Community-Based Services and Supports - Dungarvin Minnesota

- 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services. - Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution. - Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy. - Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community. - Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals. - Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people. - Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently - Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Also, supports student youth as they search for and find competitive employment and helps them explore post-secondary education. - RUMI: Connects people with disabilities with a compatible supportive roommate who can earn up to 2x more than a typical direct care position, tax-free. Drop-In Center - HOPE 4 Youth

Provides basic needs such as hot meals, grab and go foods, clothing, laundry facilities, a shower, and more. Additionally, gives access to case managers on-site to help connect youth to education, employment opportunities, health and wellness programs, stable housing, and more. Also provides a 12-unit transitional housing facility in Coon Rapids that offers supportive services to young people who are at risk of or experiencing homelessness.

Emergency Housing Services - Lakes and Pines Community Action Council

Primary services include coordinated entry access point for individuals who are homeless or at risk of being homeless. Case management is offered to prevent future issues with homelessness. Financial assistance may be available, but it is NOT provided in all situations. Requirements vary by programs and funding. Potential assistance consists of: - Funding for housing and utility costs/deposits - Funding for transportation costs via vehicle repair or public transit - Clothing and food assistance as well as other basic needs - Housing stability case management Programs may include: - Assistance for homeless ex-offenders in Chisago, Isanti, Kanabec, Mille Lacs, and Pine County, housing placement, as well as rent and deposit funds, may be available - Limited fund for short term hotel stays in tandem with case management services- ?- Long-term homelessness supportive services, also known as coordinated entry, are provided for those who qualify for short term hotel stays in Aitkin, Carlton, Isanti, Mille Lacs, Kanabec, Pine, and Chisago County to help with housing search and placement for families and unaccompanied youth - Transitional housing services for homeless youth ages 16 - 22, as well as some financial assistance for rent ? ?NOTE: Funding comes from a variety of sources, including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Family Services: - Bimaadiziwin Mikana (The Path of Life): Parent mentor program designed around healing and connecting families with community and elders. While focusing on traditional cultural lifeways, resiliency, and strengths, positive Indian parenting values are incorporated into the program. - Ombi'Ayaa Anishinabe Ininiiwug (Rise Up Original Men): Provides a place for American Indian men who want to practice spiritual, cultural, and intergenerational engagement. Activities include: Cultural events, including sugar bush and ricing Men's softball and basketball recreational leagues Traditional ceremonies, including sweat lodge Weekly father and men's group and weekly drumming group - Thinica Owichakiyapi Thipi (Helping the Homeless Build): - For individuals and families experiencing homelessness and who are not currently in housing or have not found housing: Provides resources and support for those experiencing homelessness. Services may include assisting the client with housing search assistance, tenant education, budgeting assistance, life skills training/coaching, etc. - For individuals and families experiencing homelessness or on the verge of becoming homeless that are currently in housing or have found housing: If eligible Family Homelessness Prevention and Assistance Program - FHPAP may help with rent, security deposit, utilities, and utility deposit assistance. - Note: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP as well as others. Youth Services: - Waaban Ogimaawag (Tomorrow's Leaders): Provides youth with an opportunity to become engaged in traditional American Indian service learning. Also provides tutoring to American Indian youth on diverse subject areas, including test preparation, skill building, and success strategies.

Northern Area Triage Team - Saint Louis County Public Health and Human Services

Provides triage services for situations as needed and reviews county case information. Services include: - Assistance with housing and basic needs - Community Collaborative Fund: Initial assessment for households at risk of homelessness. Individuals must have been denied by the Saint Louis County EGA or EA program, or have already used EGA/EA within the last 18 months. ESG (Emergency Solutions Grant) funds are determined to be used for rent, rental deposit, mortgage, or utilities. NOTE: There is currently no funding at this time. - Forms and certificate assistance for county programs such as SNAP, WIC, EGA/EA, GA, child care assistance, MFIP, Minnesota Supplemental Aid, or Housing Support. Also, follow up with issues regarding MNsure applications. - Referrals to community services, mental health, and chemical dependency services outside of Saint Louis County

Disability Resource Center - Access North Center for Independent Living - Northeastern MN

Resource center assists with the following services: - Advocacy: Includes areas of housing, civil and human rights, employment, transportation, and others. The Center advocates for state, local, and federal legislation. - Aging in Place Program: Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence. - Assistive Technology Network: Assists individuals in the process of obtaining assistive technology devices. - Homemaking Services: Assists individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping. - Independent living skills training - Information and Referral: Provides up-to-date disability information and referrals to various agencies and individuals within the region. - On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed; the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited. - Peer Mentoring: Assists individuals living with a disability to identify goals and create a unique plan to achieve those goals. Matches interested consumers with a peer mentor who provides support, problem-solving solutions, social skills, etc. - Public Education: Provides information, resources, and materials on accessibility guides, ADA, disability laws, etc. - Respite Care Services: Provides short-term in-home care and support when the primary caregiver or family members need to be absent from the home. - Systems Advocacy: Works to implement changes where needed at a broader systemic level. - Transition/Relocation Services: Assists individuals living with a disability in transitioning from nursing facilities and other institutions into community settings, with the goal of preventing institutionalization.

Homeless Services - Range Mental Health Center

Provides homeless outreach and case management services to adults with serious and persistent mental illness. Offers housing options including crisis beds, rental assistance, and permanent housing. Outreach, case management, and rental assistance are also available to homeless youth under the age of 24. NOTE: Single adults and youth must get a pre-screen through United Way 211 to access this program. The pre-screen information is provided to Saint Louis County Housing Assessment Team (SLCHAT).

Housing Support Services - Alluma

Provides housing support options to focus on independence and well-being within the Northwestern Apartments. Each apartment has two bedrooms, a living room, a dining area, a kitchen, a bathroom, and a large storage area. Services include: - Adult foster care program: Four-bed licensed adult foster care program for long-term transitional housing for clients with a serious and persistent mental illness. 24-hour supervision in an apartment-style environment to support and assist residents working towards the goal of independent living. - Homeless Intervention Program (HIP): Assist people living with a serious mental illness who have long-term homelessness in finding safe and affordable housing. Provides coordinated entry, including housing search assistance, outreach, case management, benefits assistance, and other services.

A young person is paired with an Independent Living Skills Counselor who acts as their Case Manager. The pair can be in touch as much as daily, and meet in person as often as needed-usually weekly. Together, the pair develops goals and plans for the young person to successfully live on their own. Provides: - Affordable housing search - Assistance in accomplishing stability - Housing counseling and financial assistance services
