Evidence-Based Health Education and Support - Southeastern Minnesota Area Agency on Aging - Rochester

Arthritis Foundation Exercise Program: Exercise, nutrition and health education, fall prevention, and medication management program for older adults in multiple locations around the region. Leaders are certified Arthritis Foundation instructors. Living Well with Chronic Conditions: A 6-session class to help people cope with chronic conditions; lead by 2 trained leaders. Various locations and trainers available through out the region. Matter of Balance: A class to learn about falls prevention that meets 8 times and is lead by 2 trainers, one whom is a volunteer. PEARLS: A community-based treatment program for older adults with depression. A PEARLS coach will meet with the person for eight, one hour sessions that are customized to support them and their needs. Learn how to problem solve and change their behavior to support their well-being. Powerful Tools for Caregivers: A 6 week class for caregivers to enable them to provide care for a loved one longer and with more confidence and control. Class is lead by 2 trainers and is available in various locations throughout the year. Tai Ji Quan: Moving for Better Balance helps older adults improve their balance and reduce the likelihood of falling. Research has shown that participants are half as likely to fall and are less fearful of falling. Participants typically have improved physical health that leads to better functioning in daily activities, improved balance, muscle strength, flexibility and mobility, and better mental health.Classes beginning in some areas in 2015.

Provides resources for older adults living alone as well as caregivers needing support. Services are designed to help maintain independence as long as feasible. Home-based services are provided according to the availability of volunteers and staff: - Homemaking: Indoor light housekeeping services include mild laundry services, floor care, kitchen/bathroom cleaning, and other indoor help. Light "fix it" services (i.e., changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans, and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver, and to learn from each other

Dementia Friends-sessions: Provides to community members of all ages. Healthcare specific educational session is available, as well as adaptations for particular workplaces, known as Dementia Friendly in the Workplace. These are one-day sessions that last from 60-80 minutes. Educational session available to community members who have attended a Dementia Friends session, to become Dementia Champions, so that they too, can provide Dementia Friends sessions. This is a one-day session. Powerful Tools for Caregiving (PTC): Educational program designed to help family caregivers take care of themselves while caring for a relative or a friend. Focus is on those caring for someone who is 55 years or older. Six weeks of 1.5 hour workshops. Offered both virtually, and in-person, depending upon the time of registration. Call to register for this workshop. Caregiver Training Workshop: This workshop is approximately 2 hours long. Must pre-register and the workshop is provided in-person. The training walks through a workbook that allows the participant (caregiver) to develop a plan of how to care for their care receiver if something should happen to them and they are no longer able to care for that older adult-either temporarily or definitively. Fraud and Scam Prevention Trainings: Provides presentations relating to fraud and scam prevention to the communities of service-upon request. ?Programs are provided at a variety of locations, including Elder Network offices and others. Some programs are available virtually.

Caregiver Support: Caregiver counselors available to provide support for caregivers to continue caring for a loved one and care for themselves. Counselors assist with making decisions and problem-solving related to caregiving. This includes individual or family counseling, support groups, and caregiver training and education. Family Caregiver Connection - Anoka County Human Services - Community Social Services and Behavioral Health Department

Provides education, support, and respite services for caregivers of adults and seniors in Anoka County. Services include: - In-Home Respite Services: Volunteers provide companionship to adult care recipients for 2 to 4 hours per week, giving caregivers a break from daily responsibilities. - Powerful Tools for Caregivers Classes: Six-session workshops that teach caregivers skills to care for themselves while caring for others. - The Gathering: An out-of-home group respite program where adult care recipients enjoy fellowship, lunch, and activities while caregivers participate in support groups, education, and time away from caregiving. Volunteers supervised by a registered nurse help operate the program. Caregivers may attend support groups even if their loved ones do not participate.

REACH Coaching - Elder Network - Southeast Minnesota

Provides Resources for Enhancing Alzheimer's Caregiver Health (REACH). This program is an approach used by caregiver consultants to support family members and others who are caring for someone with Alzhiemer's disease or a related dementia. The goals are to teach caregivers how to reduce stress, solve problems, and manage difficult behaviors the person with memory loss may display; as well as to help caregivers feel confident in the care they provide and improve their ability to cope. After the caregiver completes the REACH assessment, 4 core sessions are offered and tailored to what the caregiver most needs. The caregiver consultant holds the sessions in person (though alternative service options may be available via phone) typically over 2 - 3 months.
