• DivorceCare Group: A compassionate support group designed to help individuals heal from the pain of separation and divorce; meets Tuesdays

• Family Grace (Mental Health) Group: A 16-week small group designed to provide confidential, nonjudgemental, and inclusive community for the family and friends of those facing any type of mental health issue; meets Tuesdays

• Job Transition Support Group: For anyone unemployed, underemployed, or seeking a new opportunity; meets Mondays

• Life Groups: Provides a sense of belonging and community, spiritual growth, and a supportive environment; meetings vary

• Re-Engage Marriage Class: For marriage couples looking to find encouragement and support from others in a group setting to strengthen their marriage; meets Wednesdays

Offers 32 one-bedroom apartments. Each apartment includes appliances, storage space, off-street parking, and maintenance-free living. All utilities except phone and cable are included in the monthly rental amount. ?

Provides for the final disposition of indigent individuals. Based on the deceased and next of kin preferences, options for the disposition are direct cremation or immediate burial.

• Direct Cremation: Maximum $21588. Includes local removal, care of remains, transfer of deceased to crematory, basic services of staff, authorization, crematory charge and a non-decorative remains container. May include a short time of remembrance/memorial.

• Immediate Burial. Maximum of $2,900. Includes local removal, care of remains, vault/grave box at invoice, transfer of deceased to funeral home, basic services of staff, authorization, basic casket and transfer of deceased to local area cemetery.

Accepts and responds to and/or investigates reports of child abuse, neglect, or abandonment. Services may include:

• Accepting reports of abuse

• Assessment of the initial referral

• Consultation to individuals who suspect abuse

• Consultation with the family to determine the nature of the problem

• Dependency investigation

• Ongoing support services for children and their families

• Temporary placement of children who are removed from the home

• Works with the family toward unification or reunification

NOTE: In addition to taking reports, child protection screeners are available to explain the reporting statutes, answer questions regarding agency practices and discuss other resources and strategies when reports do not meet criteria for assignment for child protection investigation.

Prepares children for a successful transition to school by providing a pre-academic program for infants to pre-kindergarten age children. Staff work closely with parents to ensure each child achieves developmental milestones and is fully prepared to succeed in school. The center is:

• Accredited by the National Association for the Education of Young Children (NAEYC)
• Four-Star Parent Aware rated program

Provides curriculum-based learning experiences, including:

• Cultural awareness, promoting a culturally diverse clientele
• Developmentally appropriate math
• Language, literacy, and creative arts
• Social and emotional development

Children's mental health services are voluntary and include:

• Assisting families in navigating mental health services, including hospitalization and residential placements

• Conducts initial and ongoing comprehensive diagnostic assessments of need to create driven case plans

• Engaging families in supports and services to appropriately address the child's needs

• Identifying children with needs at the earliest possible time

• Information and referral to services including:

• Children's therapeutic services
• Respite services
• Parenting supports/services
• Awareness/education opportunities

• Providing case management services which includes visits with families and children within their home, community, or school settings on at least a monthly basis to address needs

Hospice care is delivered through a team of professionals under St. Mary's Hospital Hospice. The mission of Itasca Hospice is to support and assure access for patients and families to quality Hospice services as provided by Itasca Hospice, and to educate the community on issues of dying, death and grief. Hospice offers a variety of support groups dealing with illness, death and grief.

Medicare Savings Programs have four sub-programs: Qualified Disabled and Working Individuals (QDWI): Helps pay for Part A premiums only. Qualifying Individual (QI): Helps pay for Part B premiums only. Qualified Medicare Beneficiary (QMB): Helps pay for Part A and B premiums as well as deductibles, coinsurance, and copayments. Specified Low-Income Medicare Beneficiary (SLMB): Helps pay for Part B premiums only. Monthly income and countable resource limits apply. Countable resources include any checking or savings funds, stocks, and bonds. A home, one car, a burial plot, $1500 in burial expenses (if the amount is set aside), furniture, and other household items are not applied toward the resource limit. Participants pay for Medicare services that aren't covered, such as routine physicals and prescription drugs. ?Generally under these programs, premiums are put back into the monthly Social Security check, increasing participants' monthly income.

Provides general information and licensing for persons interested in becoming foster care providers. Training and support for foster care providers on emergency, short-term, and long-term placement.

Provides state-operated, secure mental health, substance use, and nursing treatment facilities for adults, including the following:

• Community Behavioral Health Hospitals (CBHHs): Provide inpatient behavioral health care for adults in six communities throughout the state. These 16-bed hospitals offer acute, short-term hospitalization services that include comprehensive assessment of mental, social, functional, and physical health; development of individualized treatment plans with medication management and 24-hour nursing care; and care delivered by multidisciplinary teams consisting of psychiatrists, general practice physicians, psychiatric nurses, psychologists, clinical social workers, addiction counselors, and occupational therapists. Services also emphasize illness management and recovery, as well as individualized discharge planning, care coordination, and aftercare planning to support a successful transition back to an appropriate community-based setting.

• Forensic Mental Health Services: Provides residential treatment services for adults in both secure and non-secure settings on a large campus in St. Peter, Minnesota. The program also operates a 32-bed North Campus facility on the north side of St. Peter and an additional 16-bed facility located off campus in St. Peter. FMHP primarily serves individuals who have been civilly committed by a court as mentally ill and dangerous, often following criminal charges in which a court determined they were not competent to stand trial or not guilty by reason of mental illness or cognitive impairment. Many patients are under more than one type of civil commitment. In addition, Community Integrated Services provides ongoing monitoring and support for FMHP patients who have been provisionally discharged by a court to live in less restrictive community settings. This statewide program serves individuals from all 87 Minnesota counties and supports patients in achieving stable, successful lives in a variety of community settings outside FMHP facilities.

• Forensic Nursing Home: Provides state-operated nursing home and provides nursing home-level care in a secure environment. The facility serves individuals who are civilly committed as mentally ill and dangerous, sexually dangerous persons, or sexual psychopathic personalities, as well as prison inmates on medical release from the Minnesota Department of Corrections. Care provided is consistent with that offered in other nursing homes and includes assistance with daily living, rehabilitation services, and end-of-life care.

• Intensive Residential Treatment Services (IRTS): Provide structured, short-term residential care for adults who no longer require hospitalization or other inpatient treatment but still need ongoing support before returning home. Clients typically have serious mental illness and may also have co-occurring conditions such as traumatic brain injury, addiction, or behavioral disorders. Services are available around the clock with support from mental health professionals and focus on improving psychiatric stability, personal and emotional adjustment, self-sufficiency, and independent living skills. Programs work collaboratively with clients to achieve individual goals and prepare them for a successful transition to home or to a less structured community setting, with an average length of stay of approximately 90 days before discharge.

• Substance Use Disorder Treatment Services: Community Addiction Recovery Enterprise (CARE) provides inpatient substance use disorder treatment services to adults at three locations statewide. The program offers specialized inpatient services for individuals who are chemically dependent and have co-occurring mental illnesses, along with relapse prevention services and comprehensive aftercare planning. All CARE facilities are locked, and clients stay an average of approximately 90 days before discharge, depending on their level of participation and responsiveness to treatment.

Participants in the program join a cohort to gain access to:

• 19 hours of approved business and child development training

• Individualized coaching

• Monthly learning community meetings

• Technical assistance

• Up to $1,000 in grant funding to help with start-up costs.

Provides a range of primary care options, including family medicine, internal medicine, and pediatrics. Services include:

• Adolescent and teen care including gynecological services like birth control, pelvic exams, pap tests and treatment for painful menstruation

• Annual physicals to monitor blood pressure, cholesterol and other health concerns

• Birth control and family planning services

• Circumcision

• Daycare, school, sports, and camp physicals and form completion

• Diagnosis and care for acute (short-term) illnesses, injuries, and infections

• Diagnosis and care for chronic conditions, such as diabetes or asthma

• Health education

• Immunizations and vaccinations, including flu shots

• Long-term primary care for adults and seniors, especially those with 1 or more health conditions:

• Arthritis

• Diabetes

• Heart disease

• High blood pressure (hypertension)

• High cholesterol

• Infections

• Skin conditions, such as minor burns or rashes

• Medication management

• Mental health screening and counseling

• Minor in-office surgery including mole removal, sutures and joint injections

• Referrals for specialty care and help to navigate the healthcare system

• Spot vision screening to check for lazy eye early and prevent loss of sight

• Well-child annual checkups and preventive care

• English Language Learning (ELL)/English as a Second Language (ESL)

A family therapist is assigned a limited caseload to work intensively with families in their homes. Family-based services include:

• Family resources

• Intensive in-home treatment

• Outpatient behavioral services

• Therapeutic foster care

Provides various basic needs programs such as:

• Emergency loans that may help cover costs such as a car repair, electricity bill, heating bill, funeral cost, medical, rent, or other need

• Food distribution program

• Food shelf

Helps people develop skills and knowledge to improve their lives and communities. Offers a variety of programs for learners of all ages. Class offerings include general sports and recreation (basketball, volleyball, flag football, baseball, t-ball, and cheerleading), a fitness center for health and wellness classes, and more.

Helps individuals and families with low incomes (including temporary low incomes) get the food they need for nutritious and well-balanced meals. Provides extra support to help stretch a household's food budget - it is not meant to cover all the groceries a family needs. Monthly benefits are loaded on an Electronic Benefit Transfer (EBT) card, which looks and works like a debit card. The card can be used to buy groceries at authorized stores, farmers markets, and online retailers. Learn more about where to shop and what can be bought. The amount of benefits received is based on income, expenses, and number of people in the household. The Supplemental Nutrition Assistance Program Employment and Training (SNAP E&T) program provides free employment and education training and support for people getting SNAP benefits. SNAP E&T participants gain support, skills, training, or experience to reach their goals. Many organizations, including technical and community colleges, community-based organizations, county agencies, tribes, and CareerForce Centers provide SNAP E&T programs and services. Programs can include career planning, job search, training for in-demand jobs, student support, financial support, and continued support once a participant has a job.

General Assistance (GA) provides a monthly cash benefit for adults and childless couples/families ages 18 to 64 who are unable to provide for themselves. NOTE: GA participants are also eligible for help with medical costs through Medicaid programs and Food Support

Provides services to children with severe emotional disturbance. Services include:

• Children's Mental Health Targeted Case Management (CM-TCM): Assist children and families in obtaining needed services, improve functioning in education, interpersonal relationships, family relationships, family functioning, and enhance daily living skills

• Children's Therapeutic Support Services (CTSS): Provides in-home, school-based, or community setting for counseling services. The program provides a safe, convenient, and nurturing environment for families and children to prevent out-of-home placement.
