• Assisted Living: For an independent senior lifestyle enhanced with personal care and health care to maintain independence. Provides a variety of social opportunities, events, entertainment, Bible studies, and classes. Trips to local restaurants and attractions are planned, and transportation is provided.

• Long-Term Care: Provides 24-hour skilled nursing care for advanced health needs. The care team includes physicians, nurse practitioners, registered and licensed nurses, nursing assistants, therapists, social workers, and chaplains who work together to provide high-quality clinical and personal care.

• Residential Hospice Care: Provides a home filled with care, comfort, and peace for those on their end-of-life journey. The care team is on-site 24 hours a day and can enable families to hand over the stress and worries of daily physical care, allowing time to focus on their time together. Also provides spacious private suites with a living room, dining room, and patio overlooking Buffalo Lake.

• Short-Term Rehabilitation: For adults of all ages when surgery, illness, or injury has brought day-to-day activities to a stop. Transitional care and rehabilitation at Lake Ridge provides a healthy balance of treatment protocols and highly skilled care, dedicated to rapid recovery. The therapists are specialists in rehabilitation care and help patients build strength and improve balance, mobility, and overall health.

The Auditor's office is responsible for property tax administration, financial record management, elections, delinquent tax records, and preparation of the annual financial statement of the county and audit activities. The Auditor's office handles the sale of licenses:

Provides a food shelf offering a wide variety of foods such as produce, meats, grains, milk, eggs, deli items, and personal care items. Families and individuals can use the food shelf once per month by appointment. The food shelf:

• Accepts donations of nonperishable food and select personal care/household items are accepted. Donations accepted include menstrual products, toilet paper, and paper towel.

• Food delivery program designed to make food access simple for those unable to visit the food shelf in person

• Offers curbside pick-up services are available during service hours. Families and individuals can pick up food as needed.

Program provides or arranges whatever support an elder neighbor needs to continue living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers, and/or paid nursing staff. Services include:

• Case management

• Door-to-door transportation provided by volunteers. Vehicles are not wheelchair accessible.

The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management) Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

A program for infants and children ages birth - 3 to help parents track their child's development. The program:

• Helps parents learn about their child's health and development

• Offers a professional, most often a nurse, to conduct home visits with the parent and child

• Offers questionnaires and handouts for fun activities

• Provides a fun way to get ideas about "what to teach" and "at what age" to teach it

Legal Clinics: Virtual advice regarding Bankruptcy, Family Law, and Social Security is available Home visits may be possible Legal Aid does not handle adoption, paternity, or wills, but special circumstance cases are considered Assistance may involve representation, advice, or referral. The demand for services is high and Legal Aid is unable to handle all requests for assistance. There are established case priorities and, in some instances, waiting lists have become necessary to allocate services. Advice appointments may occur in person, on the phone, electronically, at clinics, or outreach.

Provides older adults or individuals living with a disability with door-to-door transportation service to and from the local grocery store. Must be able to shop independently. Assistance with carrying groceries is provided. Errand services such as the bank, post office, hardware store, and/or pharmacy. Serves a small geographic area in south Minneapolis.

• Day support/day training and habilitation: Helps adults with intellectual and developmental disabilities develop adaptive, social, and life skills in a day-program setting

• Prevocational services: Prepares individuals for work by building work-related skills, routines, communication, and workplace readiness

• Employment support services: Includes employment development and employment support to help individuals find, get ready for, and maintain employment or vocational opportunities

• Vocational and adaptive skill building: Supports clients to gain adaptive skills, pursue vocational activities, and potentially artistic or community-based activities to help them contribute to society

• Behavioral support for individuals with complex needs: Supports are in place for those with more challenging behavioral, developmental, or support needs

NOTE: For clients unable to attend in person, services are offered remotely via tablet or phone to maintain a face-to-face connection while preserving social distancing

Cash-grant program for families with low incomes experiencing household emergencies. Families can receive assistance once every 12 months. The assistance received must resolve the crisis and not just delay the onset of the emergency. Assistance must be temporary and must not exceed 30 days. Families must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. Allowable Expenses:

• Housing: Rent and mortgage. Verification of payment overdue or threat of eviction notice required. Assistance for a rental unit damage deposit will not be provided if the family has already moved into the unit.

• Utilities: Verification of pending utility shut-off required

Amount of assistance provided is based on household size. Only vendor or protective payee payments will be issued.

Members of households may "shop" once a month for a variety of food. Amount of food provided is based on how many people reside in the household. Other times during the month households may receive produce and bread.
