The Hamline Midway Elders Living At Home Block Nurse Program provides or arranges support for elder neighbors to assist them in living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include: - Caregiver coaching and support groups - Community Gardening - Door-to-door transportation, both assisted and non-assisted, provided by volunteers. There is no charge, but donations are accepted. Provides resources for seniors living alone as well as caregivers needing support. Services are designed to help maintain independence as long as feasible. Home-based services provided according to availability of volunteers and staff: - Assisted Transportation: Short distance volunteer drivers provide help to access medical appointments, groceries, pharmacy, etc. Limitations and restrictions apply. - Companionship: Home visits provided by volunteers to provide non-medical support to encourage socialization - Homemaking: Indoor light housekeeping services include mild laundry services, floor care, kitchen/bathroom cleaning and other indoor help. Light "fix it" services (i.e. changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver and to learn from each other - In-Home Respite: Home visits by volunteers who visit with care receiver to temporarily relieve the caregiver. (Services are not home health aide, medical or PCA). - Social Respite: Memory Cafe meets once a month providing conversation, tactile arts/crafts projects, exercise, music and games to temporarily relieve the caregiver while providing socialization for care receivers

Allows the caregiver time to attend to other tasks and provides contact for the home-bound person - Caregiver Support Group: Helps spouses and family members with changes in the family unit; once a month in both Ely and Babbitt - Friendly Visits: Volunteer visits for area residents who might be homebound or just alone and want or need contact with the greater community - Grocery Delivery: Available for those who find it difficult or unsafe to navigate local grocery stores. Volunteers will deliver groceries to the home - Northwoods Memory Care Program: Guides clients and their families through Alzheimer's by helping to reduce stress and increasing family support - Tasty Tuesday: Provides a class to older adults at no cost at the Ely senior center. The class provides healthy recipes, food preparation, and more. Availability is limited. - Volunteer Rides: Coordinates rides that are not available through other services for local and long distance medical appointments, meetings, errands, and social events.

Deployment Services - Coping With Deployments Course: Spouses, parents, siblings, and significant others learn skill-building techniques that help them respond to the challenges of the deployment cycle - Mind-Body Workshops: Offer an alternative method of healing that engages both the mind and body to address common stress reactions that occur within military families and communities - Post-Deployment Support Resources: Information and workshops to help families re-adjust to being together after a loved one's deployment - Pre-Deployment Preparedness Tool for Family Members: Makes sure families are prepared with the information they may need throughout the deployment - Reconnection Workshops: Enhance the likelihood of positive reconnections among family members and successful re-engagement of service members and veterans in civilian life Emergency Communications - The American Red Cross independently verifies an emergency, enabling the service member's commander to make an educated decision regarding emergency leave and then provides transportation assistance and/or financial assistance if needed Financial Assistance - The American Red Cross works in partnership with Military Aid Societies to provide financial assistance which includes funds for emergency travel, the burial of a loved one, emergency food and shelter, etc. Aid Societies determine the financial assistance package that will be offered - a grant or loan. The Red Cross is the mechanism to expedite access to these financial resources. Information and Referral Services - Offers confidential services to all members of the military, veterans, and their families by connecting them with local, state, and national resources through the network of American Red Cross chapters. Local chapters develop and maintain relationships with community partners. Military families rely on the local chapter to help them identify their needs and connect them to the most appropriate chapter and community resources. These services range from responding to emergency needs for food, clothing, shelter, referrals to counseling services (e.g. financial, legal, mental health, etc.), respite care, and other resources. Military and Veteran Caregiver Network - Connect Online: Connect with caregivers in a secure, custom, peer-moderated online community - Peers engage to exchange experiences, empathy, education, and encouragement - Share With a Mentor: Share with caregivers in an understanding, one-on-one, peer-mentor relationship - Join a Support Group: Join caregivers in confidential, peer-facilitated support groups in the community and online Veteran Services - Veterans Claims for Benefits: Assists veterans and their families in preparing, developing, and obtaining evidence to support applicants' claims for veterans' benefits. Also, assists claimants seeking to appeal to the Board of Veterans' Appeals (BVA).

Counselors assist with making decisions and problem-solving related to caregiving. This includes individual or family counseling, support groups, and caregiver training and education. Chronic Conditions Support Group: An opportunity to meet with other people living with a Chronic Condition or Disability to exchange helpful tips, give and receive support, and learn about new resources. Each session offers the opportunity to learn about managing common conditions in later life, as taught by a gerontologist, while also providing valuable social and emotional support from others. Conditions may include but are not limited to: - Arthritis - Depression - Diabetes - Heart disease - High blood pressure - Parkinson's, etc. When Caregiving Ends Support Group: An Elder Network volunteer uses the book, "After Caregiving Ends, A guide to Beginning Again," to facilitate discussion and help in navigating life after caregiving journey ends. Memory Café in Olmsted County: Memory Café is an opportunity for those with dementia and their caregiver to meet in our community for casual conversation and interaction with other people struggling with memory problems. Provides relaxing activities, crafts, drinks, and snacks. Memory Café in Wabasha County: Memory Café is an opportunity for those with dementia and their caregiver to meet in our community for casual conversation and interaction with other people struggling with memory problems. Provides relaxing activities, crafts, drinks, and snacks. This group is also open to older adults without memory concerns who are seeking socialization opportunities
