Housing Services - Prairie Five Community Action

Coordinated Entry: An access point providing coordinated entry assessments to single adults and families with children who are low-income and meet eligibility requirements in the county area Family Homeless Prevention Assistance Program: This program helps families or individuals get out of homelessness or prevent homelessness from occurring. If eligible, the FHPAP program can help with the first month's rent, security deposit, or back rent/mortgage assistance up to a certain dollar amount. To be eligible for this program, individuals must: - Fill out an application - Be able to prove a housing crisis (eviction notice, mortgage papers, etc.) - Must have income During this program, all applicants who are deemed eligible will also be required to watch the "Rent Wise, Money Smart" videos. The videos go over smart shopping, facts about debt and credit reports, saving, budgeting, and more. Transitional Housing Program: Provides a long-term housing program to help an individual regain self-sufficiency. The goal of this program is to transition struggling individuals and families into stabilized living situations. The resources are offered for people of all backgrounds, including single parents, children, and individuals that were recently evicted. The program requires an assessment, application approval, weekly case management meetings, and constant contact with a case manager. If approved, Prairie Five can: - Pay a portion of rent - Assist with referrals, phone calls, setting up appointments, application assistance, resume building, job search, family development, etc. - Tenant education - Budgeting/smart money use

Transitional Living Program - Evergreen Youth & Family Services

Independent living in apartments for 16 - 21 year olds - Housing and independent living support services for homeless youth, ages 16 - 21. Please note that 16- and 17-year-olds need parent or guardian permission. We can help you negotiate with your parent or guardian if we think independent housing is the best option for you. - Housing and independent living support services for teen parents, ages 16 - 21, who are homeless or living in inadequate housing. We can help you find an apartment, complete the application, pay security deposit, and help with monthly rent. - Case management services to help you with education, employment, parenting, child care, and more. We can also help you with obtaining medical insurance and accessing medical, dental, or mental health services. - All youth participating in transitional housing have access to: Individual, and/or family counseling appropriate to the individual needs of single teens and teen parents. - Skill building workshops for single teens and teen parents to increase self-sufficiency and independence.

Emergency Housing Services - Lakes and Pines Community Action Council

Primary services include coordinated entry access point for individuals who are homeless or at risk of being homeless. Case management is offered to prevent future issues with homelessness. Financial assistance may be available, but it is NOT provided in all situations. Requirements vary by programs and funding. Potential assistance consists of: - Funding for housing and utility costs/deposits - Funding for transportation costs via vehicle repair or public transit - Clothing and food assistance as well as other basic needs - Housing stability case management Programs may include: - Assistance for homeless ex-offenders in Chisago, Isanti, Kanabec, Mille Lacs, and Pine County, housing placement, as well as rent and deposit funds, may be available - Limited fund for short term hotel stays in tandem with case management services- ?- Long-term homelessness supportive services, also known as coordinated entry, are provided for those who qualify for short term hotel stays in Aitkin, Carlton, Isanti, Mille Lacs, Kanabec, Pine, and Chisago County to help with housing search and placement for families and unaccompanied youth - Transitional housing services for homeless youth ages 16 - 22, as well as some financial assistance for rent ? ?NOTE: Funding comes from a variety of sources, including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Transitional Housing - Wright County Community Action

Provides scattered-site supportive housing to homeless individuals and families. The purpose of the program is to move economically challenged residents toward self-sufficiency by identifying and removing barriers, integrating services, and developing plans for financial stability. Participants enter into a lease for a dwelling and pay a pro-rated amount of their income for housing. Develops a mutually agreed upon housing plan to attain housing stability. Services include: - Help in obtaining adequate housing - Partial limited cost share of rent for up to 18 months to income-eligible families - Skills building in budgeting - Supportive services such as resource and referral NOTE: The program does not assist with rent for existing housing
