Community-Based Services and Supports - Dungarvin Minnesota

- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Also, supports student youth as they search for and find competitive employment and helps them explore post-secondary education. - RUMI: Connects people with disabilities with a compatible supportive roommate who can earn up to 2x more than a typical direct care position, tax-free. Housing Access Services (HAS): Provides housing assistance to people who have a disability and are receiving Medical Assistance (MA) benefits. Assists with the housing search and can assist with financial needs related to the move when applicable. Housing Stabilization Services (HSS): Housing services for Medical Assistance (MA) recipients who have a disability and are experiencing housing instability. The program assists with finding independent housing and provides ongoing support to help sustain housing.

Youth Services - Tubman

Provides violence prevention, intervention, support, outreach services, and leadership opportunities for youth in residential and community settings. Provides a variety of services to youth of all genders who have experienced relationship violence, sexual assault or exploitation, stalking, or homelessness. Programs include: Movement for Violence Prevention (MVP): Leadership development activities to assist youth-led community engagement, skill enhancement, peer connections, and tools for change to prevent youth violence. Inspiring Youth: A year-long, intensive, community-based program for youth ages 10 - 17 and their families who have been impacted by violence. Youth workers provide comprehensive and culturally responsive mentorship, support, and access to services, activities, and leadership opportunities. Youth Site-Based and Community-Based Housing: Transitional housing for youth and young adults who are victims of violence and exploitation, including sex trafficking. Safety planning and support are provided. Voices in Prevention (VIP): A 6-week violence prevention curriculum serving middle and high school students. Curriculum might also include support groups, one-on-one sessions, peer mediation, and service learning. Youth Safety Plans: Specialized safety plans for youth NorthStar Youth Outreach Center: A drop-in center for homeless youth offering advocacy, safety planning, and connections to additional community resources. Drop-in center for homeless youth: ?- Housing Hub: Connects youth experiencing homelessness or at risk of being homeless with a Housing Specialist to provide housing problem solving. Housing Specialists can assist the youth with practical solutions, identify sources, alternatives, and mediate conflict resolution between friends or family. The initial access point to be connected with a Housing Specialist is short-term with a 30-day timeframe. After 30 days initial contact, households with no other pathways identified will be connected to a Housing Assessor and placed on the Housing Registry. - Provides computers for homework and job searches, a game room, kitchen, lockers, showers, storage areas, and a washer and dryer - Staff help youth get help with employment and housing searches, connect to resources, and receive basic need items (clean clothes, food, sleeping bags, and toiletries)

Helps eligible adults with disabilities seek suitable, affordable, accessible housing that gives them more control and independence. Housing Access Services can: - Accompany individuals when looking at housing - Assist finding affordable furnishings and household items - Assist with packing and moving - Help complete rental applications and lease agreements - Help develop household budgets - Help people move to a place of their own - Meet and negotiate with landlords/property staff
