The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for: Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA) Home and Community-Based Services Waivers based on age:

• Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW)

• Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC)

Intellectual and Developmental Disability (IDD) Services (also known as Rule 185 Case Management) Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.

Provides emergency financial assistance to adults in active cancer treatment for basic, non-medical needs. Assistance is provided once per lifetime for needs such as:

• Cub Foods gift cards

• Gasoline purchases using Holiday fuel cards

• Rent or mortgage payment

• Home or cell phone bill

• Gas, electric, or water bill

• Garbage bill

• Adult tennis lessons

• Day camps with swimming, hiking, fishing, fire safety, first aid, water and boat safety, severe weather safety, baseball, soccer, lacrosse, flag football and water skiing

• Field trips

• Kop Farm community garden

• Music in the Park free concerts

• Overnight camps with fishing

• Playground programs

• Seasonal events for spring, summer, fall and winter activities

• Softball

• Tot Lot program

• Yoga

Offers educational programs such as:

• Best Buy Teen Tech Center: A drop-in space for teens ages 14 - 18 to be creative and develop new skills. The center helps youth expand their interests and discover career paths through technology-based projects in programming, film-making, music production, graphic design, and much more.

• Citizenship Classes: Prepare students to pass the United States Citizenship exam successfully. Classes are taught in English only. The class topics include the process of becoming a citizen, United States history and government, interview preparation and practice, study materials, and other resources.

• English Language Classes (ESL): Classes open to anyone who would like to improve their English skills. Students learn how to read, write, speak, and understand the English language while also gaining essential life skills that will better prepare them to navigate life in the United States. Classes are offered online on Zoom.

• GED Preparation Classes: Help students prepare to take the GED (High School Equivalent) exam. Classes cover all testing subjects, including math, social studies, science, and language arts (grammar and writing). Classes are offered in person, while Math courses are offered online on Zoom.

• Youth in Action (YA!): A Latine-focused youth program for 9th - 12th grade students in the Twin Cities Area providing mentorship, leadership development, skill-building opportunities, college preparation, community service-learning projects, and innovative programs.

Helps people develop skills and knowledge to improve their lives and communities. Offers a variety of programs for learners of all ages. Class offerings include arts and crafts classes, babysitting safety training, cooking classes, driver's education, firearm safety courses, health classes, sports and recreational classes, swimming lessons, youth enrichment classes, and more.

The program allows low-income individuals fares of $1 per trip on any bus (express or non-express) or light rail train at any time of day. Transfers included. Cards will come loaded with $5 and can be refilled at any Go-To card outlet, online, or rail ticket machines. NOTE: Discount does not apply to Transit Link or Metro Mobility buses; a partial TAP discount of $3.25 is valid for Northstar fares.

Promotes lead screening at clinics, educating high risk children and families, and providing health services to those with elevated lead levels.

Collects plasma for medical purposes and compensates donors.

Offers the Volunteer Income Tax Assistance (VITA) program to eligible individuals who need help filing their taxes. Tax appointments are offered in-person and virtually throughout southwest Minnesota. Taxes are provided for state, federal, and property tax/renter's credit returns. NOTE: Some complex tax returns cannot be completed by the VITA program.

Provides children safe and conflict-free time with parents through a variety of services delivered by trained providers. Each parenting time center provides a neutral location for drop off, pick up, and visitation of children. Services include:

• Parenting classes

• Safe exchanges and supervised visits

• Scheduling assistance

• Supervised phone calls and virtual visits

Program offers:

• Common assessments are available for those experiencing homelessness to be placed on a list for housing programs in coordinated entry

• Individuals and families experiencing homelessness can be screened through Tri-Cap for housing-related coordinated entry by completing a common assessment

NOTE: No longer assisting with direct rental assistance programs, may provide referrals Individuals and families must go through the Coordinated Entry System and must be active by completing a common assessment to be eligible for a referral to one of the housing programs.

Assistance paying child care costs for children up to age 12 and for children with special needs up to age 14. Child care costs may be paid for families while parents work, look for work, or attend school. Family size, family income, and participation in authorized activities are considerations in determining eligibility for and amount of assistance, along with available funding.

Assisted living units providing:

• Basic daily living services

• Housing

• Nutrition

• Nursing assistance

• Occupational therapy and activities

Helps families work their way out of poverty by providing temporary cash and food benefits to eligible families. While MFIP has a 60-month time limit, there are some limited exceptions and extensions. Participants are required to follow certain work rules. Failure to meet these requirements will result in MFIP grants being sanctioned or reduced. In addition:

• Child support will be pursued in those situations where one or both parents are absent
• A woman expecting a baby may receive assistance for herself during her pregnancy?- Children who are being cared for by certain relatives may receive assistance

?NOTE: MFIP recipients may be eligible for Emergency Assistance, Child Care Assistance, Medical Assistance, and/or Food Support.

Licensed residential living programs for people with acquired or congenital disabilities such as intellectual developmental disabilities, autism spectrum disorder, or brain injuries. Programming consists of:

• Cooking

• Individual living skills instruction

• Medication management

• Money management

• Positive behavior supports

• Recreational activities

• Self-help, wellness, and symptom management

• Socialization

Safe Harbor Programming for youth ages 17-24 who have been victims of sexual exploitation. Crime Victim Services: Advocacy and support and services for youth ages 9-24 who have been victims of a crime. Youth-At-Work: Assistance to low income youth ages 14-24 who are needing support and assistance finding jobs, completing applications developing a resume, interviewing skills, etc.

Financial resource services include:

• Billing and insurance program

• Patient assistance program: Assistance with payment for medications not covered by insurance or high co-pays

• Sliding fee scale program: Fee reduction program for medical and dental is available to patients based on income guidelines, regardless of other coverage. Transportation assistance may be available for Sliding Fee Scale patients who need help with the cost of getting to the clinic.

Provides more support than traditional assisted living but ideal for individuals who do not require around-the-clock skilled nursing care in a home-like setting. Services provided:

• 24 hours/7 days a week tailored assistance

• Advanced medical support

• Complex medication management

• Diabetes and chronic disease management

• Help with daily activities

• Opportunities to stay active and engaged

• Personalized care

• Transfer assistance and mechanical lifts

Governing offices for Underwood and services, including water, sewer, and economic development.

Offers older adults independence and support in a safe and secure setting. Services offered:

• Assisted living suites

• Homemade meals and three snacks daily with dietary considerations

• Housekeeping services such as linen and personal laundry services

• Medical services onsite such as nursing, home care, and physical therapy

• Opportunities to spend time in a community setting with fun activities and excursions

• Short-term out of home respite care

• Staff management onsite

• Consultation and assessment services for medical, psychological, and case-related needs in suspected child abuse or neglect cases; staff also provide professional consultation using current best practices and available research

• Expert testimony and case consultation for law enforcement, child protection, physicians, therapists, and attorneys involved in child maltreatment investigations

• Medical evaluation and diagnostic services, including scheduled appointments and 24/7 availability for professional phone consultation regarding urgent child abuse concerns

• Psychological assessment and therapeutic services for child victims, including trauma-focused evaluations and therapy

• Referrals to appropriate medical, psychological, and community resources to support ongoing care and safety planning for children and families

• Training, education, and professional development opportunities, including specialized training for child-abuse professionals and clinical training programs

Care consultation provides adults age 60 or older, their caregivers, and families with knowledge, skills, tools, and resources to help older adults lives as independently as possible. Consultants work over the phone or in the home to identify services to meet needs. There are no income guidelines, and services can be provided for as long as needed, with no minimum or maximum visits required. Care Consultants can:

• Advocate on your behalf to ensure you receive the services you need
• Connect clients to services that increase health and independence
• Create customized action plans
• Educate regarding chronic disease management and prevention
• Provide family mediation
• Fill out legal documents such as health care directives
• Identify community resources
• Identify risks in the home that compromise health and safety
• Help with managing difficult family decisions
• Help with planning for the future
• Help with problem solving

Coordinates and funds scholarships for youth and adults to attend programs in specific industries, at approved educational institutions
