- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Primary services include coordinated entry access point for individuals who are homeless or at risk of being homeless. Case management is offered to prevent future issues with homelessness. Financial assistance may be available but is NOT provided in all situations, requirements vary by programs and funding. Potential assistance consists of: - Funding for housing and utility costs/deposits - Funding for transportation costs via vehicle repair or public transit - Clothing and food assistance as well as other basic needs - Housing stability case management Programs may include: - Limited fund for short term hotel stays in tandem with case management services - Long-term homelessness supportive services in Aitkin, Carlton, Isanti, Mille Lacs, Kanabec, Pine and Chisago County to help with housing search and placement for families and unaccompanied youth - Assistance for homeless ex-offenders in Chisago, Isanti, Kanabec, Mille Lacs, and Pine County, housing placement as well as rent and deposit funds may be available -Transitional housing services for homeless youth ages 16 - 22, as well as some financial assistance for rent NOTE: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP and others.

The Housing Locator program works with serious mentally ill individuals who are referred by Beltrami County caseworkers. The Housing Locator works closely with participants to secure housing and assists the client to locate units and work with landlords.
Services provided: - Advocacy with landlords - Eviction prevention and referrals to relevant agencies for those facing eviction or a utility shut-off - Helps with the Energy Assistance Program application. Other services include: - Health and wellness services. An onsite clinic is available to be connected to a MNsure Navigator about options for insurance. - Family reunification - Learning independent living skills - Meeting Christian-based spiritual needs via TreeHouse after hours - Obtaining birth certificates, photo IDs, and other documents - Securing housing (short-term shelter and long-term options). The agency is an access point for Coordinated Entry Assessment. Staff helps youth search for affordable housing options, fill out housing applications, cover expenses associated with securing an apartment or housing, apply for financial assistance with rent and bills, find shelters in Anoka and Hennepin counties, connect youth to "Communities Host Home" of Anoka County (partnership with YMCA), and refer youth to transitional living programs. HOPE Place transitional housing program provides housing for 12 young people (ages 18 - 25) for an average of 12-24 months. - Searching for jobs (including finding employment for youth with a criminal background), filling out job applications, building resumes and cover letters, connecting youth with partner agencies to find employment opportunities, providing work-appropriate clothing, and finding transportation to get to a job - Supporting new moms through the New Moms Program

Eligibility requirements: first come, first served basis; funds are limited Homemaking Services - Assist individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping Information and Referral - Up-to-date disability information and referrals to various agencies and individuals within the region Independent Living Skills training Peer Mentoring - Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals - Match interested consumers with a peer mentor who provides support, provide problem-solving solutions, social skills, etc. Public Education - Provide information, resources, and materials on accessibility guide, ADA, disability laws, etc. Respite Care Services - Provide short-term in-home care and support when the primary caregiver or family members need to be absent from the home. Provides emergency financial assistance (emergency rental assistance) for eligible homeless youth and young families, and advocacy with landlords and utility companies as necessary. 30+ units of supportive housing for youth ages 18-24: - 2001 Apartments - Gateway Tower - Scattered site housing units NOTE: Homeless youth and young adults ages 18 - 24 years must get a pre-screen through United Way 211 to access this program. The pre-screen information is provided to Saint Louis County Housing Assessment Team - SLCHAT.
