Coordinated Entry Services (CES): The Housing Advocacy Services Department participates in the Southeast Minnesota River Valleys Continuum of Care Coordinated Entry Services (CES) for housing and support services for households experiencing homelessness Prevention Assistance: When funding is available, individuals and families will be referred to the Housing Advocacy Department for follow up on Prevention Assistance. Funds will vary on a monthly basis. The Prevention Assistance fund assists families and individuals facing eviction with back-rent financial assistance and landlord-tenant negotiation assistance. Case management includes services such as transportation assistance, information/referral, and landlord tenant law training with the goal of achieving housing stability moving forward. Rapid Re-Housing (RRH): Provides short-term rent assistance and/or deposit for individuals and families with children who are homeless to get into permanent affordable housing where the participant is the leaseholder and chooses their own apartment according to their needs. RRH includes case management services such as life skills training, transportation assistance, information/referral, landlord tenant law training, and utility assistance for up to six months with the goal of obtaining and subsequently maintaining stable housing. Households will be assessed and prioritized using the Coordinated Entry System. Long-Term Assistance: The Permanent Supportive Housing (PSH) Program provides high-barrier households experiencing homelessness with affordable rental housing and supportive services that assist participants with maintaining housing stability, improving overall wellness, engaging in the community and leading successful lives. PSH targets households that need intensive case management services due to challenges such as significant and persistent mental health issues and chemical dependency. Households will be assessed and prioritized using the Coordinated Entry System. Note: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Individuals in need of permanent supportive or transitional housing should call United Way 211 to complete a pre-screen. Once the applicant is pre-screened, if they are homeless or in need of housing assistance, they proceed through the Saint Louis County Coordinated Entry system. If homeless, households are to be assessed by a case manager using a VI-SPIDAT and supplemental questions. Households experiencing homelessness are scheduled for an appointment. Housing case managers meet with the household, complete the assessment, and initiate the process of applying for benefits, provide housing applications, and ensure the household is entered on the priority list for housing.

Front Door Screener's connect clients to: - Community resources or other providers - Coordinated Entry System (CES) assessments for families based on eligibility criteria. Families can visit the website to fill out a Families Coordinated Entry referral form. - Education or consultation of county programs - Intake assessments of county-operated programs. For clients identified as needing county intervention, a Front Door Service Planner will conduct a thorough evaluation of needs and provide short-term case management as well as assess client eligibility for possible ongoing services from the county or a contracted provider.

Shelter is available based on capacity. - Housing Hub: - ?Connects single adults ages 18 and over and families experiencing homelessness or at risk of being homeless with a Housing Specialist to provide housing problem solving. Housing Specialists can assist households with practical solutions, identify sources, alternatives, resolve immediate barriers to housing, and mediate conflict resolution between landlords, friends, or family. - The initial access point to be connected with a Housing Specialist is short-term with a 30-day timeframe. After 30 days initial contact, households with no other pathways identified will be connected to a Housing Assessor and placed on the Housing Registry. - Street Outreach: Provides support to individuals who are experiencing homelessness and living outside NOTE: People seeking temporary care for their pets must already be enrolled in a housing support program through Washington County's Homeless Outreach Services Team to see if their pet can be sheltered by referral.
