Board and Lodge: Offers four Group Residential Housing Properties (Board and Lodge homes) located within the Winona community. Qualified applicants share a home and have services provided to support their medication management, skills development, and resource connection by our qualified professionals. Hiawatha Bluffs Living (HBL): A long-term permanent supportive housing facility with a month-to-month lease. Requires an initial 1-year lease commitment. Actively utilizing treatment services is also strongly encouraged. Scattered Site Supportive Housing: Partners with landlords in the Winona area to place clients in private rental units. The size of the unit, amenities, and lease conditions depend on the landlord and are different for each scattered site unit. All clients receive meaningful support from HVMHC staff that includes things like assisting with grocery shopping and providing transportation to medical appointments. One specific scattered site housing option are the apartments at Water's Edge in Winona.

Emergency Rental Assistance Program
  • Assist with one month's rent and security deposit not to exceed the value of one month's rent. Assistance is not available to households who have received assistance from the program within a 3-year period. Payments are made directly to the landlord.
Homeless Prevention Programs
  • Prevention Services: Temporary financial assistance and/or supportive services to help people remain in their current housing situation, relocate, or divert them from entering the homeless system. Financial assistance may help with rental application fees, the first month's rent, past due rent, or utility assistance.
  • Rapid-Rehousing: Assists households to quickly exit homelessness and return to permanent housing. Provides one-time rental assistance and supportive services such as case management and housing navigation.
Homeownership Down Payment Assistance
  • Assistance under this program may include payment of reasonable loan down payments, closing, and/or costs generally referred to as settlement charges associated with the first mortgage of the home to be used as the family's primary residence. Maximum amount of assistance is $3,000 including down payment, closing, and settlement costs.

Funds will vary on a monthly basis. The Prevention Assistance fund assists families and individuals facing eviction with back-rent financial assistance and landlord-tenant negotiation assistance. Case management includes services such as transportation assistance, information/referral, and landlord tenant law training with the goal of achieving housing stability moving forward. Rapid Re-Housing (RRH): Provides short-term rent assistance and/or deposit for individuals and families with children who are homeless to get into permanent affordable housing where the participant is the leaseholder and chooses their own apartment according to their needs. RRH includes case management services such as life skills training, transportation assistance, information/referral, landlord tenant law training, and utility assistance for up to six months with the goal of obtaining and subsequently maintaining stable housing. Households will be assessed and prioritized using the Coordinated Entry System. Long-Term Assistance: The Permanent Supportive Housing (PSH) Program provides high-barrier households experiencing homelessness with affordable rental housing and supportive services that assist participants with maintaining housing stability, improving overall wellness, engaging in the community and leading successful lives. PSH targets households that need intensive case management services due to challenges such as significant and persistent mental health issues and chemical dependency. Households will be assessed and prioritized using the Coordinated Entry System. Note: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Permanent Supportive Housing serves homeless individuals with a documented disability. Participants receive a rental subsidy and accompanying support services. PSH is offered in both Beltrami and Cass Counties.
Offers on-site permanent supportive housing to 13 single parents with three or more children, as well as scattered site family housing in the community. Parents partner with case managers and support staff to develop their own goals and find resources to achieve them. Kids ages birth - 18 benefit from age-appropriate social, emotional, and academic program offerings. Support services available to housing clients include:

Permanent, Supportive Housing (Maxfield Place): Permanent, supportive housing program for adults with a diagnosed disability who are considered chronically homeless. There are 10 individual apartments for single adults which include a bed, full kitchen, and bathroom. Maxfield Place is staffed 24 hous/7 days a week and employs a caseworker, who helps residents develop a long-term plan and collaborates with other service providers for referrals.
