Provides volunteer opportunities for older adults age 55 or older

Financial Assistance Program: For patients whose insurance coverage and/or ability to pay medical bills is limited. Provides a discount on bills for emergent and medically necessary care. The program may cover current and outstanding bills and may remain in effect for 12 months after enrollment. Uninsured Discounts: Patients without insurance coverage are eligible for a discount for medically necessary services

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers.

Program helps qualified patients access emergency services and medically necessary care when they need help paying for these services.

• Art job, opportunities, and paid internships listings for those seeking employment in the arts

• Attorney referrals for artists through the Minnesota chapter of the Volunteer Lawyers for the Arts

• Career consultations for artists; information on copyright law, marketing, career planning, taxes, grants and funding to artists

• Fiscal Sponsorship and Incubator Program: Provides fiscal sponsorship for arts groups who are not ready to become nonprofits and assists them in managing their financial affairs professionally

• Free scanner access and photo documentation equipment onsite in the resource center for artists by appointment

• Work of Art Business Skills for Artists Workshops with topics such as Career Planning for Artists, Time Management for Creative People, Professional Presentation, Marketing for Artists, Legal Considerations for Artists, Recordkeeping, and Financial Management for Artists

Once enrolled in the Parent Support Outreach Program help is provided to at-risk families to avoid future incidents of child maltreatment and to enhance child and family well-being, including:

• Comprehensive assessments, early intervention, outreach, and supportive and therapeutic services

• Connect families with county and community-based services

• Limited financial assistance for families to meet basic needs

Provide for the immediate disposition or direct cremation of a deceased resident of Lake County who is otherwise unable to pay the cost of disposition of their remains.

Provides support for mothers in their breastfeeding journey to reach their breastfeeding goals. Services include over-the-phone or in-person support and connecting with community resources.

Provides Family, Friend, and Neighbor (FFN) caregivers with resources, supports, and opportunities to continue taking care of children in the best way they can. FFN caregivers include grandparents, aunts, uncles, elders, older siblings, friends, neighbors, and others who provide informal child care. FFN caregivers care for a small number of children in their home and are typically legally exempt from regulatory or licensing requirements. Many FFN caregivers also provide child care at no charge, and some are paid privately.

Provides case management to adults with serious and persistent mental illness and children with serious emotional disorders Case management services are designed to help individuals gain access to needed medical, social, educational, financial, and other services and to coordinate and monitor the delivery of these needed services

Provides investigation and intervention services on behalf of vulnerable adults who are unable to act on their own behalf, manage their own affairs, or who are in immediate danger of abuse. Types of abuse may be physical, emotional, sexual abuse, unsafe or hazardous living conditions, exploitation, neglect, or abandonment. Services include:

• Taking reports and investigating abuse and/or maltreatment

• Removal of the individual to safer surroundings

• Services necessary to remove the conditions which have created a threat to life

NOTE: Complaints, questions, or concerns involving incidents in facilities licensed by the Health Department, such as hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living and home health agencies can be referred to the Minnesota Board on Aging Ombudsman for Long-Term Care.

Homestretch Home Buyer Education Program: A homebuyer education program that provides information on home purchasing loan options, purchase agreements, loan closings, budgeting, and home maintenance after the purchase. After class completion, a certificate is obtained which may enable the participant to apply for certain loan programs. MHFA Rehabilitation Loan: Income-based deferred loans for improvements for basic, necessary housing repair to owner-occupied properties. There is no interest or monthly payment until the owner sells, transfer or move from the property or until after the 15th year when the deferred loan is due and payable. Loans is also available to mobile/manufactured homes taxed as personal property located in a mobile home park. Small Cities Development Program Rehab Loans: Income-based deferred grants to low to moderate-income households living in substandard housing. The program encourages the improvement of the general appearance and value of the housing stock while making a visible and substantial improvement in the quality of life in the community. Funds are also available to commercial and rental property owners. This program can provide upgrades to properties, helping them meet code requirements and design standards. Small Cities' grants require a match from the owner of the property. Most grants are written as a 70-80% grant funds and a 20-30% owner match for project funding. There is a maximum amount of funds that a project can receive as well. Prairie Five will assist in determining the match and other funding sources or options that may be available to fund the match requirement, if needed.
