A year-round program to provide energy saving home improvements to income-eligible households to reduce energy use and cost. The weatherization process begins with an energy audit where trained staff will assess and document the energy conservation needs of your home. Energy Auditors will:

• Check insulation levels in attic and walls

• Inspect attic for air bypasses

• Measure home for heat loss

• Perform a safety and efficiency test on the heating system and water heater

• Provide home energy education

• Provide recommendations for repair, replacement, or improvements of the heating and mechanical systems when necessary

Provides specialized care for individuals in the last stages of a terminal illness. Offers services in palliative care and pain management strategies. Additional services include:

• Bereavement and counseling support to assist family members and significant others in the grief process that occurs with the terminal illness and death of a loved one

• Care of the patient and family, which includes pain and symptom management during the last weeks and months of the patient's life

• Education and presentations to inform community members about hospice care, pain and symptom management, grieving, and bereavement

• Support services and case management for cancer and advanced disease patients and their families during the terminal phase of the patient's illness

Can accommodate up to 35 victims and survivors at a time. The average length of stay is 19 days, but staff work with each client on an individual basis to help meet their needs and secure housing. Advocates work with residents and their families, offering support and connecting them to community resources, such as:

• Connection to internal pet shelter, Linus' Pet Haven for victims who have a beloved pet

• Economic assistance

• Education/Job training

• Medical services

Over half of the residents are children. Advocates and volunteers work in partnership with mothers to provide services for the family and children. Staff work with children on:

In addition, special activities and outings are planned regularly for children to provide free time for moms to plan for the future and fun for the kids. Offers an on-site school. Provides a comprehensive low vision evaluation to assess how vision loss impacts all areas of a person's life. The assessment allows Vision Loss Resources to give recommendations on lighting, training, and devices. Deaf-Blind Adults and Children: Provides one-on-one support to people with combined hearing and vision loss, from birth through adulthood Older Adults: Provides in-home assessments and individualized training to help seniors stay independent and age in place Professionals and Caregivers: Provides hands-on trainings and informative presentations for a workplace, senior residence, civic group, or community center Working-age Adults and Young Adults Transitioning Out of High School: Provides comprehensive vision rehabilitation training to help individuals adapt and thrive at home, work, or school. Classes include orientation and mobility, technology, typing, Braille, tasks of daily living, cooking, and more.

Provides opioid addiction treatment and recovery services that may include a mix of the services below. Opioid addiction treatment specialists provide each patient with custom plans designed to support recovery.

Offers several senior nutrition programs:

• Community Dining Locations: Provides affordable meals in a community atmosphere at several senior housing center locations

• Dine-In or Take-out Service Options: Local restaurants offer meals throughout the day or over the lunch hour with no reservations required

• Meals on Wheels Home-Deliver Options: Hot, nutritious meals are typically delivered Monday - Friday to homebound older adults. Frozen meals are available for weekends, holidays, and other closure days. Clients choose their delivery schedule (minimum of 3 meals per week). They may also choose to have a hot meal delivered daily by a volunteer or choose a combination of hot meals and frozen meals delivered on the days that work best for them.

• Bundled Service Program: Nutritious frozen meals, food shelf commodities, and Nutritional Assistance Program for Seniors (NAPS) food boxes are available to older adults living in rural areas outside of the local Meals on Wheels delivery area. 10 or 14 meal packs are delivered in Aitkin, Itasca, Lake, and Saint Louis counties. This program also serves as a safety net for participants, as check-ins are provided to the individual.

In addition to nutritious meals, the service includes nutrition screening, assessment, and education. A Registered Dietician completes a nutritional analysis and approves the cycle menus. Meals comply with the most recent Dietary Guidelines for Americans and provide a minimum of 1/3 of the dietary reference intake established by the Food and Nutrition Board.

• Psychiatric Evaluation: A comprehensive psychiatric evaluation is provided to understand and diagnose mental illnesses for both children and adults. Care coordination is also provided to support and guide clients in all aspects of their health and well-being.

Provides housing assistance for low-income individuals and families that include at least one adult with a serious mental illness while they wait for a Housing Choice Voucher (HCV) or another rental subsidy. Eligible use for Bridges funding includes temporary rental assistance payments and security deposits paid directly to the landlord on behalf of qualified participants. Administers programs that offer housing assistance payments as well as support services to assist the family in becoming more self-sufficient through the following services: Administers federal Section 8 rental assistance program in all of Anoka County, all of Carver County, all of Ramsey County except for St. Paul, and all of Hennepin County except for Minneapolis, Bloomington, Richfield, St. Louis Park, and Plymouth. Administers other federal and state-funded rental subsidy programs throughout portions of the Twin Cities metropolitan area, including: Bridges, Shelter Plus Care, and Housing Assistance Program. In each of these programs, housing is coordinated with the applicable social service provider for eligible families. Administers federal low-rent, scattered site housing program in the cities of Blaine, Coon Rapids, Maple Grove, Edina, Eden Prairie, Minnetonka, Golden Valley, Shoreview, Roseville, and Plymouth

Provides establishment and enforcement of child support orders. Services include: ?- Collecting current and past-due support payments

• Locating non-custodial parents

Provides assistance, investigates, and/or intervenes on behalf of vulnerable adults who are unable to act on their own behalf, manage their own affairs, or who are in immediate danger due to physical or emotional abuse, unsafe or hazardous living conditions, exploitation, neglect, or abandonment. Services may include:

• Authorization for medical treatment

• Consultation to individuals who suspect abuse

• Removal of the individual to safer surroundings

• Services necessary to remove the conditions which have created a threat to life

Cash-grant program for families with low incomes experiencing household emergencies. Families can receive assistance once every 12 months. The assistance received must resolve the crisis. Families must use their own money first. The amount of assistance provided might not cover the entire emergency, but it can help. The types of assistance the program may help with are for rent assistance, rent deposit, electric, gas, heating fuel, water, burial assistance, or other emergency need based on available funding.

Financial counselors can help individuals understand patients' responsibilities and may offer no-interest payment plans. Other options may include: Community Care Program: Individuals who are uninsured or underinsured and have incomes less than 200% of the Federal Poverty Guideline may be eligible for help to pay their medical bills
