Housing: - Women and children live in a shared living community - The house has two living units, each unit accommodating up to a total of 8 women and their child(ren) - Residents may stay for up to 1 year Long-term stability plan: - Each resident is required to develop a long-term stability plan - Meet with a case manager monthly to work on the plan - Residents are expected to be employed - Residents work on monthly budgeting - Work on parenting goals and participate in referred parenting programs - Participate in recommended community-based services such as counseling, chemical dependency or mental health aftercare
Transitional housing program that fulfills the need for healing time and personal development that combines best practices with a Christian environment. Services include: - Safe housing; food and clothing referrals; case management/advocacy; support group; domestic violence education courses; individualized therapy; life skills education; financial education and counseling; assistance to pursue education, jobs and housing; bible studies and prayer ministry - Women and children receive a comprehensive program for 12-18 months with the goal to equip and empower them to live free of abuse - Referrals are given to men who have been abused (i.e. Cornerstone, Domestic Abuse Project and Tubman) or wish to change abusive behavior (i.e. community groups) - Programs for women and children provide paths to emotional, spiritual and physical safety, and health. Mentors, tutors and educators are provided to educate, equip and empower women to increase confidence and quality of life as well as family stability. Goal is to help women and children transition to lives free of abuse.

Independent living in apartments for 16 - 21 year olds - Housing and independent living support services for homeless youth, ages 16 - 21. Please note that 16- and 17-year-olds need parent or guardian permission. We can help you negotiate with your parent or guardian if we think independent housing is the best option for you. - Housing and independent living support services for teen parents, ages 16 - 21, who are homeless or living in inadequate housing. We can help you find an apartment, complete the application, pay security deposit, and help with monthly rent. - Case management services to help you with education, employment, parenting, child care, and more. We can also help you with obtaining medical insurance and accessing medical, dental, or mental health services. - All youth participating in transitional housing have access to: Individual, and/or family counseling appropriate to the individual needs of single teens and teen parents. - Skill building workshops for single teens and teen parents to increase self-sufficiency and independence.

Potential assistance consists of: - Funding for housing and utility costs/deposits - Funding for transportation costs via vehicle repair or public transit - Clothing and food assistance as well as other basic needs - Housing stability case management Programs may include: - Assistance for homeless ex-offenders in Chisago, Isanti, Kanabec, Mille Lacs, and Pine County, housing placement, as well as rent and deposit funds, may be available - Limited fund for short term hotel stays in tandem with case management services- ?- Long-term homelessness supportive services, also known as coordinated entry, are provided for those who qualify for short term hotel stays in Aitkin, Carlton, Isanti, Mille Lacs, Kanabec, Pine, and Chisago County to help with housing search and placement for families and unaccompanied youth - Transitional housing services for homeless youth ages 16 - 22, as well as some financial assistance for rent ? ?NOTE: Funding comes from a variety of sources, including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Children must be residents of Jeremiah Program in order to be eligible. - Housing program offers 39 units, two- and three-bedrooms, meeting room, laundry room, computer lab, lending library, personal development classes, educational programs (tutoring and life skills), employment program, case management, supportive counseling, and parenting education. - Support for low-income mothers who are motivated to move from economic dependence to self-reliance

Most transitional houses are Single Room Occupancy (SRO), though entire houses may be available for families. Veterans must be able to live independently. The length of stay varies, but the maximum stay is two years. The rent is determined on a sliding scale - no greater than $500/month. All units require a sober environment (no drugs or alcohol on the premises) but does not require sobriety. The only exception is Building 47 on the Minneapolis VA Medical Center Campus as this transitional housing requires sobriety.

Housing is made up of 9 private rooms each containing a bed, sink, microwave, small fridge, TV, and bathroom/shower.
