Based on individualized service plans, tenants can receive the following services: Crisis intervention, parenting education, individual and group counseling and therapy, job readiness and retention skills training, chemical health support and therapy, and life skills training. Community-building activities are scheduled on a monthly basis to provide education as well as develop a sense of community among the tenants. These activities include on-site sessions as well as off-site outings. Provides homeless, mentally ill single parents with permanent, affordable, chemically-free housing so that these families can move towards stability.
Assists homeless single adults currently residing in a Hennepin County shelter who have been referred by a county screener in finding safe, decent and affordable housing in Hennepin County. The Rapid Exit Manager also assists clients with meeting and maintaining basic needs and connecting to other resources needed to achieve self-sufficiency.
Family shelter information and locations for Hennepin County funded shelters. Families with children in need of shelter must contact the Hennepin County Shelter Hotline during regular hours. If needing a shelter afterhours, families can contact Greater Twin Cities United Way 211 for information about a family shelter in Hennepin County.
Scattered site rapid re-housing program for low income, single-parent families who are homeless. Services include assistance in finding affordable housing, case management, and advocacy. Families usually consist of two or more dependent children (of any age), and program services are provided for up to two years.

Village of Hope provides safe, secure emergency housing for families 24 hours/day ~ 7 days a week. We are open 365 days a year. The environment that a family experiencing homeless comes into is very important. We work to create a calm, safe environment in which families can build a future. Families can stay with us for 90 days, as long as they are actively looking for housing. Each family has their own apartment. The primary focus of the Homeless to Housing team is to fill system gaps by offering case management to people who otherwise lack support and access to resources by helping them secure permanent housing. The Homeless to Housing teams can help single adults, families, youth, and veterans who are staying in shelter or those living unsheltered with the following: 1. Referral 2. Intake 3. - Individuals can obtain a community voice mail number with a 651 area code, to use on job applications, housing applications, and stay in touch with friends and family. Individuals will be able to retrieve messages from any touch tone phone. - Offers assistance to apply for the Lifeline cellphone program. Eligible participants will receive a free monthly phone plan which includes 1,000 minutes of talk, 500 units of text, and 100 MB of data.

Emergency Shelter and Day Center for people experiencing homelessness. Servants of Shelter operates a 5 apartment unit Family Shelter. Shelter guests receive Case Management and Housing Resource Navigation services during their 30-90 day stay. SOS operates a Day Center with laundry, showers, a communal kitchen, hygiene items, and more. SOS also operates two Permanent Supportive Housing programs for people living with disabilities who are also experiencing homelessness.

Front Door consolidates intake for most programs and connects clients to the level of service that best meets their needs. Front Door Screener's connect clients to: - Community resources or other providers - Coordinated Entry System (CES) assessments for families based on eligibility criteria. Families can visit the website to fill out a Families Coordinated Entry referral form. - Education or consultation of county programs - Intake assessments of county-operated programs. For clients identified as needing county intervention, a Front Door Service Planner will conduct a thorough evaluation of needs and provide short-term case management as well as assess client eligibility for possible ongoing services from the county or a contracted provider.

Provides a non-emergency response to homeless concerns in public settings with a mobile outreach team able to visit individuals in the community. Can help address immediate basic needs for safety and survival outside with items such as food, personal grooming supplies, outdoor gear, clothing, and shelter referrals. Low income and individuals experiencing homelessness can pick up a free Handbook of the Streets from 9 am - 4 pm Monday - Friday or download the PDF from the website.
