Transitional Housing - Minnesota Assistance Council for Veterans

Provides a number of transitional housing units located in the Twin Cities metro, Duluth, and Saint Cloud. Most transitional houses are Single Room Occupancy (SRO), though entire houses may be available for families. Veterans must be able to live independently. The length of stay varies, but the maximum stay is two years. The rent is determined on a sliding scale - no greater than $500/month. All units require a sober environment (no drugs or alcohol on the premises) but does not require sobriety. The only exception is Building 47 on the Minneapolis VA Medical Center Campus as this transitional housing requires sobriety.

Financial assistance may be available, but it is NOT provided in all situations. Requirements vary by programs and funding. Potential assistance consists of: - Funding for housing and utility costs/deposits - Funding for transportation costs via vehicle repair or public transit - Clothing and food assistance as well as other basic needs - Housing stability case management Programs may include: - Assistance for homeless ex-offenders in Chisago, Isanti, Kanabec, Mille Lacs, and Pine County, housing placement, as well as rent and deposit funds, may be available - Limited fund for short term hotel stays in tandem with case management services- ?- Long-term homelessness supportive services, also known as coordinated entry, are provided for those who qualify for short term hotel stays in Aitkin, Carlton, Isanti, Mille Lacs, Kanabec, Pine, and Chisago County to help with housing search and placement for families and unaccompanied youth - Transitional housing services for homeless youth ages 16 - 22, as well as some financial assistance for rent ? ?NOTE: Funding comes from a variety of sources, including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Housing Resource Line - Dakota County Employment and Economic Assistance

Provides housing assistance for eligible individuals, including homeless and near-homeless people, through a variety of programs: Emergency Shelter Clearinghouse: Offers emergency shelter placement, eviction-prevention assistance, and information on community housing resources and referrals. Individuals seeking temporary care for their pets must already be enrolled in a Dakota County Supportive Housing Unit program to be evaluated for pet sheltering by referral. Housing Related Coordinated Entry: Provides intake and housing assessments for single adults and families who are homeless or at risk of losing their housing. HUD Transitional Housing and Supportive Housing Program: Offers advocacy and case management for eligible homeless families with minor children and pregnant individuals. Participants may qualify for up to 24 months of transitional housing rental subsidy.

Youth Services - Tubman

Provides violence prevention, intervention, support, outreach services, and leadership opportunities for youth in residential and community settings. Provides a variety of services to youth of all genders who have experienced relationship violence, sexual assault or exploitation, stalking, or homelessness. Programs include: Movement for Violence Prevention (MVP): Leadership development activities to assist youth-led community engagement, skill enhancement, peer connections, and tools for change to prevent youth violence. Inspiring Youth: A year-long, intensive, community-based program for youth ages 10 - 17 and their families who have been impacted by violence. Youth workers provide comprehensive and culturally responsive mentorship, support, and access to services, activities, and leadership opportunities. Youth Site-Based and Community-Based Housing: Transitional housing for youth and young adults who are victims of violence and exploitation, including sex trafficking. Safety planning and support are provided. Voices in Prevention (VIP): A 6-week violence prevention curriculum serving middle and high school students. Curriculum might also include support groups, one-on-one sessions, peer mediation, and service learning. Youth Safety Plans: Specialized safety plans for youth NorthStar Youth Outreach Center: A drop-in center for homeless youth offering advocacy, safety planning, and connections to additional community resources. Housing Services - Prairie Five Community Action

Coordinated Entry: An access point providing coordinated entry assessments to single adults and families with children who are low-income and meet eligibility requirements in the county area Family Homeless Prevention Assistance Program: This program helps families or individuals get out of homelessness or prevent homelessness from occurring. If eligible, the FHPAP program can help with the first month's rent, security deposit, or back rent/mortgage assistance up to a certain dollar amount. To be eligible for this program, individuals must: - Fill out an application - Be able to prove a housing crisis (eviction notice, mortgage papers, etc.) - Must have income During this program, all applicants who are deemed eligible will also be required to watch the "Rent Wise, Money Smart" videos. The videos go over smart shopping, facts about debt and credit reports, saving, budgeting, and more. Transitional Housing Program: Provides a long-term housing program to help an individual regain self-sufficiency. The goal of this program is to transition struggling individuals and families into stabilized living situations. The resources are offered for people of all backgrounds, including single parents, children, and individuals that were recently evicted. The program requires an assessment, application approval, weekly case management meetings, and constant contact with a case manager. If approved, Prairie Five can: - Pay a portion of rent - Assist with referrals, phone calls, setting up appointments, application assistance, resume building, job search, family development, etc. - Tenant education - Budgeting/smart money use

Marlene's Place addresses the needs of homeless teen parents and their children by providing safe housing and a supportive environment that allows them to heal, learn, grow, and thrive. Services focus on stabilizing pregnant and parenting youth and their young families through direct housing and supports, building youth's connections and resources to foster self-sufficiency, and preventing and ending homelessness across two generations.

An American Indian culturally grounded transitional living program available to youth between the ages of 16 - 21 who have no parental substitute or foster home to which they can safely go. This intervention program emphasizes training, education, and employment goals and is designed to develop a support system to meet the needs of the body, mind, and spirit. Services emphasize the relationship between youth and positive role models (staff and elders) to teach new behaviors, learn appropriate ways to express feelings, and manage everyday living. Creating community and cultural connections prepares youth for independent or interdependent living, helping to break the cycle of homelessness.

Rental Assistance Programs - Washington County Community Development Agency

- Bridges for Long-Term Homeless Households: Provides temporary housing for individuals and families who meet the definition of long-term homeless and has a serious and persistent mental illness with a referral from Washington County Community Services - Housing Choice Vouchers (Section 8): Vouchers assist the tenant in renting a unit from a private landlord. Unit must be affordable and rent must be reasonable for the area. This type of assistance is transferable. It is assigned to the tenant and moves with the tenant so long as the tenant remains income eligible and has not violated any program rules - Public Housing: Operate a 40-unit, one bedroom, apartment complex, Whispering Pines in Forest Lake. Qualifying individuals pay up to 30% of their adjusted monthly income for rent. This type of assistance is not transferable. If the participant moves, the participant loses their assistance. Preference for elderly individuals ages 55-61 or person with disability

Rezek House Transitional Living Program - Lutheran Social Service of Minnesota - Metro

Provides a two-year transitional living program for youth ages 16 - 21 experiencing homeless. Rezek offers 10 efficiency units for single youth as well as two one-bedroom units for parenting youth. Case management and life skills training provided.
