Medical Model Children's Advocacy Center (8 am - 4:30 pm)

• Child-focused, facility-based program that conducts forensic interviews and forensic medical exams on pediatric and adolescent patients. Representatives from many disciplines (law enforcement, child protection, prosecution, mental health, medical and victim advocacy) work together, making team decisions about the investigation, treatment, management, and procescution of child abuse cases. Sexual Assault Nurse Examiner (SANE) Program

• The SANE program is available 24-hours a day, 7 days a week at Sanford Health, Bemidji Emergency Department and RiverView Health, Crookston Emergency Department. Care provided by a Sexual Assault Nurse Examiner can include, as appropriate:

• Evidentiary examination - Referrals - Emergency contraception - Sexually Transmitted Infections prophylactic treatment

• Pre-trial and pre-sentence investigation/supervision
• Risk assessments
• Mental health, drug, DWI, and veteran's courts

Assists individuals and families experiencing a crisis by providing:

• Adult and child mental health mobile crisis response services. Interventions are face-to-face, short-term, intensive mental health services.

Staff are mental health professionals and practitioners which:

• Can refer to other county social service programs for ongoing support

• Help individuals navigate the health care system to find services and resources

• Provide counseling and support for individuals and families to manage a crisis

• Support individuals experiencing a crisis identify and utilize better coping skills

Offers immunizations for children/youth and adults. Services include childhood immunizations and adolescent/adult immunizations including:

Helps older adolescents who "age out" of out-of-home placement prepare for the transition to independent living. Group meets twice a month during the school year and weekly in the summer. The program provides:

• Budgeting assistance and education

• Career exploration and vocational assessment

• College application fees

• Driver's training

• Employment skills training

• Financial assistance for damage and utility deposits, or rent on apartments or dormitory rooms (only for youth who "age out" of placement at age 18 or older)

• Group and individual life skills training

• Household supplies for a first apartment

• Retreats and wilderness trips

• Supervised 24 hours/7 days a week living arrangements

• No more than 5 adults in family setting

Art and theater classes and workshops including painting, drawing, photography, jewelry making, glass art, pottery, and sculpture.

Connects and provides older adults and their caregivers with services in their community to allow them to live long, healthy, and independent lives in their own homes. Services include:

• Food Assistance: Congregate dining locations offering low-cost meals, low-cost frozen meals, home-delivered meals, NAPS, and SNAP

• Healthy Living: Blood pressure checks, fall prevention screenings, telehealth conferences, Tai Chi and yoga sessions, and stretching/balance education

• Information Services: Offers memory loss education and Dementia Friends, Medicare counseling and assistance, fraud/scam identification and prevention, assistance with internet search, assistance with Healthcare Directives and Power of Attorney forms, and classes to older adults

• Mobile Community Center: Brings services and socialization to the door or community. The vehicle is equipped with high-speed internet, flat-screen TV, applications for programs, and meeting space for coffee and conversation.

• Social: Offers card parties, coffee and conversation time, caregiver respite, karaoke parties, video calls and chats with friends, and social media assistance

Offers outpatient therapy services integrated into the resident's routine for those who have been discharged from an inpatient care but need continued therapeutic support. Outpatient services provide flexibility and continuity of care, helping individuals maintain or improve their functional abilities while remaining in their own homes. The types of therapies offered include occupational, physical, speech, and more. Treatment includes:

• Adaptive living techniques, joint protection, and motor coordination

• Cognitive, visual, and perceptual retraining

• Comprehensive home programs and patient education

• Specific exercises to restore motion, restore strength, and minimize pain

• Speech and language pathology

• Strengthening muscle groups to restore large and fine motor skills

• Prosthetic training for enhanced mobility

Provides a variety of classes and activities for community members of all ages including:

• Adult enrichment classes

• Adult recreational sports

• Exercise classes

Patients who need help paying their bill or prescriptions can call to discuss special circumstances or request a payment plan

• Allina Partners Care Program: Provides assistance to patients who do not qualify for Medical Assistance or MinnesotaCare in Minnesota, or BadgerCare in Wisconsin, and whose annual incomes are at or below 275 percent of the federal poverty level

• Uninsured Discount Program: Provides a discount on medically necessary hospital charges for individuals who do not qualify for Medicaid or Allina Health Partners Care

• MedCredit Financial Services: Provides a financing option to patients who can't afford to pay high monthly payments for their medical bills over a short period of time

• Prescription Assistance Program: Patients can speak to staff or contact an Allina Health health care provider to send a referral letter to the program

• BabyLine: An assistance line that is staffed with Ob/Gyn nurses available 24/7, 365 days a year, with a focus on expectant and new parents, up to six weeks after delivery.

• Car Seat Safety Program: Learn how to use a car seat the right way and receive a convertible car seat or booster seat. Members ages eight and younger or women who are at least six months pregnant are eligible. Call Member Services to find a car seat education provider.

• Nurse Navigator Program: An assistance line that is staffed with professionals who can help answer questions regarding understanding health care and benefits and how to choose a treatment option. Call member services for assistance.

Minnesota's version of TANF (Temporary Assistance for Needy Families). Statewide MFIP helps families work their way out of poverty by providing temporary cash and food benefits to eligible families. Statewide MFIP has a 60-month limit, there are some limited exceptions and extensions to the time limit. Participants are required to follow certain work rules. Failure to meet work rules will result in their MFIP grants being sanctioned or reduced. In addition:

• Child support will be pursued in those situations where one or both parents are absent

• Children who are being cared for by certain relatives may receive assistance?class

• Individuals expecting a baby may receive assistance for themselves during their pregnancy

NOTE: MFIP recipients may be eligible for Emergency Assistance, Child Care Assistance, Medical Assistance, and Supplemental Nutrition Assistance Program (SNAP).

Provides school-based mental health services for students identified by their school as needing additional support beyond what the school offers as part of their education system. School-based therapists provides counseling services onsite in the school setting, removing barriers of access for children and families. With early identification, evaluation, and treatment, children and adolescents living with mental illness can achieve success in family life, school, and work.

Provides advice over the phone on juvenile court issues, including:

• Child safety cases

• CHIPS (Child in Need of Protection or Services) petition

• Terminations of parental rights
