• An energy auditor will evaluate and determine what improvements will be completed by a licensed private contractor for eligible homeowners and renters experiencing low income.

• Weatherization measures may include insulation and infiltration work, furnace maintenance, venting of water heaters, heating and exhaust systems, and installation of smoke detectors and carbon monoxide detectors when needed. This work is done as a one-time improvement to the dwelling.

Offers a nurse line available 24 hours/7 days a week for individuals who may need to speak with a nurse about medical care and where to go for care. Nurses can provide medical advice on what potential options are available, such as virtual care, primary care, urgent care options, or seek emergency care if needed at a Johnson Memorial Health Services location

Support groups and education:

• Celebrate Recovery: A christ-centered program offering hope and healing for a variety of issues, including anxiety, anger, divorce, food issues, codependency, relationship problems, alcohol/drug addiction, and more; meets Tuesdays

• DivorceCare Group: A compassionate support group designed to help individuals heal from the pain of separation and divorce; meets Tuesdays

• Family Grace (Mental Health) Group: A 16-week small group designed to provide confidential, nonjudgemental, and inclusive community for the family and friends of those facing any type of mental health issue; meets Tuesdays

• Job Transition Support Group: For anyone unemployed, underemployed, or seeking a new opportunity; meets Mondays

• Life Groups: Provides a sense of belonging and community, spiritual growth, and a supportive environment; meetings vary

• Re-Engage Marriage Class: For marriage couples looking to find encouragement and support from others in a group setting to strengthen their marriage; meets Wednesdays

Residential treatment for chemically dependent men, primarily for persons with adjustment difficulties and/or alcohol problems. Services provided in the program may include:

• Alcoholics/Narcotics Anonymous

• Anger management

• Breathalyzing and random drug screening

• Chemical dependency education

• Christian 12-step groups

• GED assistance

• Individual and group counseling

• Relapse prevention classes

• Work Therapy (8 hours per day, 5 days per week)

Minnesota's version of TANF (Temporary Assistance for Needy Families). Statewide MFIP helps families work their way out of poverty by providing temporary cash and food benefits to eligible families. Statewide MFIP has a 60-month limit, there are some limited exceptions and extensions to the time limit. Participants are required to follow certain work rules. Failure to meet work rules will result in their MFIP grants being sanctioned or reduced. In addition:

• Child support will be pursued in those situations where one or both parents are absent.

• A woman expecting a baby may receive assistance for herself during her pregnancy

• Children who are being cared for by certain relatives may receive assistance

?NOTE: MFIP recipients may be eligible for Emergency Assistance, Child Care Assistance, Medical Assistance, and Supplemental Nutrition Assistance Program (SNAP).

Adult Field Services include the following:- Administers the completion of court-ordered community service and the payment of restitution on misdemeanor, gross misdemeanor, and felony probation cases. Provides information on Hennepin County adult probationers.

• Oversees the Adult Sentencing to Service Program, which allows offenders to complete court-ordered obligations and learn new work skills. STS Adults program call 612-596-9220

• Screens persons booked into the Adult Detention Center for release eligibility

• Supervises adults conditionally released on pretrial status

• Prepares presentence reports for the district court concerning felony, misdemeanor, and gross misdemeanor offenders

• Provides counseling to defendants and monitoring of compliance with court-ordered conditions

• Provides parole and supervised release for Hennepin County adult offenders

• Provides probation supervision for adult offenders who are referred by the 4th Judicial District Court

Provides police services for the community of Maple Grove.

Provides state-funded food assistance to legal noncitizens ages 50 or older who do not qualify for the Supplemental Nutrition Assistance Program (SNAP, formerly called Food Stamps or Food Support) because of their citizenship status. Monthly benefits are loaded on an Electronic Benefit Transfer (EBT) card, which looks and works like a debit card. The card can be used to buy groceries at authorized stores, farmers markets, and online retailers. Learn more about where to shop and what can be bought. The amount of benefits received is based on income, expenses, and number of people in the household.

Provides outpatient chemical dependency treatment services to youth struggling with chemical dependency and mental health disorders. Offers services that are evidence-based practice models by addressing both mental health and substance use through collaborative treatment plans. Prevention 4 Teens is an education-based substance use prevention program for youth who are experiencing problems associated with alcohol or other drug use but do not meet diagnostic criteria for substance use disorder or require more formal interventions. Offers guidance, education, and support for families through two sessions with a counselor.

Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. County provides assessment for SUD funding for eligible persons. Chemical health services referral provided for:

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers. • Under both programs Part B premiums are put back into the monthly Social Security check, making participants' monthly income increase.

• The QMB program will pay the Medicare co-payments and deductibles. For example, if enrolled in the QMB program, the participant does not have to pay the annual part B deductible, the 20% part B co-payment, or the part A hospital deductible

• The participant pays for Medicare services that aren't covered, such as routine physicals and outpatient prescription drugs.

Cash-grant program for adults with low incomes experiencing a household emergency. Adults can receive assistance once every 12 months. The assistance received must resolve the crisis. Adults must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. Emergency General Assistance provides short-term, one-time assistance for persons in a financial crisis because of fire, flood, storm, illness, accident, theft, utility shut-off, eviction, foreclosure, or other emergency need.

Provides part-time paid work experience in community services. Enrollees work 20 hours per week and are paid the state or federal minimum wage, whichever is higher. Following a period of on-the-job training, host work sites are encouraged to hire their enrollee.

• Short-term support counseling for individuals who have been sexually assaulted, including individuals used in prostitution and sex trafficking. Counseling is also available for family members and friends of sexual violence victims.

• One-to-one peer counseling

• Hospital Counseling/Advocacy: Counselors are available to provide counseling, offer information, provide options, and support sexual assault victims at the hospital when they come in for an evidentiary exam. If the victim wishes to make a report to law enforcement, the counselor will remain to provide support. If, upon arriving at the hospital, the sexual assault advocate is told that the victim does not wish to have an advocate, a packet of written information will be left for the victim, and the advocate will leave. Advocates provide support to family and friends of victims in the hospital.
