Offers groceries for those in need once a month. Amount provided depends on size of family. Clients are able to choose which food they want. Food donations are accepted.

The program helps people with disabilities learn new and healthy independent living skills. Staff teach new skills with hands-on instructions and assistance, delivered in the home or community with transport provided.

This is a Medicaid program for beneficiaries who need help in paying for Medicare Part B premiums. For those who qualify, the Medicaid program pays full Medicare Part B premiums only. Eligibility begins the month of application and, if eligible, retroactively for up to 3 months. Note: There can be a lag time before SSA is able to stop taking the premiums out of the recipient's benefit check but the recipient will be reimbursed by SSA for all months eligible for SLMB and/or QMB or QI. The reverse is true when these programs terminate: recipients should be told to put aside premium costs from first month of termination on.

Provides religious education

Provides employment and training services to workers laid off from their jobs due to no fault of their own. Services include:

• Career planning and personal counseling for career decision making, developing job goals, and personal planning

• Job-seeking skills workshops on preparing resumes and cover letters, learning and practicing interviewing skills, internet job search, and networking

• Placement services, including leads developed by counselors and agencies

• Support services in certain cases are available for expenses such as child care and transportation, to enable participants to achieve their employment or training goals

• Training and retraining resources to update skills or acquire skills in a new field

ECFE is a parenting education program for all Minnesota families with children between the ages of birth to Kindergarten entrance. Learning opportunities through ECFE may occur in a variety of settings, such as the families' homes, district or community sites, and online parenting education and support. Programming and education services often include parents/caregivers and children but may just include parents/caregivers. Works to strengthen families and engage the ability of all parents to provide an environment that supports the healthy development of their child. Services may include:

• Early childhood screening for children ages 3 - 5

• Home visits

• Information on community resources

• Parent-child activities

• Parent discussion groups

• Play and learn activities for children

Nutrition Education and breastfeeding program that helps pregnant women, new mothers, babies, and young children eat well, learn about nutrition, and stay healthy. Services include:

• Benefits to purchase nutritious foods from local grocery stores

• Individualized nutrition education and counseling

• Referrals to health care and other services

Assistance paying child care costs for children up to age 12 and for children with special needs up to age 14. Child care costs may be paid for families while parents work, look for work, or attend school. Family size, family income, and participation in authorized activities are considerations in determining eligibility for and amount of assistance, along with available funding.

Functions as a community-based hub for people to access, integrate, and coordinate the range of services they require. Vail Clubhouse centers on community, relationship building, and opportunities to help:

• Reduce isolation and promote recovery through social support services
• Improve mental and physical health through wellness education and support
• Obtain and maintain safe, affordable housing
• Achieve vocational goals

A nine-bed adult residential crisis stabilization unit that provides a short-term safe place for individuals to stay while working through a mental health crisis. The unit is staffed 24 hours/7 days a week and offers counseling and education to clients in managing their mental health symptoms. All admissions are voluntary and based on bed availability.

• Base fare: $2; rush hour (6 am - 9 am and 3 pm - 6:30 pm weekdays) $2.50; express fare .75 extra during rush hour. Express fare .50 extra during non-rush hours.

• Reduced fare, $1, to senior citizens weekdays during NON-rush hours, all day Saturday, Sunday, and holidays. ID required with this fare - driver's license or Minnesota ID with a "T" endorsement, Medicare card, or railroad retirement card.

• Reduced fare, $1, for youth ages 6 - 12 available weekdays during NON-rush hours, all day Saturday, Sunday, and holidays.

• Children ages 5 and under ride free when accompanied by one paid adult fare (limit 3 per adult). Youth 13 - 17 may qualify for a reduced fare card through schools and employers.

• Reduced fare, $1, for persons with disabilities (those unable to qualify for a Minnesota driver's license because of physical or mental disability) all day, every day, including weekends and holidays. Certified identification is required - Medicare card (valid during non-rush hours only), Metro Mobility card, Metro Transit temporary ID card, or Minnesota driver's license/state ID with an "L" or "A" endorsement. • Free Transit Program for Disabled Veterans: Provides free rides on any fixed route service in Minnesota. Disabled veterans with a service connected disability must show a Veterans Identification Card (VIC) issued by a VA Medical Center with the words "Service Connected" or the initials "SC" below the photo. These cards are available at the VA Medical Centers in Minneapolis or St. Cloud.

Tax deductible donations can be made to Simpson Housing Services. Personal hygiene items, non-perishable food items, cleaning products, and household items are accepted. See website for specific items and an urgent needs list. Items not accepted:

Provides investigation and intervention services on behalf of vulnerable adults who are unable to act on their own behalf, manage their own affairs, or who are in immediate danger of abuse. Types of abuse may be physical, emotional, sexual abuse, unsafe or hazardous living conditions, exploitation, neglect, or abandonment. Services include:

• Taking reports and investigating abuse and/or maltreatment

• Removal of the individual to safer surroundings

• Services necessary to remove the conditions which have created a threat to life

NOTE: Complaints, questions, or concerns involving incidents in facilities licensed by the Health Department, such as hospitals, nursing homes, boarding care homes, supervised living facilities, assisted living and home health agencies can be referred to the Minnesota Board on Aging Ombudsman for Long-Term Care.

Minnesota's Medicaid program for people with low income. Most people who have MA receive health care through health plans. Members who do not receive health care through a health plan receive care on a fee-for-service basis, with providers billing the state directly for services they provide. MA pays for a variety of services like doctor visits, prescriptions, and hospital stays. Some services and prescriptions may require prior approval. MA pays for medical bills going back three months from the month the application is received. NOTE: People with disabilities who work, even if they're over the income or asset limits of MA, may still qualify for a program called Medical Assistance for Employed Persons with Disabilities - MA-EPD.

MA-QWD is for employed people under age 65 with disabilities who lost their RSDI benefits because their income exceeds Substantial Gainful Activity (SGA) limits. QWD Program pays for Medicare Part A premium if eligible. This is the only benefit provided by QWD.
