Homelessness Prevention - Anoka County - YMCA of the North
Provides emergency rental assistance with goal-oriented case management to address the immediate housing crisis, prevent homelessness, and promote long-term housing stability by addressing barriers related to homelessness. Supportive services may include goal setting and stability planning, budgeting support, employment search, transportation assistance, childcare and benefits navigation, help identifying healthcare providers, and much more.

Provides case management services to help facilitate transitions, minimize barriers, and help youth and young adults set and achieve their goals.

Housing Services - Prairie Five Community Action
Coordinated Entry: An access point providing coordinated entry assessments to single adults and families with children who are low-income and meet eligibility requirements in the county area
Family Homeless Prevention Assistance Program: This program helps families or individuals get out of homelessness or prevent homelessness from occurring. If eligible, the FHPAP program can help with the first month's rent, security deposit, or back rent/mortgage assistance up to a certain dollar amount.
To be eligible for this program, individuals must:
- Fill out an application
- Be able to prove a housing crisis (eviction notice, mortgage papers, etc.)
- Must have income
During this program, all applicants who are deemed eligible will also be required to watch the "Rent Wise, Money Smart" videos. The videos go over smart shopping, facts about debt and credit reports, saving, budgeting, and more.
Transitional Housing Program: Provides a long-term housing program to help an individual regain self-sufficiency. The goal of this program is to transition struggling individuals and families into stabilized living situations. The resources are offered for people of all backgrounds, including single parents, children, and individuals that were recently evicted. The program requires an assessment, application approval, weekly case management meetings, and constant contact with a case manager. If approved, Prairie Five can:
- Pay a portion of rent
- Assist with referrals, phone calls, setting up appointments, application assistance, resume building, job search, family development, etc.
- Tenant education
- Budgeting/smart money use

Provides an online application to residents who have past due rent in order to help households remain in their current housing situation. Financial assistance may be provided for past due rent and the Family Homelessness Prevention and Assistance Program (FHPAP), the Local Affordable Housing Aid (LAHA), and the Statewide Affordable Housing Aid (SAHA) funding.

- Purchase and installation of necessary home security devices
- Reimbursement of towing and storage fees incurred due to impoundment of a recovered stolen vehicle
- Replacement of necessary property that was lost, damaged or stolen as a result of a crime
- Transportation to locations related to the victim's needs as a victim, such as medical and criminal justice facilities
- Victim reimbursement for reasonable travel and living expenses incurred due to a change in venue
Victims of crime have also received funding through the program for items such as repairing a broken door, installing new locks, and replacing clothing or bedding taken as evidence.

Agriculture: Works directly with the beginning farmers and build relations between them, local, and regional markets
Asset Building: Assists individuals to achieve long-term economic goals through asset building. Helps establish finacial literacy, identifying edication and training programs. Assists in developing budgets and savings plans for home buyers.
Emergency Housing Services: Assists immigrants and refugees to secure affordable, safe rental housing. Participants in this program are assisted with housing placement, a small short-term rental subsidy (based on income eligibility) and intensive case management to help families overcome barriers to self-sufficiency and obtain sustainable housing.
Homeownership Counseling: Provides one-on-one counseling and group education workshops to educate individuals on the home buying process and determine whether they are ready for homeownership. Individual counseling sessions include housing needs assessment, financial assessment, credit review, and implementation of an action plan to reach each participant's housing goals.
Refugee IDA Program: An one-to-one match program to help refugees save money towards either college, car, home and owning their business. The program provides access to financial trainings, workshops, resources. Southeast Asian Farmers Education: Helps Hmong, Burmese, Karen, and Bhutanese farmers learn how to best grow their farm businesses. Program provides the resources, training, and technical assistance to help beginning and seasoned farmers to improve their income.
