Minnesota SAGE Program: Provides free breast and cervical cancer screening and follow-up services to uninsured and underinsured women over the age of 40.

The family medicine team provides primary care services to your whole family. Offering expectant mothers for OB checkups throughout pregnancy to regular physical.

Provides health care in a "health home" approach designed to create an ongoing relationship with a primary care provider (PCP); focus on the whole individual rather than a single symptom; enable a PCP to partner with other specialists, health care services, and community services if needed; include individuals and their families to be involved in decisions; work together to plan care based on the patient's needs. • Access to Benefits: Community representatives can help individuals move towards self-sufficiency or access community benefits and support. Representatives can help with MNsure applications, Open Access voicemail, SNAP applications, Social Security applications, Veteran's benefits, and more. Schedules may vary when community representatives are onsite.

• Hot meals available which include breakfast and lunch throughout the week and coffee daily

• Laundry services available to current residents of the shelter program

Additionally, gives access to case managers on-site to help connect youth to education, employment opportunities, health and wellness programs, stable housing, and more. Also provides a 12-unit transitional housing facility in Coon Rapids that offers supportive services to young people who are at risk of or experiencing homelessness.
