Provides skilled nursing care for individuals who require transitional care after surgery, illness, or injury. Services include:

Patients who need to speak with a financial counselor, need cost of care estimates or determine if they may qualify for financial assistance to pay for procedures may reach out about options. Financial counselors can help individuals understand patients' responsibilities and may offer no-interest payment plans. Program designed to meet the specific educational and employment needs of youth. Services include:

• Education: Assists with obtaining a social security card, birth certificate, and/or state ID, application assistance for food stamps, Medical Assistance, or financial aid, advocating at school, connecting with GED tutors, and preparing for college applications. Financial assistance for obtaining vital documents is available.

• Employment: Assists all youth with online job searches, applications, resumes, and practice interviewing. Providing work appropriate clothing, transportation in the form of bus passes and bus tokens to get to and from work and for interviews and Lyft/Uber for a job interview, and referrals to partner organizations to find other employment resources. YouthLink partners with HIRED/Goodwill Easter Seals.

• Housing for families of children who are undergoing treatment for cancer and other life-threatening illnesses

• Families assigned their own room. Each family supplies its own food

• Outside and inside play area, living rooms, recreation rooms, and kitchens

• University hospitals provide shuttle service

• Volunteers provide rides to grocery stores, Target, and perform many other helpful duties

Offers a blend of individual and group therapy, nutritional guidance, and psychiatric care tailored to support adolescents and adults in their recovery from eating disorders. Outpatient treatment: Provides one on one appointments with therapists and registered dietitians. Primary care clinician and psychiatrist sessions are available to patients when needed, and group sessions may be recommended to supplement treatment. Intensive outpatient treatment: Provides intensive treatment program spanning four to six weeks, including twice weekly three-hour group sessions focused on developing coping skills and psychotherapy. Supported meals by dietitians are provided to participants on group session days. In addition to group programming, patients continue to have individual appointments with their therapist, registered dietitian, and primary care clinician.

Financial counselors can help individuals understand their financial responsibility, provide cost estimates, and discuss payment options such as no-interest payment plans. Other options may include: Charity Care / Financial Assistance: Individuals who are uninsured or underinsured may qualify for financial assistance based on income. Patients with household incomes below 200% of the Federal Poverty Guidelines may qualify for a full discount. In comparison, those with incomes between 200% and 300% may qualify for a partial discount on medical bills.

Provides religious education

Habilitation: provides training and vocational skills so that clients may eventually be able to work in the community in competitive employment.

• Trains and equips individuals with disabilities for useful employment.

• Assists persons with disabilities in attaining individual physical, social, emotional, and vocational goals.

The Supplemental Nutrition Assistance Program Employment and Training (SNAP E&T) program provides free employment and education training and support for people getting SNAP benefits. SNAP E&T participants gain support, skills, training, or experience to reach their goals. Many organizations, including technical and community colleges, community-based organizations, county agencies, tribes, and CareerForce Centers provide SNAP E&T programs and services. Programs can include career planning, job search, training for in-demand jobs, student support, financial support, and continued support once a participant has a job.

Change Step: A program for military service members and veterans who have used abusive behaviors with their intimate partners. Change Step helps end abusive behaviors and replace them with actions that build and support healthy relationships. It is developed specifically for military service members and veterans of the armed services and incorporates the impact of military culture, deployment, PTSD, resulting in separation from family, and the effects of combat. Intervention and Prevention Group Therapy: for adults who are interested in addressing the use of violence in intimate and familial relationships. Hosting 5 groups, three days a week (2 in the morning, 3 in the evening) for individuals who are currently or have a history of used violence in relationships. This program is 24 group sessions plus periodic individual psychotherapy sessions to address program and goal progression and reflection. Transitional Age Youth (TAY) Intervention and Prevention Group Therapy: services are offered through a partnership with juvenile probation. Eligible participants are ages 18 - 24 years old and are referred through juvenile probation, the program is 24 weeks long.

Offers three community centers for members of the Fond Du Lac band. All facilities feature a gymnasium, weight room, and library. The Cloquet Community Center offers a pool and hot tub. The community centers provide fun and educational activities throughout the year. Activities offered:

Multi-generational recreational and fitness-based activities including community and after school enrichment programs, community computers, fitness classes, pickle ball courts, and walking programs.

A state and federally funded program for expectant mothers and children ages birth - 5 years old. The program offers comprehensive services to enrolled children and families, giving priority to limited-income families and children with special needs. Head Start programs offer a nurturing environment that supports the healthy growth and development of each child in the context of family, culture, and community.

• Financial Assistance Program: For patients whose insurance coverage and/or ability to pay medical bills is limited. Provides a discount on bills for emergent and medically necessary care. The program may cover current and outstanding bills and may remain in effect for 12 months after enrollment.

Free, confidential 24 hours/7 days a week referral and information service for individuals and families facing mental and/or substance use disorders Provides referrals to local treatment facilities, support groups, and a community-based organization. Callers can also order free publications and other information. Behavioral health treatment services locator for persons seeking treatment facilities in the United States. NOTE: Does not provide counseling services. Specialists will answer calls and transfer calls to state services or other appropriate intake centers in their state.

Provides supportive services working in partnership with the Minnesota Department of Health's Safe Harbor and other federal agencies for youth aged 28 and under in the Twin Cities metro area and surrounding suburbs who are at risk of or already experiencing sexual violence or trauma.

• Provides prevention, intervention, and aftercare supportive services for any young person aged 28 and under in the seven-county metro area who has experienced or is at risk of experiencing various levels of violence and trauma, focusing on sexual exploitation/sex trafficking.

• Gender-specific services for female/femme/trans youth age 17 and under in Ramsey, Hennepin, and Anoka counties offering one-on-one supportive services to young people who are justice system involved, experiencing sexual exploitation/human trafficking, or who are at high risk.

• Provides one-on-one life coaching and mentoring support aimed at helping assess and meet basic needs and goals, including strength-based, harm reduction, non-judgmental, and trauma-informed best practices

• Provides group-based education aimed at decreasing risk factors and increasing protective factors/resilience to prevent human trafficking and other forms of sexual trauma or domestic violence, and promotes healthy relationships

• Provides opportunities for youth to safely engage in pro-social activities with peers, such as camping and outdoor activities, field trips, and skill-building workshops

• Culturally specific services for Native youth aged 24 and under in the seven county metro area who have experienced exploitation or trafficking including one-on-one supportive services, opportunities to connect or reconnect to traditional healing and cutural practices, and to engaged in culturally specific pro-social activities, events, and workshops.

Provides nursing home care services, including:

• Inpatient rehabilitation, including speech, physical, and occupational therapies

• Short-term and long-term skilled nursing care in all private rooms

The Food Support Program assists low-income persons and families so they can purchase food and better meet their nutritional needs. The program is not intended to supply all of a person's or family's food needs. Instead, it helps while the person or family makes an effort to become self-supporting.
