Provides a two-year transitional living program for youth ages 16 - 21 experiencing homeless. Rezek offers 10 efficiency units for single youth as well as two one-bedroom units for parenting youth. Case management and life skills training provided.
Housing: - Women and children live in a shared living community - The house has two living units, each unit accommodating up to a total of 8 women and their child(ren) - Residents may stay for up to 1 year Long-term stability plan: - Each resident is required to develop a long-term stability plan - Meet with a case manager monthly to work on the plan - Residents are expected to be employed - Residents work on monthly budgeting - Work on parenting goals and participate in referred parenting programs - Participate in recommended community-based services such as counseling, chemical dependency or mental health aftercare
Journey Transitional Living Program Program to establish independent living arrangements and skills training for homeless youth Red Path Program Program created to assist youth leaving placements in foster care, residential treatment, group homes, or other residential care facilities with living skills classes, transition planning, and case management. Includes: - Budgeting and finance - Juvenile justice - Healthy relationships - First impressions - Renter's education - Sexual health - Diversity - Leadership and community development Emergency Youth Hotline Program providing temporary shelter for runaway or homeless youth, free consultation and counseling as well as support, therapy sessions, and assistance communicating with family to resolve conflicts. Will work with county social services to resolve issues and provide referrals for housing and basic needs. Safe Place Offers youth younger than age 18 immediate access to services and shelter any time - 24 hours a day, 365 days a year Youth Street Outreach Provides outreach for at-risk youth

- Bridges for Long-Term Homeless Households: Provides temporary housing for individuals and families who meet the definition of long-term homeless and has a serious and persistent mental illness with a referral from Washington County Community Services - Housing Choice Vouchers (Section 8): Vouchers assist the tenant in renting a unit from a private landlord. Unit must be affordable and rent must be reasonable for the area. This type of assistance is transferable. It is assigned to the tenant and moves with the tenant so long as the tenant remains income eligible and has not violated any program rules - Public Housing: Operate a 40-unit, one bedroom, apartment complex, Whispering Pines in Forest Lake. Qualifying individuals pay up to 30% of their adjusted monthly income for rent. This type of assistance is not transferable. If the participant moves, the participant loses their assistance. Preference for elderly individuals ages 55-61 or person with disability

Youth workers provide comprehensive and culturally responsive mentorship, support, and access to services, activities, and leadership opportunities. Youth Site-Based and Community-Based Housing: Transitional housing for youth and young adults who are victims of violence and exploitation, including sex trafficking. Safety planning and support are provided. Voices in Prevention (VIP): A 6-week violence prevention curriculum serving middle and high school students. Curriculum might also include support groups, one-on-one sessions, peer mediation, and service learning. Youth Safety Plans: Specialized safety plans for youth NorthStar Youth Outreach Center: A drop-in center for homeless youth offering advocacy, safety planning, and connections to additional community resources. Independent living in apartments for 16 - 21 year olds - Housing and independent living support services for homeless youth, ages 16 - 21. Please note that 16- and 17-year-olds need parent or guardian permission. We can help you negotiate with your parent or guardian if we think independent housing is the best option for you. - Housing and independent living support services for teen parents, ages 16 - 21, who are homeless or living in inadequate housing. We can help you find an apartment, complete the application, pay security deposit, and help with monthly rent. - Case management services to help you with education, employment, parenting, child care, and more. We can also help you with obtaining medical insurance and accessing medical, dental, or mental health services. - All youth participating in transitional housing have access to: Individual, and/or family counseling appropriate to the individual needs of single teens and teen parents. - Skill building workshops for single teens and teen parents to increase self-sufficiency and independence.

Coordinated Entry: An access point providing coordinated entry assessments to single adults and families with children who are low-income and meet eligibility requirements in the county area Family Homeless Prevention Assistance Program: This program helps families or individuals get out of homelessness or prevent homelessness from occurring. If eligible, the FHPAP program can help with the first month's rent, security deposit, or back rent/mortgage assistance up to a certain dollar amount. To be eligible for this program, individuals must: - Fill out an application - Be able to prove a housing crisis (eviction notice, mortgage papers, etc.) - Must have income During this program, all applicants who are deemed eligible will also be required to watch the "Rent Wise, Money Smart" videos. The videos go over smart shopping, facts about debt and credit reports, saving, budgeting, and more. Transitional Housing Program: Provides a long-term housing program to help an individual regain self-sufficiency. The goal of this program is to transition struggling individuals and families into stabilized living situations. The resources are offered for people of all backgrounds, including single parents, children, and individuals that were recently evicted. The program requires an assessment, application approval, weekly case management meetings, and constant contact with a case manager. If approved, Prairie Five can: - Pay a portion of rent - Assist with referrals, phone calls, setting up appointments, application assistance, resume building, job search, family development, etc. - Tenant education - Budgeting/smart money use

Transitional housing program that fulfills the need for healing time and personal development that combines best practices with a Christian environment. Services include: - Safe housing; food and clothing referrals; case management/advocacy; support group; domestic violence education courses; individualized therapy; life skills education; financial education and counseling; assistance to pursue education, jobs and housing; bible studies and prayer ministry - Women and children receive a comprehensive program for 12-18 months with the goal to equip and empower them to live free of abuse - Referrals are given to men who have been abused (i.e. Cornerstone, Domestic Abuse Project and Tubman) or wish to change abusive behavior (i.e. community groups) - Programs for women and children provide paths to emotional, spiritual and physical safety, and health. Mentors, tutors and educators are provided to educate, equip and empower women to increase confidence and quality of life as well as family stability. Goal is to help women and children transition to lives free of abuse.
