benefits: Community representatives can help individuals move towards self-sufficiency or access community benefits and support. Representatives can help with MNsure applications, Open Access voicemail, SNAP applications, Social Security applications, Veteran's benefits, and more. Schedules may vary when community representatives are onsite.

• Hot meals available which include breakfast and lunch throughout the week and coffee daily

• Laundry services available to current residents of the shelter program

• Lockers available for use for 30 days

• Mail service

• Phone charging stations

• Showers, towels, and toiletries for clients

• Supportive Services: Provides comprehensive help through medical care, vision screenings, housing services, referral to mental health assessments, legal services, financial services, and job opportunities

• Offers grief support to Allina Hospice's patients' families and friends for 13 months following the death. Grief support includes Individual counseling, referrals to groups and other mental health providers, book recommendations, etc.

• Support groups are open to anyone in the community in addition to maintaining a list of other grief support groups

Child and Adolescent Gender Services Helps families understand various gender issues by developing customized treatment plans that may include:

• Individual, family, and/or group therapy and psychoeducation

• Communication and consultation with psychiatric and general mental health providers (when applicable)

• Medical care including: individualized information sessions on puberty suppression and hormone therapy; gender affirming medication interventions including puberty suppression, men- strual suppression, and hormone therapy

• Care coordination to help clients navigate the healthcare system and access resources

Adult Gender Services Services for adults and their families may including:

• Individual, family, and/or group therapy and psychoeducation

• Therapeutic sessions to explore and discuss relationships and sexual health (pleasure-oriented positive sexuality)

• Psychiatric referral

• Individualized information sessions on gender affirming hormone therapy options

• Gender affirming medical care including menstrual suppression and individualized hormone therapy, including low dose options

• Stabilization of common medical conditions, with referral to specialty and primary care as needed

• Gender affirming preventive care counseling

• Referral letters for medical interventions such as surgery

• Care coordination to help clients navigate the healthcare system and access resources

Therapeutic and Peer Support Groups Meets twice a month, topics include:

• Exploring embodiment goals and navigating medical interventions

• Coming out to friends and family

• Navigating social transition

• Sexuality, dating, and friendships

Provides triage services for situations as needed and reviews county case information. Services include:

• Assistance with housing and basic needs

• Community Collaborative Fund: Initial assessment for households at risk of homelessness. Individuals must have been denied by the Saint Louis County EGA or EA program, or have already used EGA/EA within the last 18 months. ESG (Emergency Solutions Grant) funds are determined to be used for rent, rental deposit, mortgage, or utilities. NOTE: There is currently no funding at this time.

• Forms and certificate assistance for county programs such as SNAP, WIC, EGA/EA, GA, child care assistance, MFIP, Minnesota Supplemental Aid, or Housing Support. Also, follow up with issues regarding MNsure applications.

• Referrals to community services, mental health, and chemical dependency services outside of Saint Louis County

Mental health intake and assessment of requests for case management services (Rule 79 services) and assisting with support services such as:

• Counseling

• Cultural Preservation and Pride: Provides festivals, storytelling circles, and cultural showcases encompassing identity and culture

• Economic Independence: Provide programs and tools to help families build economic independence including job training, business consulting, and financial literacy programs

• First-Generation Student Scholarship and Mentorship: Offer academic and scholarship resources, college readiness support, and mentorship to help students achieve their educational goals

• Information and Resources: Connect families to culturally competent resources in health care and mental health services

• Immigration and Legal Education: Offers information to immigrant families about immigration and provide referrals to legal resources

Network of Mental Health professionals and associates throughout the state of Minnesota, that offers memberships for mental health practitioners interested to be part of the network, to connect, share ideas, and resources to organize and offer conferences; workshops; and online groups on matters of psychology.? Helps refugees and immigrants overcome language, cultural, and economic barriers to accessing health care and health information. Services include health education, assistance with health insurance, navigation of health and social support systems, and culturally responsive referrals.

Provides anonymous counseling for older adolescents, individual adults, couples, and families. Particular focus is placed on those who lack access to care, insurance, or who can't wait for appointments. Professional clinicians who are volunteers provide help for anxiety, concerns about loved ones, depression, grief, relationship problems, substance abuse, and trauma. Services may include:

• Hearth Connection Homeless Program/Adult Mental Health Targeted Case Management (AMH-TCM): For people who have long histories of homelessness that are interested in receiving and engaging in supportive services to help end their homeless situation. Head of household ages 18 and older must have been homeless continuously for a year or more or have experienced 4 episodes of homelessness in the past 3 years. Participants work with a case manager to find and keep affordable housing and meet other personal goals addressing areas that have contributed to homelessness. The caseworker acts as a referral source, advocate and support person for the participant, helping connect them with resources and programs. Housing subsidy vouchers are awarded based on regional availability. Applicants must complete a VI-SPDAT assessment through Coordinated Entry first before accessing this program.

• Prepetition screening for all individuals petitioned for civil commitments for a mental illness due to being a danger to themselves or others

Provides assessment, intake, information and referral, and case management for adults with mental health needs. Case management services address individual needs related to diagnoses such as:

• Acute/situational depression

• Bipolar disorder

• Borderline personality disorder

• Major depression

• Schizophrenia

Additional services may include case planning, consultation, crisis intervention, and referral to legal commitments or guardianship/conservatorship.
