24-unit permanent supportive housing program for single individuals experiencing chronic homelessness - 4 units reserved for veterans - 2 units are ADA accessible - Each unit has a microwave, small fridge, and an in-unit bathroom - Free onsite laundry, a full communal kitchen and shared living spaces - On-site case management and a 24/7 staffed front desk
Based on individualized service plans, tenants can receive the following services: Crisis intervention, parenting education, individual and group counseling and therapy, job readiness and retention skills training, chemical health support and therapy, and life skills training. Community-building activities are scheduled on a monthly basis to provide education as well as develop a sense of community among the tenants. These activities include on-site sessions as well as off-site outings. Provides homeless, mentally ill single parents with permanent, affordable, chemically-free housing so that these families can move towards stability.
Saint Francis Apartments provides housing and supportive services for seniors. Services include: - 32 units to people experiencing chronic homelessness who qualify for the Department of Housing and Urban Development's (HUD) project-based permanent supportive housing (PSH) subsidy vouchers - 10 units are offered to those experiencing long-term homelessness. These units are for people who have the means to pay market rate rent or who qualify for Housing with Support (HS) subsidies as per Saint Louis County

Additionally, emphasizes on fathers and father-figures facing challenges not only in securing housing, but also in overcoming obstacles related to parenting.

Owns and operates a number of permanent housing units throughout Minnesota. Duluth, Mankato, and Saint Cloud offer permanent supportive housing and affordable, independent permanent housing in Minneapolis, Saint Paul, Saint Cloud, Duluth, and Rochester. These units consist of efficiencies, one-bedroom, two-bedrooms, and Accessory Dwelling Units (ADUs) which are similar to a tiny home. Tenant screening criteria can be more lenient than community landlords.

There are 10 individual apartments for single adults which include a bed, full kitchen, and bathroom. Maxfield Place is staffed 24 hous/7 days a week and employs a caseworker, who helps residents develop a long-term plan and collaborates with other service providers for referrals. Coordinated Entry Services (CES): The Housing Advocacy Services Department participates in the Southeast Minnesota River Valleys Continuum of Care Coordinated Entry Services (CES) for housing and support services for households experiencing homelessness Prevention Assistance: When funding is available, individuals and families will be referred to the Housing Advocacy Department for follow up on Prevention Assistance. Funds will vary on a monthly basis. The Prevention Assistance fund assists families and individuals facing eviction with back-rent financial assistance and landlord-tenant negotiation assistance. Case management includes services such as transportation assistance, information/referral, and landlord tenant law training with the goal of achieving housing stability moving forward. Rapid Re-Housing (RRH): Provides short-term rent assistance and/or deposit for individuals and families with children who are homeless to get into permanent affordable housing where the participant is the leaseholder and chooses their own apartment according to their needs. RRH includes case management services such as life skills training, transportation assistance, information/referral, landlord tenant law training, and utility assistance for up to six months with the goal of obtaining and subsequently maintaining stable housing. Households will be assessed and prioritized using the Coordinated Entry System. Long-Term Assistance: The Permanent Supportive Housing (PSH) Program provides high-barrier households experiencing homelessness with affordable rental housing and supportive services that assist participants with maintaining housing stability, improving overall wellness, engaging in the community and leading successful lives. PSH targets households that need intensive case management services due to challenges such as significant and persistent mental health issues and chemical dependency. Households will be assessed and prioritized using the Coordinated Entry System. Note: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP and others.
