The Family Home Visiting Program's goal is to prevent child abuse. Public Health Nurses provide home visits over an extended period to develop goals for the child(ren) and family, provide education on child development and home safety. The nurses also provide developmental screenings, environmental assessments and referrals to other community resources that may be beneficial for the family. This program also provides information about safe sleep for infants including specific guidelines for parents.

Provides home care services for persons coming home after a hospital stay, surgical procedure or are unable to leave their home for medical care. Services include the following: - Assistance obtaining medical equipment and supplies - Assistance with Medicare, Medicaid or commercial insurance billing - Coordination of community resources - Home health aides provide personalized support for daily life activities, such as bathing and dressing. - Home infusion therapy for patients requiring intravenous medications - Home safety evaluation and fall prevention education - Home telemonitoring - Lab tests - Medical social services - Nursing services (including assessment, patient education, medication management and skilled nursing procedures) - Nutrition counseling - On-call nursing consultation 24 hours/day - Physical, occupational and speech therapy - Physician communications - Wound care and educational support for ostomy and wound management Note: Does not provide Personal Care Assistance (PCA) services.

Physical therapy uses exercises, manual therapy, and other techniques to treat conditions that affect an individual's physical function. Physical therapy may help an individual: - Avoid the need for medication or surgery - Improve mobility, coordination, flexibility, and strength - Recover after an injury, illness, or surgery - Reduce pain - Regain independence Therapists may also conduct evaluations like functional capacity evaluations. Fall Prevention Physical therapists work with individuals to prevent falls by: - Assessing an individual's risk factors, including certain medical conditions and medications - Educating an individual about fall prevention - Helping make an individual's home safer - Providing dizziness and balance care - Teaching an individual exercises to improve their strength and stability - Testing an individual's balance and ability to walk Orthopedic Rehabilitation Physical Therapy Type of physical therapy that's used to treat conditions like: - Arthritis - Back, neck, jaw, joint, or muscle pain - Broken bones - Injuries or conditions that affect the shoulder, elbow, hip, knee, foot, or ankle - Sprains and strains - Sports and exercise injuries Physical Therapy for Children Therapists help children improve functional gross motor skills like rolling, sitting, walking, or skipping. Physical therapists also teach children how to use a wheelchair, walker or other type of mobility equipment. Specialized PT Services Physical therapists offer specialized physical therapy services for the treatment of: - Back and neck pain - Balance and dizziness-related problems - Hand conditions - Lymphedema - Neurological conditions like stroke or brain injury - Pain rehabilitation - Pelvic floor disorders

Volunteers screen homes for potential safety hazards such as scatter rugs, lack of lighting, lack of grab bars, etc and will provide recommendations for follow-up to improve safety

- Child safety seat checks - Fire department - Home Safety Survey: Provides home inspection, identifying potential hazards and discussing solutions. Also creates a fire escape plan for all members of the family, practices fire drill, provides smoke alarms and fire extinguisher inspections, educates families on poisons, carbon monoxide, radon, burn and fall prevention as well as crime prevention. Upon completion, a free smoke alarm is installed (if needed) and homeowners receive coupons for safety products discussed during the survey.

The Living at Home Block Nurse Program provides support services to elders and their caregivers so they may continue living in their home as long as possible. Home Repair Program: Provides volunteer-delivered repairs including weatherizing, cleaning, installing flooring, patching and painting, siding, and landscaping, and timely contractor-delivered repair or replacement of essential systems, such as HVAC, electrical, plumbing, outer envelope, and roofs that are critical to healthy, livable homes. Safe at Home Program: Provides minor home safety and accessibility modifications at no cost to homeowners in need. Repair services will allow homeowners to remain independent and live safely in their homes for a longer period of time. Types of repairs may include grab bars, handrails, wheelchair ramps, and shower seats.

The public health nurse may also assign a licensed practical nurse or home health aide to assist in the care of the client. Home health aides/elder care aides provide personal care (hair washing, showers, nail care, etc.), visiting, light housekeeping, and other services for clients with medical or health problems. Aides may also monitor vital signs and provide basic health services. The public health staff is available to perform home safety checks for families with children or for elders. A limited number of safety items may be available following the home safety check. Instruction on how to use this equipment will be provided, although the nursing staff cannot install the equipment for families. Community Health Representatives (CHR) Transportation Provides transportation for individuals to medical, dental, and optical appointments only after all other resources are exhausted. Appointments should be confirmed with the CHR's schedule to assure that clients arrive on time to their appointments. No one under the age of 18 will be transported without a parent. All persons being transported are required to wear a seat belt at all times. Delivers medical prescriptions to homebound elders who have no other means of obtaining their medication.

Home safety program for owner-occupied single family homes. Contact the program or go online to receive a safety checklist. The homeowner should go through the checklist and make needed changes, then contact the Fire Department by filling out the form enclosed with the checklist. Firefighters will then make an appointment to visit the home and make sure all smoke alarms are working. While supplies last, homeowners can also receive four free smoke alarms, one stovetop fire extinguisher, and a digital carbon monoxide detector.
