Waivered Services: - Available to persons with a variety of disabilities - Waivered services are flexible and can be provided in a variety of settings - CADI (Community Alternatives for Persons with Disabilities) Waiver is available to adults and children with disabilities who need a nursing facility level of care: - TBIW (Traumatic Brain Injury Waiver) is available to adults and children who have had severe brain injuries from an accident, or due to stroke, heart attack or disease - MR/RC (Mental Retardation and Related Conditions) Waiver provides supports to adults and children with developmental disabilities - EW (Elderly Waiver) provides supports to seniors at home or in adult foster care who would otherwise have to live in a nursing home - AC (Alternative Care) Waiver provides supports to seniors at home or in adult foster care who would otherwise have to live in a nursing home, who because of their assets don't qualify for traditional Medical Assistance programs. Waivers pay for services such as personal care assistants, independent living skills counseling, homemaking, respite care, in-home family support, and other services Personal Care Attendants: - Provide assistance with daily living activities such as hygiene, grooming, dressing, taking medications, simple housekeeping, and other tasks - PCAs also provide behavioral monitoring and supervision to help clients with behavior problems - PCAs can help clients with FAS to live in a variety of settings, including foster care, their own homes and apartments, and family homes Other services: - In-home family supports and respite RSI provides support to caregivers and families of people with disabilities - Respite services gives caregivers and families a break for a few hours to a few days at a time. Respite can be provided in or outside of the family home - In-Home family supports services helps adults and children with daily life management skills, such as self-care, socialization, sensory/motor development, behavior management, community living and healthy recreation. - Respite and in-home family supports are waivered services programs - Adults and children who do not qualify for the MR/RC waiver may be eligible to receive similar kinds of help through the CADI or PCA programs

Individuals receiving services have 1:1 staffing. Flexible based on individual needs. Services are provided in the family home, at a Hiawatha location or elsewhere in the community. Activities are customized to the individual's wants and needs. Examples of activities are similar to those of the LEEP program, but may also include: - Assistance in getting up and ready for school/work - Assistance with homework - Participation in evening or weekend leisure activities Life Enrichment and Exploration Program - LEEP For older school aged youth and adults who have disabilities. Programs runs Monday - Friday after school, non-school days, after day programs and during the summer. Provides a supervised environment with structured learning and recreational activities. Activities may include: - Crafts and cooking - Domestic training - Personal safety training - Social skills training - Summer field trips Respite Offers temporary care within the community based homes for individuals whose caregivers may need a helping hand. This is offered to individuals who are currently living at home but on occasion the families may need a break. This offers families the opportunity to spend some quality time with their other children, covers assistance in family emergencies, gives families an opportunity to travel for work or leisure, and assures the families that their child is safe and is enjoying their time away also.

Individualized Home Supports without Training - Services for people who live in their own home or family home that provide support in the following community living service categories: - Community participation - Health, safety, and wellness - Household management - Adaptive skills - Complimentary support plan (SP) - Informal and formal person-centered outcomes *Designed for adults or children who require support in one of the above-mentioned service categories. Specialist Services - Provision services that extend past the scope and duration of available waiver and state plan services including: - Assessments/individual program plan and program development - Training and supervision of staff - Monitoring of specific program implementation Respite - Short-term care services provided to a person when their primary caregiver is absent or needs relief. Services include: - In-home respite: Service is provided to a person in their home or the family home. - Out-of-home respite: Service is provided to a person in a licensed or unlicensed setting that is not the person's home. - Respite level A: Active, out in the community - Respite level B: Stay at home and local outings only - Respite level C: Sleep in - Respite level D: 24-hour period Homemaker Services - Services that help a person manage general cleaning and household activities. Services include: - Cleaning: light housekeeping tasks, laundry services - Home management: cleaning, transportation arrangement, meal preparation, shopping for food, and household items - Assistance with ADLs: cleaning, assistance with ADLs (ambulating, bathing, dressing, eating, grooming, toileting) *Training is not covered under this service. To receive homemaker services, homemaker cleaning must be the primary service provided. County-Funded Semi-Independent Living Services (SILS) - Services needed to maintain and improve an adult's capacity to live in the community. Supports people in ways that enable them to achieve personally desired outcomes and lead self-directed lives including: - Money management - Meal preparation - Shopping - Obtaining and maintaining a home - Self administration of medication - Formal skill building

Provides basic and in home support services for older adults and people with disabilities in their homes or in a community setting to ensure the health and safety of the person and maintain their independence within the community. Basic Support Services - Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision, or physical assistance to remain in their own homes. - 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person's home due to a health/personal emergency. - Companion services: Non-medical care, supervision and socialization to a person age 18 or older. - Night supervision: Provides overnight assistance and monitoring by an awake staff in the person's home. - Respite care services: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care. Intensive Support Services - Positive support services: Services to increase positive behavior and decrease or eliminate severe, challenging behavior. - Specialist services: Services designed to promote staff and caregiver competency to meet a person's needs in eligible areas. - Independent living skills training: Services that develop, maintain and improve the community-living skills of a person. - Semi-independent living skills: Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community. - Residential-based habilitation: Including in-home family support and supported living services for adults in their own home: Supported living services provided in a licensed site, including a foster care home, a community residential setting, or a supervised living facility.
