Advocacy Services for Older Adults and Caregivers Program - Three Rivers Community Action

Provides information, resources, and referrals for services to older adults and their caregivers. Assistance may include application assistance with financial programs, homecare and support group resources, healthcare directive completion, food programs, issues related to caring for someone living with dementia and more. Advocates can assist by telephone, home visit, or a family meeting.

Provides Alzheimer and dementia care in a secure setting with 24 hours/7 days a week onsite healthcare staff, health monitoring, medication management, and skilled RN oversight. Provides private Care Suites with a 5 to 1 customer to staff ratio, 24 hours/7 days a week onsite healthcare staff, medication management, skilled RN oversight, and spiritual care team. Also partners with additional support providers such as hospice and can serve either long-term or respite customers. Provides transitional care services including short-term and respite health care and rehabilitation. Skilled nursing care, 24/7 service, and rehabilitation services including physical therapy, occupational therapy, and speech therapy. With programs for short-term dementia, medication regulation, and a cardio-pulmonary program.

Family Caregiver Connection - Anoka County Human Services - Community Social Services and Behavioral Health Department

- Educational forums about caregiving provided to the community - In-home Respite Services: Volunteer provides companionship for adult care recipients for 2 - 4 hours per week. Provides respite from caregiving responsibilities to individuals providing 24 hours/7 days a week care. - Powerful Tools for Caregivers Classes: 6 sessions designed to provide caregivers with skills they need to take care of themselves - The Gathering: Out-of-home group respite program for caregivers and adult care receivers. Gives caregivers the opportunity to have several hours' respite. Members of Anoka County faith communities have the opportunity to share in the care of the community's seniors. Volunteers supervised by a registered nurse operate the program. Participants enjoy fellowship, lunch and activities while caregivers enjoy support group meetings, education and time away from caregiving. Caregivers may attend support group meetings whether their loved ones participate in The Gathering or not.

Individual Community Living Support - ICLS - HealthStar Home Health

ICLS offers caregiver assistance to those who need intermittent or moderate supervision or physical assistance to stay in their home. Services available may include the following: - Active Cognitive Support: Caregivers provide in-person or remote check-ins to provide support, guidance and problem-solve concerns related to daily living - Activities of Daily Living: Caregivers assist with dressing, grooming, bathing, and transferring (walking) - Adaptive Support: Encourage self-sufficiency and reduce the need for assistance. ICLS caregivers work with the client to develop and practice individualized strategies that promote independence, such as using calendars, making lists, and teaching how to use assistive technology. - Community Living Engagement: Helps clients access resources, services, and activities outside the home. These services help the individual develop and maintain a support system in the community. - Household Management Assistance: Caregivers assist with meal planning and preparation, light housekeeping, laundry and budgeting/money management - Health, Safety, and Wellness: Caregivers will identify any changes in the client's health, including monitoring any existing health conditions, with referrals to case managers as needed
