Mental Health Navigator - Central Minnesota Mental Health Center - CMMHC
A confidential phone line is staffed by a Mental Health Navigator specifically trained to guide individuals to mental health resources. The Mental Health Navigator listens to an individual's story. Then, basic background information like age, location and insurance information (if any) is gathered to help narrow down service options. From there, the Mental Health Navigator searches local resources to help determine treatment options and community service providers to meet the individual's needs. Resources include therapists, clinics, detox programs, peer support, and other opportunities within Central Minnesota Mental Health Center's programs and in the community.

Provides basic and in home support services for older adults and people with disabilities in their homes or in a community setting to ensure the health and safety of the person and maintain their independence within the community.
Basic Support Services
- Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision, or physical assistance to remain in their own homes.
- 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency.
- Companion services: Non-medical care, supervision and socialization to a person age 18 or older.
- Night supervision: Provides overnight assistance and monitoring by an awake staff in the person's home.
- Respite care services: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

- Positive support services: Services to increase positive behavior and decrease or eliminate severe, challenging behavior.

Licensed mental health professionals complete diagnostic assessments working to understand questions, concerns and worries as well as each child's strengths, challenges, and history.
Washburn provides counseling, therapy and support for families available at all of their sites. This includes help with with common concerns such as:
- Abuse and parent-child conflicts
- Attention problems and acting-out behaviors
- Depression and anxiety
- Difficulty dealing with loss or family changes
- School adjustment problems
Quick Access Program: Provided in partnership with Medica. Supports children experiencing mild mental health challenges or situational life stressors. Provides a brief mental health assessment and access to clinical care for kids ages 5 - 17 who can benefit from short-term therapy intervention (up to 6 sessions). Aims to reduce the amount of wait time it takes for children and families to receive help with mild mental health challenges or situational life stressors. Available to families enrolled with Medica insurance. Services are not for children or families who have long-standing mental health concerns, a history of past mental health treatment and multiple diagnosis, or safety concerns.

- Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution.
- Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy.
- Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community.
- Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals.
- Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people.
- Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently
- Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Also, supports student youth as they search for and find competitive employment and helps them explore post-secondary education.
