The county will assess individuals who need help paying for a chemical use assessment or treatment services through the Behavioral Health Fund. Connects and provides older adults and their caregivers with services in their community to allow them to live long, healthy, and independent lives in their own homes. Services include:

• Food Assistance: Congregate dining locations offering low-cost meals, low-cost frozen meals, home-delivered meals, NAPS, and SNAP

• Healthy Living: Blood pressure checks, fall prevention screenings, telehealth conferences, Tai Chi and yoga sessions, and stretching/balance education

• Information Services: Offers memory loss education and Dementia Friends, Medicare counseling and assistance, fraud/scam identification and prevention, assistance with internet search, assistance with Healthcare Directives and Power of Attorney forms, and classes to older adults

• Mobile Community Center: Brings services and socialization to the door or community. The vehicle is equipped with high-speed internet, flat-screen TV, applications for programs, and meeting space for coffee and conversation.

• Social: Offers card parties, coffee and conversation time, caregiver respite, karaoke parties, video calls and chats with friends, and social media assistance

Provides services to send and receive money, pay bills, purchase money orders, and obtain a prepaid card, and offers a locator tool to find the nearest service location.

• Individuals can safely and anonymously dispose of prescription drugs

• Medicines Accepted from Households: Prescription, over-the-counter and pet medications. No medicines from businesses, including health care facilities, long-term care facilities, pharmacies, doctors' offices, and veterinary clinics can be accepted.

Providing emergency financial assistance grants to eligible union members. These are once in a lifetime grants, and the amount of funding allowed individuals is subject to change based on needs and/or the availability of funding.

People in need of pet food may come up to 5 times in a 12-month period. We do require them to have an ID with them

Fire prevention and emergency response services

Offers an income-based financial assistance program for any emergency and any other medically necessary services provided and billed through the centralized Patient Financial Services and available to patients with medical bills. Financial assistance applies to both hospital and clinic balances. Applicants must meet income guidelines to qualify for assistance.

Provides physical, occupational, and speech therapies with a focus on functional needs and overall wellness. Occupational Therapy: deals with issues of range of motion and strength related to the upper extremities, activities of daily living, feeding, cognition, and pain related to the neck, upper back, arms, and wheelchair positioning. Physical Therapy: deals with issues of range of motion, strength, transfer training, balance, gaid, and pain issues related to lower extremities and back. Speech Therapy: deals with issues of communication, swallowing, language and higher cognitive function.

• Charity Care: Offers financial help to patients who cannot pay all or part of their medical bills. 50-100% of the bill may be paid.

• Pharmacy Assistance Fund: Provides one-time (once a year) prescription assistance to patients experiencing financial hardship. Request must not exceed $500.

• Uninsured Discount: Provides a discount on medically-needed services for uninsured patients

Provides outpatient addiction treatment for adults with cognitive impairments, including drug and alcohol counseling using evidence-based approaches (individual and group therapy, expressive therapies, and mindfulness). Substance Use Disorder (Rule 25) and Problem Gambling (Rule 82) assessments are available by appointment. Problem gambling treatment is offered in person and via telehealth.

Provides a 5 day supply of food is provided once per month. Accepts non-perishable food and perishable items in season for donation.

• Community and adult basic education and senior citizen programs including ABE, ESL, and GED classes.

• Free breakfast and lunch are provided in the summer for children ages birth - 18. Adults can eat for $4. Meals are provided at the Intermediate School:

Provides state-funded food assistance to legal noncitizens ages 50 or older who do not qualify for the Supplemental Nutrition Assistance Program (SNAP, formerly called Food Stamps or Food Support) because of their citizenship status. Monthly benefits are loaded on an Electronic Benefit Transfer (EBT) card, which looks and works like a debit card. Provides a non-emergency medical transportation service for individuals with health insurance coverage and private pay. The van is wheelchair accessible providing safe transportation to medical appointments.

Provides a few options for patients who have received services through a Lake View Clinic and/or Hospital. Options offered:

• Financial Assistance Program: Helps individuals who are unable to pay all or part of their Lake View medical bills and have questions regarding a bill

• Elevate Patient Financial Solutions: Lake View partners with Elevate to provide free help to uninsured patients to find the most appropriate assistance from a variety of resources to help pay for medical bills

• Zero Interest Patient Financing: A payment plan option to help patients with their cost of service. The program provides a zero-interest revolving credit account with flexible payment terms and manages out-of-pocket expenses, deductibles, and insurance co-payments.

• Lyngblomsten Apartments: A 105-unit HUD-subsidized apartment community at 1455 Almond Avenue in Saint Paul for older adults who are 65 and older. Independent living with income-based rent and an active waiting list

• Lyngblomsten at Lino Lakes: A comprehensive older adults living campus at 6070 Blanchard Boulevard in Lino Lakes offering independent living (apartments and townhomes), assisted living, and memory care with community amenities.

• The Heritage at Lyngblomsten: Older adult living apartments at 1440 Midway Parkway in Saint Paul. Market-rate rental housing with optional supportive services available.
