Community Support Program (CSP) helps people to maintain functioning in their homes or community. Services are provided individually or in groups by one of the following staff: mental health practitioner, mental health worker, or certified peer specialist. Peer mentoring services provides help with problem-solving, goal setting, social skills, and community integration; peer mentors can provide feedback, advocacy, support, and role modeling. Services available 1:1 or in a group setting. Services are provided by a person who has a lived experience with a mental health diagnosis. Peer Mentoring is offered through the CSP in Big Stone, Chippewa, Kandiyohi, Lac qui Parle, and Swift counties.

Provides cash assistance to help adults receiving Supplemental Security Income - SSI pay for their basic needs. Adults who are blind, have a disability, or are age 65 and older but don't qualify for SSI because of their income may still be eligible for MSA if they meet income requirements. The basic monthly MSA grant is calculated using either the maximum SSI amount (Federal Benefit Rate) for the adult or their countable income (if they do not receive SSI), and the MSA standard set by state law. Special Needs Grant MSA also may provide special payments for adults who need:

• Certain home repairs

• Certain household furniture and appliances

• Guardian or conservator service fees

• Help paying for housing costs through MSA Housing Assistance

• Medically prescribed special diets

• Representative payee services

? Adults who receive MSA are also eligible for help with medical, food, and housing costs through Medical Assistance - MA, the Supplemental Nutrition Assistance Program - SNAP, and MSA Housing Assistance programs.

Refugee Cash Assistance provides temporary financial assistance for refugees and eligible entrants while they are in the process of resettling in the United States. NOTE: Able-bodied RCA recipients are required to participate in an employment and training program. RCA recipients who are disabled or 65 years of age or older are required to apply for SSI.

Supports independence and allows older adults to remain in their homes and communities. Services for household and outdoor maintenance include:

• Home barrier removal, disability-related projects such as grab bar installation

• Homemaking (dishwashing, laundry, grocery shopping, meals, and cleaning)

• Interior and exterior painting

• Lawn mowing

• Snow removal

• Seasonal outdoor work (raking, pruning, weeding, mulching)

• Minor repairs (leaky faucets, door handles, replace light bulbs)

Provides police and emergency response for the city of Aitkin

Provides a comprehensive plan for counseling, education, and support. The goal is to meet the mental health needs of White Earth Tribal members and their families by increasing family interactions, healthier lifestyles, and positive communication for all members. Services include:

• Adult Rehabilitative Mental Health Services (ARMHS): Holistic approach that places the person at the center of all interventions. The goal is to restore each person's ability for independent living, socialization and effective life management. ARMHS instructs, assists, and supports the person in areas such as medication, education, and monitoring basic social and living skills. Assistance is also provided with mental illness symptom management, household management, employment support and transition to community living.

• Assessments and evaluations for mental health. Diagnostic assessments and psychological testing for all ages.

• Case management services

• Children's Therapeutic Services and Supports (CTSS) to provide psychotherapy and skills training

• Crisis intervention

• Mental health counseling for adults, children, and families. Services are based on individual, child, and family needs.

Provides psychiatry services including evaluations, treatment, and medication management for depression, anxiety, and other psychiatric disorders. Services are provided at all HDC locations and at contracted locations.

Provides general information and licensing for persons interested in becoming foster care providers. Training and support for foster care providers on emergency, short-term, and long-term placement.

Discount cards provide savings of up to 80% on prescription drugs. Pet prescription drug discount cards are free and available online:

• Cards never expire and can be used immediately

• Information about pet medications

• Print the card from the website; it's pre-activated and ready to use. Print or text of the coupon can be used. Present the pet drug coupon to the pharmacist when paying for prescriptions.

Training for parents on how to manage their child's behavioral issues while improving their relationship with their child. Assistance includes:

• Aggressive behaviors
• Attention deficit hyperactivity disorder (ADHD)
• Exposure to alcohol/drugs before birth

• Severe temper tantrums
• The impact of abuse/neglect

Requires a 12-14 week commitment to an hour-long session per week

Partners with members of the anti-hunger community to address the challenges of food insecurity throughout the state of Minnesota. Advocates on hunger issues at the local, state, and national levels encouraging legislators and other decision makers to lend a compassionate voice to public policy debates that impact Minnesota families living in poverty.

Works to develop customized jobs in the community that capitalize on each individual's interests, abilities, and personal goals. Provides personalized support to each individual, including job coaching, skills training, and job search assistance. Assists individuals living with disabilities who are looking for:

• Employment opportunities

• Interested in learning a new skill or is working on developing new ones

• Wants to be involved within the community

Provides comprehensive care management and coordination for older adults and people with chronic or complex health conditions. Support patients and caregivers by answering questions, facilitating communication among providers, helping implement individualized care plans, identifying community resources, supporting transitions of care, assisting with paperwork, and participating in care planning and conferences as needed.

• Provides education and car seat distribution to eligible clients
• Specific car seat availability and eligibility depend on insurance, child age, and resource availability as determined by intake. Car seat distribution appointments take 60 - 90 minutes
• Provides car seat checks for those with existing car seats that want to make sure the seat is installed correctly

• Provides bus service in Apple Valley, Burnsville, Eagan, Prior Lake, Rosemount, Savage, and Shakopee
