The Hamline Midway Elders Living At Home Block Nurse Program provides or arranges support for elder neighbors to assist them in living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include: - Caregiver coaching and support groups - Community Gardening - Door-to-door transportation, both assisted and non-assisted, provided by volunteers. There is no charge, but donations are accepted. - Educational forums about caregiving provided to the community - In-home Respite Services: Volunteer provides companionship for adult care recipients for 2 - 4 hours per week. Provides respite from caregiving responsibilities to individuals providing 24 hours/7 days a week care. - Powerful Tools for Caregivers Classes: 6 sessions designed to provide caregivers with skills they need to take care of themselves - The Gathering: Out-of-home group respite program for caregivers and adult care receivers. Gives caregivers the opportunity to have several hours' respite. Members of Anoka County faith communities have the opportunity to share in the care of the community's seniors. Volunteers supervised by a registered nurse operate the program. Participants enjoy fellowship, lunch and activities while caregivers enjoy support group meetings, education and time away from caregiving. Caregivers may attend support group meetings whether their loved ones participate in The Gathering or not.

This program is an approach used by caregiver consultants to support family members and others who are caring for someone with Alzhiemer's disease or a related dementia. The goals are to teach caregivers how to reduce stress, solve problems, and manage difficult behaviors the person with memory loss may display; as well as to help caregivers feel confident in the care they provide and improve their ability to cope. After the caregiver completes the REACH assessment, 4 core sessions are offered and tailored to what the caregiver most needs. The caregiver consultant holds the sessions in person (though alternative service options may be available via phone) typically over 2 - 3 months.

Volunteers will deliver groceries to the home Northwoods memory care program: Guides clients and their families through Alzheimer's by helping to reduce stress and increasing family support Tasty Tuesday: Provides a class to older adults at no cost at the Ely senior center. The class provides healthy recipes, food preparation, and more. Availability is limited. Volunteer Rides: Coordinates rides that are not available through other services for local and long distance medical appointments, meetings, errands, and social events.

Provides resources for seniors living alone as well as caregivers needing support. Services are designed to help maintain independence as long as feasible. Home-based services provided according to availability of volunteers and staff: - Assisted Transportation: Short distance volunteer drivers provide help to access medical appointments, groceries, pharmacy, etc. Limitations and restrictions apply. - Companionship: Home visits provided by volunteers to provide non-medical support to encourage socialization - Homemaking: Indoor light housekeeping services include mild laundry services, floor care, kitchen/bathroom cleaning and other indoor help. Light "fix it" services (i.e. changing light bulbs, smoke detector batteries, etc.), meal preparation, menu planning, picking up groceries and medications, help with organizing bills, etc. Supportive programs: - Caregiver Consulting: Helps caregivers manage and live their lives while caring for another person, develop individualized plans and connect with resources in the community - Caregiver Support Groups: Professionally facilitated, confidential environment for caregivers to share concerns, feelings regarding their role as caregiver and to learn from each other - In-Home Respite: Home visits by volunteers who visit with care receiver to temporarily relieve the caregiver. (Services are not home health aide, medical or PCA). - Social Respite: Memory Cafe meets once a month providing conversation, tactile arts/crafts projects, exercise, music and games to temporarily relieve the caregiver while providing socialization for care receivers

Works with organization to identify and implement changes needed to better assist their customers and their loved ones. Evidence-based programs helps promote self-management and understanding those with chronic health conditions (such as diabetes and chronic pain), fall prevention, and encourage improving the overall state of well-being through education, fitness instruction, and resources.
