Independent Living Skills - Lutheran Social Service - Northeast Region

Independent Living Skills (ILS) Group Training: Youth meet weekly to discuss topics such as goal setting, employment, housing options, post-secondary education, and money management. Youth attending the 18-week Independent Living Skills program (60 curriculum hours) may be eligible for 0.5 high school credits. Transition Planning: Individual transition planning services for youth that will age out of care into independence; provides one-on-one case management services. Leadership and Community Development: Leadership opportunities are offered through our Youth Leadership Council. The Duluth Youth Leadership Council is a group of current and foster youth aged 16-24 who plan quarterly projects to raise awareness of issues in the foster care system and advocate for youth currently in care.

Direct Access allows an individual to go directly to a provider they choose to receive a comprehensive assessment and access care immediately. The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Health Services - Le Sueur County Public Health

- Blood pressure screening - Breastfeeding mothers support - Case management and care coordination - Child and teen checkup outreach - Family planning services - Financial assistance available for prenatal care, pelvic exams, PAP smears, pregnancy testing, etc. - Follow Along Program: Home visits for newborns, including questionnaires sent to the home until child is four years old. If something on a questionnaire causes concern, a nurse will visit the home and take action if appropriate. - Food recall information - Home health care including home nursing with blood pressure screening, diabetes education for newly diagnosed patients, instructions for new diets, and monitoring of medication administration. Home Health Aides assist with bathing and personal care, meal preparation, and assistance with hair and oral care. Physical therapists, occupational therapists, and speech therapists provide services also. - Immunizations: available for children and young adults, birth through age 18. Hepatitis B and Tdap for a fee for adults. Flu shots available from October through December. - Mantoux testing (tuberculosis) - Loan closet - Maternal and child health (car seats available for low-income families) - Prenatal care and education including safety and proper use of car seats - School health nurses consult with teachers on topics such as AIDS, teen pregnancy prevention, self-breast exams, and nutrition. Preschool screenings as well as immunization, vision, and hearing clinics.

Children's Mental Health Targeted Case Management Services - Ross Resources, LTD

- CMH-TCM, are activities that are designed to help a child and their families coordinate needed mental health services. Case Management services include assisting a family in accessing services including, social services, educational services, health services, recreation services and related services in the area of volunteering, advocacy, transportation and legal services. Case Managers assist families in obtaining a comprehensive diagnostic assessment to determine eligibility. Once determined, Case Managers assist families in developing an Individual Family Community Support Plan that assures continuity of care. Case Managers are required to consistently assess and reassess the delivery, appropriateness and effectiveness of services over time. - A child is determined to be eligible for case management services by the county if they have a mental health diagnosis that is causing interference in multiple life areas (school, home, peers and community). The mental health diagnosis could include ADHD, anxiety disorder, depression, oppositional defiant disorder, etc. CMH-TCM is a voluntary service offered to families to assist with service coordination. - Adolescents ages 5 - 18 are eligible for services, and services can continue for youth over age 18 if they choose and they are still enrolled in school. CMH-TCM services can continue into the adult mental health system if needed and the case manager can assist in that transition process. - There is no charge to families for this service; CMH-TCM can be billed through MA, IM Care and private insurance.
