Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. Services include:

• Assessing individuals who need help paying for a substance use assessment or treatment services through the Behavioral Health Fund

• Referral to local community service providers for a substance use assessment or treatment services. Individuals may go to any provider of their own choosing for the assessment and/or treatment.

Provides temporary financial assistance for refugees and eligible entrants while they are in the process of resettling in the United States. NOTE: Able-bodied RCA recipients are required to participate in an employment and training program. RCA recipients who are disabled or 65 years of age or older are required to apply for SSI.

Contracts with Change Healthcare to assist patients with Minnesota Healthcare Program or Hennepin Healthcare Charity Care applications. Representatives can be accessed at the Clinic & Specialty Center, Whittier Clinic, or Brooklyn Park Clinic. MNsure Enrollment On the MNsure marketplace, individuals can find, compare, and choose healthcare coverage, including commercial health insurance plans and low-cost or free plans through government programs. A financial counselor can help complete the online application. If eligible for a government program such as Medical Assistance or Minnesota Care, individuals may enroll through MNsure year-round. Uncompensated Care Enrollment If not eligible for coverage through MNsure, individuals may qualify for the Uncompensated Care discount program. The program provides discounted care based on family size and annual household income. A financial counselor can answer program questions and help complete the application. Probation and parole supervision for juvenile offenders and adult misdemeanor offenders who reside in or are convicted in Sibley County. Within this office, the MN Department Of Corrections provides adult felony probation and gross misdemeanor supervised release supervision to Sibley County.

Residents are expected to adhere to house rules and encouraged to participate in programs, which can range from minimal involvement such as case management to full participation in the Intensive Outpatient Program (IOP). The housing program accommodates from low level offenders to level 3 offenders offering support and rehabilitation to all individuals seeking services.

Helps people without children who can't work enough to support themselves, and whose income and resources are very low, to pay for basic needs. The maximum monthly benefit is:

• $128 for a person living in a licensed residential facility or receiving substance use disorder treatment services

• $350 for a person living in the community

• Emergency funds may be available if the individual cannot pay for basic needs, such as shelter or food, and their health or safety is at risk because of this

People who get GA are also eligible for help with medical and food costs through Medical Assistance - MA and the Supplemental Nutrition Assistance Program - SNAP.

The Food Support Program assists low-income persons and families so they can purchase food and better meet their nutritional needs. The program is not intended to supply all of a person's or family's food needs. Instead, it helps while the person or family makes an effort to become self-supporting.
